The Valley Area Agency on Aging is a non-profit agency serving persons age 60 and older, and/or disabled individuals in Genesee, Lapeer and Shiawassee Counties. VAAA is able to assist you with the following services: Information and Assistance: If you have a question, this is a good place to start. Our Information Specialists can provide resources on services and programs that can work for you. MI Choice Waiver Program: The program is designed to keep you and your loved ones living independently at home while receiving the services needed. A team consisting of a Nurse and Social Worker will help determine the services that are needed. Care Management: Those eligible for the Care Management Program are assessed by a Social Worker and Nurse to create a plan of care. Through the care plan, services can be arranged to work with the participant and family depending on the needs of the individual. KISS Program: The KISS (Keeping Independent Seniors Safe) program provides a telephone call every day to ensure the senior is safe. PACE may be able to assist those struggling to maintain their independence in their home environment if they are: 6 Age 55 or older 6 Living in Genesee County 6 Certified as meeting Michigan criteria for nursing home level of care 6 Able to live safely in the community with the support of PACE services Genesys was designated by the State of Michigan as the exclusive PACE provider for Genesee County. Neglect The abuse of older adults can take many forms and represents a serious, under reported problem in every community Neglect is the harm to an adult’s health or welfare that is the result of the adult’s inability to respond to a harmful situation [self neglect] or the failure of a person who in the Adult Protective Services Act (Public Act 519 of 1982) as the harm or threatened harm to an adult’s health or welfare caused by another. The term “elder abuse” includes physical abuse, emotional aspect of the adult’s health or welfare to respond to a harmful situation or to provide adequate food, clothing, shelter, or medical care [caregiver neglect]. An example of caregiver neglect would be leaving an elderly person who is in a wheelchair due to a stroke in front of the TV all day without meals or assistance in getting to the bathroom. exploitation. The authority you give your agent can be as broad or narrow as you choose. Wills Preparing a Will is the simplest way to make sure that your assets (real and personal property) will be distributed according to your wishes. A Will is a public document. However, did you know that if a person dies with assets in their name without a co-owner or beneficiary, then their Last Will and Testament will go through probate court? Probate is the legal process for proving and administering a Will. The probate process can be very time consuming and expensive and is always public. A Will names heirs and a personal representative, the person(s) who manages and distributes the assets according to your wishes after your death. This is often a family member or friend. Sometimes the personal representative will have to post a bond to perform their duties. Last, unhappy family members can object to the Will – we often call this process the “family feud.” The terms of the trust are limited only by your imagination. For example: you can provide for the future of your special needs loved one without affecting their governmental benefits; providing for education of a child or grandchild and make sure your assets remain in the bloodline. Typically you are the Trustee, the manager of your Living Trust, just as you are the manager of your assets now, nothing changes. The “revocable” aspect of a Living Trust means you are in control. It means once you create it, you can always change it. You can revoke it or amend all or part of it. The “living” aspect means that it is created and funded (assets transferred to you as trustee) while you are alive, and provides for your benefit during your lifetime and upon your disability, if drafted properly. Medicaid is available only to certain lowincome individuals and families who fit into an eligibility group that is recognized by federal and state law. Medicaid does not pay money to you; instead, it sends payments directly to your health care providers. Depending on your state’s rules, you may also be asked to pay a small part of the cost (co-payment) for some medical services. What is not covered? Medicaid does not provide medical assistance for all poor persons. Even under the broadest provisions of the Federal statute (except for emergency services for certain persons), the Medicaid program does not provide health care services, even for very poor persons, unless they are in one of the designated eligibility groups. Low income is only one test for Medicaid eligibility; assets and resources are also tested against established thresholds. As noted earlier, categorically needy persons who are eligible for Medicaid may or may not also receive cash assistance from the TANF program or from the SSI program. Medically needy persons who would be categorically eligible except for income or assets may become eligible for Medicaid solely because of excessive medical expenses. Medicaid is a state administered program and each state sets its own guidelines regarding eligibility and services. Many groups of people are covered by Medicaid. Even within these groups, though, certain requirements must be met. These may include your age, whether you are pregnant, disabled, blind, or aged; your income and resources (like bank accounts, real property, or other items that can be sold for cash); and whether you are a U.S. citizen or a lawfully admitted immigrant. The rules for counting your income and resources vary from state to state and from group to group. There are special rules for those who live in nursing homes and for disabled children living at home. Medicare is health insurance for people age 65 or older, under age 65 with certain disabilities, and any age with permanent kidney failure (called “End-Stage Renal Disease”). Following is a description of the choices and then a listing of local facilities. Keep in mind that the services and amenities offered vary from facility to facility, even within the same group. For example, in independent living facilities, some of them provide extra services like meals or activities, and some do not. Another example is in assisted living facilities, some will accommodate residents in wheelchairs, and some will not. Of course it is always best to thoroughly investigate and compare facilities and ask a lot of questions, in order to match the resident to an appropriate facility. Independent Living Facilities: These can be government subsidized for people with limited income and assets, or non-subsidized for anyone. They can vary from individual apartments with no additional services, to facilities that provide all meals and activities, and everything in between. Usually there would not be 24 hour supervision, although home care agencies can be contracted to provide extra care as needed for personal care, medications, food, or companionship. Assisted Living Facilities: These facilities usually have between six to 40 residents and includes 24-hour supervision of the residents. These facilities would offer help with activities of daily living, feeding, dressing, personal care, medications and so forth. These facilities would take care of all the resident’s needs up until the time they would need a nursing home. Memory Impaired Facilities: These are like assisted living facilities that specialize in residents who are afflicted with dementia and Alzheimer’s disease. There should be close supervision, and there may be locked or alarmed doors for the resident’s safety. Nursing Homes: These would serve patients who need constant skilled nursing services that are unable to be provided at an assisted living or foster care facility. Also nursing homes can provide rehabilitation for patient’s injuries with the hope of returning home or to an independent living or assisted living facility. These are the categories we have listed in this directory, but in fact there is a lot of overlap of services and different levels of care offered from one facility to another.
