Torsio uteri VET Lichtegg – Dr. Franz Schlederer
EFB 2013
Types of torsio uteri •  Around 90% le>side torsio •  Rightside torsio is rare. •  Front-‐end und back-‐end calf posiDons . •  RotaDon of the uterus mostly from 180°-‐270°, 90° •  RotaDon is o>en not recognized by the farmers. VET Lichtegg – Dr. Franz Schlederer
EFB 2013
Methods of uterus retorsion •  Manually •  With Instruments -‐ Caemmerer´s torsion fork/detorsion bar -‐ Kalchschmidt‘s calving hook -‐ GYN-‐sDck (new methode!) •  Indirectly -‐ barmethod (video) •  SecDo with retorsion VET Lichtegg – Dr. Franz Schlederer

GYNsEck® by Dr. Schlederer THE „BIRTH“ of the IDEA: ….twisted uterus „in back-‐end posiDon“….. „I wasn´t strong enough to do a manual retorsion……no help to do the „bar-‐method“….. I had to improvise“ VET Lichtegg – Dr. Franz Schlederer

The real clue: A CombinaEon tool •  GYN-‐sDck unites exisDng systems like torsio fork -‐ Caemmerer and Kühn´s obstetrical crutch in one tool. •  BUT it is much more than this: •  elasEc material aspect – came up as an accidental result of my cooperaDon with the school of technology in my home town which is specialised in plasDc materials. OperaEonal procedure GYN-‐sEck ® VET Lichtegg – Dr. Franz Schlederer
EFB 2013
Advantages of GYN-‐sEck® •  Shorter duraEon of obstetric intervenDons •  Gentle handling: the cow is likely to keep standing (only liIle abdominal press) •  Easy Controlling of retorsion •  Tissue -‐ safer than exisDng tools (plasDc, design, legs are fixed) •  ReposiDon of a torsio uteri in a lying cow is possible with the GynsDck VET Lichtegg – Dr. Franz Schlederer
EFB 2013
Further advantages •  Fixed birth ropes on the legs (compared to tradiKonal systems where the cuLs oLen glide from the fork and can damage Kssues) •  A>er the rotaDon the fixed ropes can be used immediately for pulling out the calf. No need for changing ropes (less exhausKng for vet and animal). VET Lichtegg – Dr. Franz Schlederer

The cow is lying dead calf, torsio fricEon movement VET Lichtegg – Dr. Franz Schlederer
EFB 2013
Old system – Caemmerer´s Fork exhausEng handling…and many risks… VET Lichtegg – Dr. Franz Schlederer
EFB 2013
„Torsional moment“ •  When we start rotaDng the material takes up energy of the human force which is exposed immediately when the calf gets turning. (the sDck gives a „kick“) -‐> easier and effort-‐saving turning. •  Physical force on the calf-‐extremiDes is reduced significantly. Another advantage is that the material of the ropes is elasDc (compared to chains). VET Lichtegg – Dr. Franz Schlederer
EFB 2013
ReposiEon of PosiEonal Anomalies •  Lower and anterior posiDon (in front-‐end or back-‐end) •  Breech presentaDon: Kühn´s obstetrical crutch à bujocks pushed cranial and so the extremiDes can be changed to caudal more easily. •  Pushing back the fetus by using the neck chest sling (so called „ski jumper posiDon“) •  CorrecDon of the posiDon of the calf´s legs by using the new sling VET Lichtegg – Dr. Franz Schlederer
EFB 2013
„Ski Jumper“ PosiEon VET Lichtegg – Dr. Franz Schlederer
One rope system EFB 2013
Summary of applicaEons •  Torsio uteri dexter et sinister •  ReposiDon of posiDonal anomalies •  PosiDoning the calf for secDo (bejer posiDoning for an easier surgery) •  Hydrops aszites and „gas-‐calf“ (dead, emphysematous) VET Lichtegg – Dr. Franz Schlederer

Dr. Schlederer`s eyehook for calves and foals Smooth to the calf´s orbita, safer and easier to handle VET Lichtegg – Dr. Franz Schlederer

pulling of the
head the
compression of
the muscles and
skin of the neck
can be avoided.“

EYE-‐hook „the ergonomic design“ VET Lichtegg – Dr. Franz Schlederer

EYE-‐hook – elasEc fixaEon Two ways of applying the rope The ergonomic design of the hook leads to a good holding in the orbita
