The Center’s mission is to create a caring and supportive environment committed to the highest standards of quality health care. We lost our revered leader, Gordon (Skip) Hargraves in April, after his courageous battle with cancer. Skip was completely engaged in the business of this institution, right to the end. His seven years of service as President were stamped with his dedication and vision for the next chapter in our 147-year history: completing the plans to build The Uplands, a desperately needed continuing care retirement community (CCRC). As of this writing, we have submitted the draft of the Environmental Impact Statement, and we await the completion of the "findings”. We are confident that the findings report will validate the responsible care, diligence, and financial discipline we have invested over the last eleven years. It is our hope that ground breaking will occur in 2014. There are presently only three CCRC's on Long Island to serve an aging population of 2.5 million seniors. The need is clear. As a non-profit facility, St. Johnland Nursing Center serves the local and broader communities with nursing care, rehabilitation services, the traumatic brain injury program, comprehensive dementia care (one of the few dedicated units on Long Island) medical and social model day care programs and home care. We have met the challenges of evolving medical insurance and reimbursement legislation, staying true to the high standards of our mission. We count on the generosity of our friends and donors to ensure continuing the outstanding reputation we have built in our long history. While 2012 was a strong year, thanks in large measure to your on-going support, health care is at a critical crossroads and we shall need to rely on private, corporate and foundation gifts to sustain many programs. Please note that we have a Planned Giving program, with an array of ways to provide St. Johnland with resources while you receive an annual income. St Johnland is operated by an outstanding and committed staff, overseen by the volunteer Board of Directors. We are charged with maintaining a viable institution, both in care and with fiscal prudence. We take that charge very seriously. It should be noted that during the aftermath of Hurricane Sandy, many staffers stayed in the Nursing Center without complaint to help care for our residents as well as new patients evacuated from other facilities. Heartfelt thanks go to the employees of St. Johnland, as well as the board, for their support during my first year as President. Calabrese 2 LETTER FROM THE CEO As we reflect on 2012, it was a challenging one for every health care provider: increasing expenses, declining income, and significant regulatory changes in reimbursement methodology on both the State and Federal level. It causes all of us to review our resources and establish ways to work more efficiently as we continually look for innovative strategies to provide outstanding care to our residents. Each day approximately 370 people receive care from our dedicated staff at St. Johnland. S ome are residents who experience long-term stays in the skilled nursing f acility or the Alzheimer’s/dementia unit. Others have temporary stays in the Head Injur y Rehabilitation or Subacute units. Still others visit daily to attend the Adult Day Health Service or Social Adult Day Programs while many are served in their homes by the Long Term Home Health Program. St. Johnland’s vision for the future includes the development of a continuing care retirement community, which you can read about in this report. We look forward to providing this independent living option to our neighbors. The impetus is not so different from the vision our founder, Dr. William Muhlenberg, had ove r 147 years ago—t o establish essential programs that provide all the services needed in the community we serve. In addition to a comprehensive range of clinical care, we offer gourmet dining, with customized menus for each resident and healthy and nutritious choices daily. Our on-site medical staff ensures that residents receive the finest care in a swift and professional manner. Recreational activities and programs are diverse and range from group activities to encouraging residents to continue with former interests and hobbies. In 1866, William Augustus Muhlenberg, D.D., an educator, minister, and a prominent social innovator, raised the necessary funds to purchase 500 acres of woodlands and fields on the north shore of western Suffolk County, now known as Kings Park. Over the years, the original purpose of caring for the needy, young and old, was maintained. In the early 1950s, the Board resolved to specialize in care for the elderly. From that time until the mid-1970s, approximately 90 residents at any given time were cared for at St. Johnland. In the early 70s, the Board recognized the need to streamline the care by having one building as well as the need to upgrade the facility. Just after Christmas in 1975, the residents moved into the new and larger facility which is currently in use. Since that time, additional services have been added to the skilled nursing facility. In the 1990s, a Head Injury Rehabilitation Unit and an Alzheimer’s / Dementia Unit were added. A subacute care program was added in 2004. St. Johnland also provides services to those living at home in the community through adult day and home health care programs. Currently, St. Johnland accommodates more than 350 people each day with these services. The original philosophy of caring for the needy continues. Dr. William Muhlenberg, an Episcopal minister, educator and social innovator from New York City, purchases 500+ acres and begins building St. Johnland as a self-supporting industrial village, complete with residences, schoolhouse and gymnasium 1866 Women live at St. Johnland and the Babies’ Shelter opens Joan R. The initial planning stage to bring The Uplands at St. Johnland, a premier Continuing Care Retirement Community, is close to completion and we expect the development stage will begin in 2014. We are dedicated to creating an attractive, gracious community that will include townhouses, spacious apartment homes and common spaces to rival some of the more popular resorts. We are eager to share our plans for this new retirement lifestyle to our area as soon as they solidify. 6 The Uplands will enable individuals age 62+ to have peace of mind by providing a retirement setting offering a full range of life-care options, including independent living, assisted living and skilled nursing, all on the St. Johnland campus. If residents experience financial setbacks, they will be able to remain at The Uplands with full care. Aside from offering a much-needed residential option for local seniors, The Uplands will bring a broad array of benefits to our local community: • The Uplands will be built on property owned by St. Johnland for nearly 150 years, allowing us to pass along a savings to the residents. • The Uplands will allow individuals to age in place, first in independent living, then in assisted living and skilled nursing, if needed. • Planned with a sensitivity for conservation, The Uplands will be a beautiful complex built to fit into the natural landscape of the surrounding campus, maintaining 50% of the existing natural habitat The setting will be well buffered from the road, with a 200-foot setback without disturbing the natural topography or wildlife. • The Uplands will provide life care, with a full array of services and activities to ensure all residents will enjoy their senior years with independence and dignity, in a safe and caring environment. Presently, there are only three CCRCs for all of Long Island serving a population of more than 7.5 million people. The need is clear. St. Johnland has always been committed to providing outstanding healthcare services to the community. The Uplands will meet the need for senior retirement housing in a safe and attractive setting. 7 Remembering Skip Hargraves, President: 2000 to 2012 Spoken by members the Board of St. Johnland Nursing Center St. Johnland Board Meeting April 25, 2012 Skip’s goal was to be fully engaged right up to the end. Even when his energy flagged, he remained stalwart and resolute in how we were to conduct ourselves and the business at hand. Skip was always connected and available. His leadership style was utterly engaged with St. Johnland. He was a man of strong opinions, but also possessed the capacity to listen to others’ points of view. Many were the times we did not share the same view, but he always heard the other side. ß Along with indomitable courage came healthy doses of humor, intelligence, dignity, compassion for others, and interest in all that was around him. He enjoyed being quirky and catching one off balance. Skip was a fine mentor, and it was my privilege to learn from him, often by example. ß Skip’s enthusiasm and commitment for St. Johnland’s future were unwavering. His leadership will long be remembered. ß Skip was an inspiration to all who were involved with him over these years and embodied the lesson to live every day to the fullest. All of us will remember “Sir Skip” Hargraves. ß Skip maintained a strong level of competition and the admiration and appreciation of the good friends with whom he worked and played tennis. ß Skip was a lovely man, a good friend, a fighter, always upbeat and will be missed deeply. ß From his early days on the St. Johnland board, Skip was an eager participant and had indicated his interest in becoming President. Whenever I would meet him, he would ask: “Is it my time yet?” And when it was, he was committed. ß Skip contributed his all in everything he did; he approached every task with great enthusiasm and just the right touch of good humor. St. Johnland was the apple of his eye and we will miss his inspired leadership. ß Skip was a no nonsense character - just my kind of guy! He was a great mentor, a very thoughtful colleague, a fun guy and a wonderful friend. ß Skip was truly an inspiration to me with the way he remained up-beat and fully engaged and never losing his great sense of humor. ß It was a great pleasure to know and work with Skip. He had a direct, reasoned and logical approach to challenges and he had great insight into human nature as well as business matters. How to Be an Informed Advocate for Young Athletes”, a discussion with neurologist Dr. Max Rudansky. 9th Annual Advisory Board Symposium “Follow Your Genes: Decision Making and Your Personal Genome”—a panel discussion with nationally recognized speakers on the latest research and clinical applications.
