To receive your Mail-In Rebate, follow these instructions:
1. Complete all information requested on this offer form.
2. Circle the qualifying dbx product purchase and the purchase
date on your receipt or invoice. Your receipt/invoice
must be dated between November 1st, 2011 and
December 31st, 2011.
Photocopies of the receipt/invoice are acceptable.
3. Remove the original UPC from the dbx product package. Photocopies of the UPC will not be accepted.
4. Mail this completed offer form, a copy of the receipt/
invoice and the original UPC in a single envelope to the
address below. Submissions must be received no later than 45 days after
purchase date to qualify.
