Please fill in this order form and hand it in to your school with your payment of
£7.00 (payments should be made to your school by the date stated by your school
in the letter that you received with this leaflet). If paying with cash please enclose the
correct amount as change will not be given.Your Sun Bag will be delivered to school,
to be kept on school premises for use during school hours.
If you have more than 1 child at school, please fill in a separate form for each child.
Please note that your school will order the correct colour hat for their uniform.
