Hotel Accommodations
The University of Chicago Kovler Diabetes
center will cover the cost of accommodations
for families coming from out of town for the
night of Wednesday, July 17, 2013 at the Marriott
Downtown Magnificent Mile For overnight hotel
accommodations for Thursday, July 18, 2013 and
Friday, July 19, 2013, the Kovler Diabetes Center
has reserved a block of hotel rooms at the Marriott
Downtown Magnificent Mile with a special group
rate of $159 per night (standard room rate) and
will be the responsibility of each family attending
the conference.
Any and all incidental charges, tipping, room
service and other hotel room services throughout
your stay in Chicago are also the responsibility of
forum attendees. Guests will be asked for a credit
card upon arrival for incidentals.

Parking
If you plan to drive from your home to Chicago,
valet parking is available during the duration of the
Conference July 17 - 20 for a fee and self-park is included at no cost. However, once you have parked
your vehicle you may not leave the garage, as the
voucher does not include in-and-out privileges.
We DO NOT encourage families to rent a car if
flying to Chicago, as public transportation and taxicabs are readily available to and from the Chicago
Marriott Downtown Magnificent Mile. Taxicabs
and public transportation are to be covered by
all attendees independently. The average cost of
a taxicab from Midway or O’Hare to the Chicago

Photography, Film & Publicity
Video Diary
A video diary room will be available at the conference where you may
film you or your family’s personal journey with Monogenic/Neonatal
Diabetes. We hope that you will contribute to the video diary, but this opportunity is entirely voluntary. Video diary appointments can be made
during the online registration (KovlerDiabetesCenter.org/BeInspired)
or at the Conference Registration desk during the Family Forum. Filming will take place in a private setting on the fourth floor of the Chicago
Marriott. We will schedule sessions with all families interested in participating. Please indicate on the Registration Form (at KovlerDiabetesCenter.org/BeInspired) if you are interested in participating so that we can
accommodate all families who wish to volunteer.
Photography and Film
We would like to photograph and film some individuals attending particular parts of the meeting. We will only include you and your family if
you are happy to take part. We are thrilled to welcome a documentary
filmmaker at the meeting, who will film some of the event for a documentary about Monogenic Diabetes. Please indicate on the Registration
Form (at KovlerDiabetesCenter.org/BeInspired) if you are interested in
participating. Additional release forms will be available at the meeting.
Publicity
Newspaper, television, radio and other media representatives may be
present and may wish to interview families about Monogenic Diabetes.

Child Care
Children of all ages are welcome to attend the conference. If you would like to attend sessions
without your children, you may wish to use the optional child care sessions that we are providing
at no cost. We are happy to take care of children who have diabetes as well as those who do not
have diabetes.
Child care sessions will be managed by our experienced team, including nurses, dieticians, child
life specialists, certified diabetes educators and experienced volunteers. Supervision provided by
our team will include play time, snack time, and organized activities such as board games, coloring, reading, DVDs, etc. Children are also welcome to bring their own toys and personal favorites.
Be sure to label your items or toys properly.
For all child care sessions, the caregiver to child ratios will be: 1:1 for children < 1 year, 1:2 for
children aged 1 – 2 years, 1:3 for 3 –7 years and 1:5 for those > 7 years. Each family will need to
complete the child care registration form. In addition, each child will need to be signed in and
out of these sessions by his/her parent or primary caregiver. After completing and submitting the
online child care form, you will receive a child care consent form in addition to film, photography
and media consent forms. If you have any questions regarding child care, please contact Peggy
Hasenauer at Diabetes@UChospitals.edu or by calling 773.834.4789.
Important Points for Families
• Child care services will be offered on Thursday and Friday, July 18 and 19 from 8:30 am – 4 pm.
• The child care room is located on the third floor of the Chicago Marriott, just down the hall from the
main conference space. Parents are welcome to visit the child care room throughout the day.
• If parents or primary caregivers are leaving the Chicago Marriott building during the conference for
any reason during child care hours, please notify child care staff prior to leaving the Chicago Marriott
and verify contact information.
• Please bring all important contact information with you, including names and phone numbers of your
primary care physician, pediatrician, family practice physician and your local pharmacy.
• We encourage all parents to eat breakfast and lunch with their children on Thursday and Friday in
the main conference space. Gluten-free, low-fat, healthy snacks will be offered to children in the child
care room mid-morning and mid-afternoon unless you specify that your child have his/her
own snacks. (Exact meal and snack menus will be provided to families upon request.)
• We will ONLY check blood sugars or administer medications on an emergency basis. In all other cases,
parents should bring all necessary medications and supplies to the conference and administer blood
sugar checks and/or all medications to children directly.
• We will contact parents for diaper changes as needed, as this is a service we will not be providing. If we
have your permission to escort your child to the bathroom, please include this on the online child care
registration fields.
• Child care will not be provided Wednesday or Friday evenings, but family-friendly activities will be
offered. Complimentary child care will be available on Thursday evening.
• Please ensure that all personal toys, books and games are clearly labeled with the child’s name and
family name.
• We will ask that all children with food allergy wear a special bracelet we design for them.

Kovler Mission & Vision
Caring for individuals with diabetes throughout their lifespan—from infants
to adults—is our mission at The University of Chicago Kovler Diabetes
Center. We pursue our mission through the following:
Diagnosis
Comprehensive, complete and accurate diagnosis, including genetic
testing capabilities.
Treatment
Each treatment plan is highly individualized to meet each patient’s
specific needs. We use the most effective and innovative medications and
technologies to stabilize blood sugar and minimize long-term complications.
Complications
Our multidisciplinary approach addresses the many ways that diabetes can
affect a person’s physical and emotional well-being.
Prevention
Preventing diabetes before it begins is the most effective intervention.
Research programs at the Kovler Diabetes Center target people at high risk
for diabetes, including African-Americans and Hispanics.
Education
Through one-on-one training and group classes recognized by the American
Diabetes Association, we teach all our patients about everything from
monitoring blood glucose, to healthy eating, to using insulin pumps and
glucose meters.
