ELIGIBILITY: Entries are open to five-person teams (men, women and mixed teams) consisting of FOUR AMATEURS and ONE PROFESSIONAL or
DESIGNATED PROFESSIONAL. Teams must represent a WSGA or PNGA member golf club or a business. Amateurs must have a USGA Handicap
Index or RCGA Handicap Factor, with the limit for men at 36.4 and women 40.4. There is no age limit.
FORMAT: Team with the lowest net team score, using three best net scores of the five players on each hole, will have their names engraved on
the crystal Evans Cup perpetual trophy.
PRIZES: Amateurs play for merchandise certificates and professionals play for cash prizes.
ENTRY FEE: $400 per each amateur entrant and $150 per each professional entrant ($1,750 per team). NOTE: A club may choose their team
or teams in any manner they wish. A team entry fee of $1,750 must be paid by every team. Single entries will be accepted, but filled on an as
needed basis. Submission of a single entry does not guarantee you a place on any specific team. ENTRY MUST ARRIVE AT THE WSGA OFFICE BY
4:30 P.M. ON MONDAY August 22 , 2011.

HOSTED BRUNCH: A hosted contestants’ brunch at the club will be ongoing from 10:00 – 11:45 a.m.
HOSTED COCKTAILS/BANQUET/AWARDS CEREMONY/AUCTIONS: Hosted cocktails and a silent auction will begin immediately after play. Hosted
contestants’ banquet, awards presentation ceremony, and a live auction will begin at approximately 6:00 p.m. Tickets for guests may be purchased in advance for $60 (includes cocktails and dinner). Please indicate the number of banquet reservations on the entry form and remit payment for all guests along with entry fee.
DRESS CODE: Appropriate golf attire is required while on club property. Appropriate attire does not consist of short shorts, tanktops, t-shirts,
sweatpants, spandex or unkempt clothing of any kind. Jeans or denim of any kind or color or golf sandals may not be worn. Bermuda length
shorts, which must reach at least mid-thigh, may be worn. For men, all shirts must have collars. For women, all shirts/blouses must have either
collars or sleeves. The Evans Cup of Washington is a spikeless event. Please wear the appropriate footwear. NOTE: For the cocktail reception/
silent auction and banquet, it is requested that men wear a coat and tie and women wear appropriate dinner attire.
CARTS: Each team will receive two complimentary automotive carts.
ADDITIONAL MISCELLANEOUS: Complimentary warm-up range balls, special tee prizes, on course prizes, on course beverages, and much more
will be included throughout the day.
After an entry form has been received, a confirmation of entry, with additional information about the day’s activities, directions to the club, list of
auction items, etc., will be sent to each member of each team.
