Patient Transportation Gurney Instruction Sheet - Angio-Plex

RE: Patients requiring gurney transportation
Dear Transport Drivers:
We are a new location servicing patients with dialysis access problems, and would
like to provide you with assistance in transporting patients to our site. California
CardioVascular Institute is located on the first floor of a residential complex. The
building's management requests that we do not bring gurneys through the lobby
area. Please enter through the single glass door on the side of the building facing
Wilshire Blvd. It is located just before the bus stop on the corner of Wilshire and
Bixel. The glass door is marked with our logo above.
If further assistance is needed, please contact our receptionist for additional
directions, alternate parking locations, and alternate routes to enter our center
within the main building. Our address is 1100 Wilshire Blvd., Suite 101,
Los Angeles, CA 90017 and our number is (213) 223-1100.

From the Harbor Freeway (110) going South:
Take the Wilshire Blvd exit -7 Turn right onto Wilshire Blvd -7 Proceed two blocks to Bixel St and turn Left -7 Proceed
one block to Ingraham St. (between 7th St & Wilshire Blvd) and turn Right -7 Entrance to the parking structure is on the
Right. Look for "Visitor's Parking". Take parking ticket and proceed to elevator up to the first floor. Proceed into the
lobby area and we are located just past the Concierge in Suite 101.

Take the 9th St Exit -7 Proceed a short distance to Figueroa St and turn Left -7 Proceed to Wilshire Blvd and turn Left -7
Turn Left onto Bixel St -7 Take the first Right onto Ingraham St (between th St & Wilshire Blvd) -7 Entrance to the
parking structure is on the Right. Look for "Visitor's Parking". Take parking ticket and proceed to elevator up to the first
