Dr. Petros Koidis Dear colleagues, The Balkan Stomatological Society carries on an intense activity in the scientific field, favouring the authentic values, the real standards and the practical impact of the on going research. The binominal scientific research- practice correlated with the interdisciplinarity field governs the basic goals of this prestigious society. The fact that each year the BaSS congress takes place in a different country is not accidental, but it represents the result of a competition, materializing in a real opportunity of knowing new specialists from each clinical entity of dental medicine within the Balkan space. This year, the Balkan congress takes place in Romania, a hospitable country, with a millenary history, where culture and spirit get together. In the same time, we must discover dental medicine in Romania as meeting the European standards as well as those who practise it authentic sources for new collaborations. We are sure that this year, too, the Balkan congress wil mark another important step in the scientific evolution of this society, reuniting a fine record of scientific certainties, as well as fine memories of the human contacts made here. Popa“, U.M.Ph, Iaşi, Romania President of Romanian Society of Oral Rehabilitation Dear colleagues, We need effective models to be professional. Professional performance needs direct access to the European requirements, standards and innovations. The 16th Edition of the Balkan Society of Stomatology Congress reunites after 10 years the Balkan Elite (represented by the Executive Committee of BASS and the counselors of the Balkan countries: Greece, Cyprus, Turkey, Serbia, Bulgaria, Albania, Croatia, Slovenia, Bosnia and Romania) as well as outstanding names of the world dental medicine. The event is to take place in Bucharest between April, 28 and May, 1, 2011. This edition novelty is the merging of General Medicine and contemporary Dental Medicine domains to provide a unique complex approach of the oral pathology patients. Over 1000 Balkan participants will be actively involved in Conferences, Round Tables, Scientific Works, and they will participate at the Exhibition of Dental Medical Devices and Materials “BaSS Expo 2011” as well as at the Symposiums where Hands-On are to be distributed. The organizational support provided by ICOI (the International Congress of Oral Implantologists) and by the Romanian Academy is a warrant of the high practical and academic standards that must characterize a scientific event. The Romanian Society of Oral Rehabilitation and specialized Romanian organizations are also bringing their contribution to the success of the most important Congress of Dental Medicine organized in Romania in the past few years. Dr. Andrei ILIESCU "Carol Davila" U.M.Ph., Bucharest, Romania The 16th Congress of Balkan Stomatological Society takes place in Romania for the second time since its creation, which is a real privilege for us, that of hosting this prestigious event reuniting prestigious names of international dental medicine. The scientific subject of the congress is Update in Dental Medicine which is a real challenge to everything that is new, to self evaluation and to a constant connection to standards. It is impossible to reach the performance level in a complex field such as dental medicine, without an yearly update of our knowledge in the company of famous names of contemporary medicine. The confluence of General Medicine with Dental Medicine completes the concept of rehabilitation, providing it the real values for a practical materialization. The mixed sections of oral communications create some authentic pre-requisites of interdisciplinarity, the actual beneficiary being the patient who will benefit of an election approach from an integrative point of view. The great number of oral presentations and posters, the variety of the approached subjects, the diversity of theme conferences constitute some obvious motivations for a real future success of this congress; each participant will leave after its end professionally and spiritually enriched. From 1985, he was an associate professor at the Department of Oral Function and Prosthetic Dentistry of the Radboud University, Nijmegen, The Netherlands. Topics of specific interest are: geriatric dentistry, prosthetic dentistry, maxillofacial prosthetics, oral implantology, tooth wear, and halitosis. A PhD-study on geriatric dentistry and prosthetic dentistry was finished in 1990. As from 1999, he is Professor in Geriatric Dentistry at the Radboud University, Nijmegen, The Netherlands. Professor De Baat is (co-)author of several publications in peer reviewed journals and books and professional journals, and he is editor of several textbooks. This demographic shift will have important implications for health care services. More (frail) older people will present more morbidity and care dependency and, consequently, will need an increasing proportion of health care services. Those older people, who are not able to function independently, are often supported by domiciliary care service or admitted to care homes. Advances in oral health care and treatment during the last decades have resulted in a reduced number of edentulous individuals. A still increasing number of dentate older people have tooth wear, periodontal disease, oral implants, and sophisticated tooth- and implant-supported restorations and prostheses. Hence, they are in need of both preventive and curative oral health care continuously. Complexity of the oral conditions, oral mucosal lesions, systemic diseases, and medication use make (frail) older people more vulnerable to oral problems than younger age groups, in particular when they are cognitively impaired. Weakened oral health due to neglect of self care and professional care and due to reduced oral health care utilization is already present when (cognitively impaired) older people are still community-dwelling. At the moment of care home admittance, many older people in countries all over the world are in need of oral health care urgently. If their needs are not met, their oral health will be persistently poor and will utmost probably further deteriorate during their residency because of increasing care dependency and subsequent lack of adequate oral health care. ORAL SURGICAL AND ORTHODONTIC MANAGEMENT OF IMPACTED MAXILLARY CANINES Assoc. Prof. Dr. CENA DIMOVA, PhD, MSc, DMD Faculty of Medical Sciences, Studies for General Stomatology University “Goce Delcev” – Stip FYROM The aim of this study is to present the contemporary approach as well as the interdisciplinary management of impacted canines in the upper jaw. Impacted canines are common and frequent complain. After the third molars, the maxillary canines are the most commonly impacted permanent teeth. About one third of impacted maxillary canines are positioned labials or within the alveolus, and two thirds are located palatals. The appropriate timing and surgical procedure for uncovering an impacted canine are determined by specific criteria. Although the mechanical management of impacted teeth is a routine task for most orthodontists, certain impactions can be frustrating, and the esthetic outcome can be unpredictable if the oral surgeon uncovers the impacted tooth improperly. When referring a patient to have an impacted tooth uncovered, the orthodontist might assume incorrectly that the surgeon knows which surgical procedure to use. However, if not instructed properly, the surgeon could select an inappropriate technique, leaving the orthodontist with the difficult if not sometimes lengthy and challenging task of erupting the impacted tooth into the dental arch. On the other hand, if the correct uncovering technique is chosen, the eruption process can be simplified, resulting in a predictably stable and esthetic result. This is very important in the surgical-orthodontic treatment of impacted maxillary canines. Retention of permanent maxillary canines incurs the risk of complications, among which resorption of permanent incisors is quite frequent. Early diagnosis of resorption is difficult as in most cases it occurs posterior or anterior in the mid-root of the lateral incisor. Studies have demonstrated the necessity to take X-ray pictures not later than at the age of 10 to supplement the clinical examination whenever resorption is suspected. If in the oral cavity examination, lateral incisors exhibit atypical position, i.e. the crowns are distally protruding and deviated and canine ridge is palpable, resorption can be recognized. Completing the orthodontic part of the therapy does not mean finalization of the definitive treatment of the impacted canines. Popa" University of Medicine and Pharmacy – Iasi Romania Introduction: Oral rehabilitation is the integrative concept reuniting each segment of the dento-maxillar apparatus affected by edentation under its various clinical forms, without eluding the complications induced and the influence played by the general status within the chosen therapeutic algorithm. The purpose of this study is to analyze the extent to which the therapeutic solutions within the oral rehabilitation get closer to the therapeutic ideal, as well as the modality in which the reality of the clinical cases reflected in the particular aspects of the prosthetic field, the general condition, the socio-economical criteria influence the clinic finality. Methods: The lot of selected patients, with a complex pathology, in the field of partial edentation, particularizes the algorithm of rehabilitation, as a result of the factorial cumulus generated on the one hand by therapeutic possibilities corroborated with parameters which aim at the prosthetic field and on the other hand with the limits related to the general conditions of the patient, with the local biological and clinical factors, the biomechanical concept or socio-economical criteria. Results: The choice of the therapeutic solution in agreement with the particularity of the clinical case is the result of a complex algorithm, synthesizing the clinical and paraclinical data through the classical methods or the computerized variant, corroborated with the simulative aspects of 3D modeling The implanto-prosthetic therapy, the threshold of exigency in the variety of therapeutic solutions of the current dental medicine plays an important role in the therapeutic solutions under analysis, the therapeutic variants bearing certain divers aspects dictated by the architecture of the prosthetic field, the type of complications, the augmentation techniques used and the application of the bio-mechanical principle. Conclusions: The rehabilitation of partial edentation starts from the simplest solutions and ends with the most complicated and ideal ones, representing a permanence in the field of possibilities and limits, with the patient in the middle of this circle, being ideally ware of the possibility of the best firm solution of treatment, dictated by the deficiencies appeared or preexistent to diminish the emotional field with the certainty of choice in the therapeutic solution. THE ROLE OF PDGF IN COMBINATION WITH BONE AUTOGRAFT IN PREVENTION OR REVERSION OF BONE RESORPTION ANTONIS KONSTANTINIDIS Greece A stable relationship between the soft tissue and the underlying supporting crestal bone is important in implant-supported restorations. Unfortunately, loss of crestal bone or “dieback” or “dish-shaped” defects to the first coronal implant thread is commonly observed immediately after installation of implant abutments. They are also known as “peri-implant bone remodeling” and were taken into account for the definition of the implant success criteria from 1986 (Albrektssa et all 1986). The purpose of the present lecture is to address the multifactorial etiology of the periimplant cervical bone remodeling and mainly to analyze the resorption caused by trauma during the wound healing and osseointegration after implant installation. Today's patients are also expecting to have an immediate restoration after losing their teeth. Although the success rate of 90-95 is impressive. The new technique of 4D planning and navigation could bring us closer to this goal. By using a CT which is fed into the computer in digital form the dentist is able to carry out perfect implant planning. Both, the position of the implants in the vicinity of anatomical structures and the future prostheses can be planned exactly. To optimize this technique in the daily practice, we transfer the 3D data to the dental lab with a special Lab Technology (GPIS) the lab technician is preparing a surgical guide, temporary or final abutments and a provisional restoration for the patient.
