In the past year, we served more than 950 patients in the home and provided over 7000 individual wellness and screening services utilizing the latest technologies. In an extremely challenging health care environment, RVNA performs a unique role. We serve the under and uninsured who do not have access to adequate medical care. We provide a range of services from maternal/child care to chronic disease management, immunization clinics, and educational and wellness programs. We give the young, the elderly, the disabled, the chronically ill and the terminally ill, dignity in health care, where they want it most – in their own homes. RVNA collaborates and partners with the Town’s Health, Fire, and Police Departments and stands ready to serve in the event of emergencies, mass pandemics or natural disasters. We listen to the needs of the community and newly launched an ‘Aging in Place’ program called “Quality Living at Home” that empowers individuals to remain in their homes in a safe, healthy and socially connected environment. Our remarkable care is provided to the greater Ridgefield area: Ridgefield Redding Danbury Georgetown Wilton Easton Weston Bethel Brookfield New Fairfield Newtown Southbury The need for home health care continues to grow dramatically due to an aging population and efforts to reduce the cost of hospital care. RVNA is in greater demand than ever, and the importance of our work will only increase in years to come. We are a vital resource to the towns we serve! Now imagine this is your child and you see it everyday. That’s reality for Jodie and George, parents of EJ Carfi. Imagine the frustration of a young mother who wants to keep her child healthy, properly immunized and ready to begin the school year – but has insufficient insurance to cover a “well care” visit. Perhaps this seems like an unlikely situation in our community – unfortunately, it’s more common than you think. Many families are under-insured (even though they are employed) and do not have the resources to provide their children with the basic immunizations or physical examinations. That’s where RVNA makes a difference – providing the services regardless of the family’s ability to pay. EJ Carfi has epidermolysis bullosa (EB) and everyday activities result in painful sores, blisters and infections requiring daily care and frequent bandage changes. Like the New York Yankee heroes he admires, EJ has the will of a champion to rise above his disease every day and tries to enjoy doing things all 12 year old boys like to do. “ It’s hard to live with but I move along as best I can.” ~EJ Carfi “ His sense of humor, positive attitude and willingness to attempt things that are difficult for him amaze and inspire me!” ~Anne Stewart, BSN, RN RVNA has cared for EJ since he was an infant. “The nurses made it possible for me to cope from the start. They were adaptive and helpful. I could not have managed the baths without the help of the nurses and home health aides. They made it possible for me to work; the nurses and home health aides gave 150%. They were more than caregivers, they became our friends and family.” ~Jodie Soderland Anne Stewart, Maternal/Child Care Nurse and 2009 Nightingale recipient for Excellence in Nursing, shares a special bond with EJ. home healthcare Well Child Clinic partially funded by RVNA Well Child Clinic also keeps kids healthy with low cost care! Well Child Clinic provides care for children from birth to 18 years. The clinic provides well child exams, sports, camp and school physicals, plus all required state and school immunizations for a nominal fee of $50. (sliding scale available) Laura Giovannoli, RVNA RN and 2009 Nightingale Award Recipient for Excellence in Nursing, brings smiles as well as remarkable care to one of our beloved patients. maternal / child care 2|3 staying close to you when needed: The relief of “family” support with the knowledge of a professional: Marianne Lehane, RN feels privileged to share in the process of getting patients to the place they desire most – HOME. “I am your daughter when your daughter is not here” Laura Switzer, LCSW, told Mrs. Anna Mae Hartman when they met over 4 years ago. Bunny Horman, Anna Mae’s daughter describes Laura as ‘the most wonderful proponent for my mother and I can’t describe what a relief that is.” At 96, Mrs. Hartman has lived in her home for 35 years and it is where she wants to stay. Bunny lives out of state and communicates frequently with Laura about her mother’s care. It’s no wonder that Elaine Brennan, an RN who works at Bethel Health Center, chose Marianne to provide home care services for her Aunt Flo. Marianne followed Aunt Flo through her hospitalizations, emergency room visits and rehabilitation stays. She kept Aunt Flo’s niece and RVNA’s case manager informed of any clinical issues that may have affected her recovery. Marianne also identifies any home care services or medical equipment needed for a safe discharge. In this case, Marianne recommended a Nurse, Physical Therapist, and Home Health Aide so that Aunt Flo could return to her prior quality of life and continue her recovery in the comfortable surroundings of her own home. Aunt Flo is so pleased with RVNA staff and says “I look forward to all of my visits!” Marianne Lehane, RN, RVNA Clinical Liaison at Danbury Hospital and 2009 Nightingale Award Recipient for Excellence in Nursing, brings comfort as well as care to Aunt Flo. nurse liaison “ My priority is to find the right match in caregivers for my clients; they are the bright spot in my clients’ lives!” ~Laura Switzer, MSW, LCSW, ACSW “I come about every 8 weeks to see Mom and I am calmer and can enjoy quality time with her. We go to lunch, visit family, or go shopping. Because of the support team Laura has put in place, I don’t have to spend time cleaning, bill paying or running errands. Without a live-in and Laura’s care management, Mom would be in an institution. Even on hospital admissions, Laura meets Mom at the ER. With good care, nutrition and medication oversight that Laura’s program provides, Mom is able to remain in her home.” ~ Bunny Horman Anna Mae Hartman and Ray Bennett are in Laura Switzer’s Senior Care Management Program. RVNA Travel Clinic provides consultations/immunizations and other important health information for adults who travel outside the U.S. RVNA Well Child Clinic keeps kids healthy with affordable preventive health care to children from birth to age 18. Shingles and HPV (to prevent cervical cancer) vaccine, in addition to other routine immunizations, keep residents of all ages safe and healthy. RVNA administered more than 10,000 flu doses to area residents. Seasonal Flu, H1N1 (Swine) Flu & Pneumonia vaccine clinics offered at multiple area wide clinics help keep communities healthy. Weekly free Blood Pressure Screenings offered area wide and at community events. responders to emergency situations: In emergency situations such as snowstorms, power outages and threats to public health and safety, RVNA is designated by the Ridgefield Health Department as a resource for health support in the community. RVNA is the mass immunizer for the town of Ridgefield to administer H1N1 (Swine) Flu vaccine. RVNA worked with the Ridgefield and Bethel Health Departments to secure vaccines. The clinics were organized and staffed by RVNA nurses and volunteer nurses. Phone lines were manned by RVNA staff and Community Emergency Response Team (CERT) volunteers. Andrea Rink, RN performs an N-95 respirator mask fit test. Martha Bryce, RN, is wearing the mask under the hood that is used to perform the testing. In the event visits are made to patients with an infectious disease, such as H1N1 (Swine) Flu, the N-95 respirator mask will protect RVNA staff and ensure all are able to receive care in the home. In the event of a fall or emergency, help is available at the push of a button. RVNA staff sets up the equipment in your home and demonstrates how it works. The Lifeline Personal Help Button connects you to a trained Personal Response Associate who can send help quickly – 24 hours a day, 365 days a year. RVNA will be notified in the event of any emergency. Sandy Weinberg BSN, RN, Director of Community Programs, reviews a travel consultation with Carl and Donna Warren. Gary Holland GIFT OF TIME AND EXPERTISE Gary Holland has served on RVNA’s board of directors for six years, including Chair of the By-Laws committee, and as a Health Fair volunteer. For the last year Gary has chaired a new Technology committee. He has introduced RVNA to the “Art of the Possible” by championing a technology paradigm shift throughout the agency. Because of his knowledge, expertise and patience, RVNA has adopted many new technological advances and processes enabling the agency to function more efficiently and effectively. community honorees Claire Kirby, RN GIFT OF TIME AND EXPERTISE Claire Kirby was first employed by RVNA in 1974 as a staff nurse and retired after 25 years of service. During her employment she developed a reputation in the community unlike any other RVNA nurse. With patients, Claire demonstrated boundless warmth, compassion and care but never tolerated excuses for not helping oneself recover. She motivated everyone to be the best they could be. After retirement, she tirelessly worked at RVNA in every volunteer capacity from flu clinics to Health Fair to the monthly blood pressure clinics. She has become a role model for everyone associated with RVNA – we are proud to know her, work with her and call her our own. Danbury Hospital | Frank J. Kelly, CEO THE PARTNERSHIP AWARD Danbury Hospital has been a major supporter of RVNA’s mission and vision spanning the decades. Danbury Hospital has helped fund the annual Health Fair, Well Child Clinics, free and reduced fee services such as the Monthly Visit Program for the elderly, and community programs for the underinsured and uninsured.
