For questions or feedback, please contact Plus a mechanical option Or select a contraindication Select a pharmacologic and/or risk option The electronic order set (CPOE Powerplan) for VTE prophylaxis has been modified to integrate the most recent guidelines from the American College of Chest Physicians (ACCP) and the American College of Physicians (ACP) for VTE prophylaxis in medical inpatients. The Powerplan has been designed to allow efficient selection of evidence based prophylaxis options based on your assessment of VTE risk. It will also provide documentation shortcuts to help physicians stay in compliance with Medicare’s VTE core measure requirements. New Venous Thromboembolism (VTE) prophylaxis order set for medical patients New VTE Prophylaxis Order Set for Medical Patients 2 HIPAA TIP QUESTION: WHAT CAN I DO TO PROTECT PATIENT PHI DURING MY DAILY ACTIVITIES? ANSWER: The following is a list of tips that can safeguard and help prevent a HIPAA violation. Everyone is responsible in the prevention of a HIPAA violation. Part I of II 1. Keep documents, forms, papers, etc. face down or covered. Do not leave anything with PHI visible to unauthorized individuals, patients, family members, visitors and vendors walk throughout our hospital on a daily basis, leaving anything with PHI visible is a risk and potential HIPAA and California Law violation of Carelessness and Unauthorized Access. 2. Dispose of documents, labels, notes, reports, wrist bands and anything else containing PHI, in shredding bins. Never place items with PHI in a trash can. If you can’t peel off labels from IV bottles, use a permanent marker to black out the patient information. 3. Avoid discussions about patients in public areas; elevators, hallways, cafeteria, when on your phone, etc. or anywhere where others can over hear. 4. You may need to ask family members and visitors to step outside of the patient room prior to discussing or administering procedures, etc. Provide confidentiality by checking with your patient first before discussing PHI with your patient in the presence of others. 5. Select the correct printer when printing patient results / charts from Power Chart or other Cerner applications. When selecting your printer queue, be sure you’ve selected the printer in your area / department. Printing to the wrong printer can result in a HIPAA violation of “carelessness” and California Law violation. 6. Do not share, discuss, disclose, email, text or post any patient information or use personal electronic devices to take, post or share photographs of patients with coworkers, family, friends, etc. or on social networking sites. . 3 American Heart Association Heart Walk Sunday October 3 Rainbow Lagoon, Long Beach 4 Ways to Get Involved 1.
