Dear Residents,
The Streamwood Park District is reaching a major
milestone this year, as it celebrates its 50th
Anniversary. Celebration plans are well under way
and will take place all year long.
We invite you to be a part of the celebration through
our events or by signing the message book for the
time capsule. We are asking Streamwood residents
to share their special memories and photos of the
District from the past 50 years. If you have any
images or memories please share them with us. Any

Proof of residency is required (State ID or Driver’s License) every time you register for a program.
Participants will be charged non-resident fees without appropriate proof of residency. Mastercard,
VISA, Discover, as well as cash, personal checks, and money orders are acceptable forms of payment.

fun during our annual Touch-A-Truck event. This free

pressure, body compostion, flexbility, and muscular
strength and endurance. Personal training for program
design will be discussed, as well as, the analysis of your

This 4-minute workout consists of 20 seconds of all-out
effort (very high intensity training) followed by 10
seconds of rest; repeated 8 times. This class will include
no less than 5 different Tabata Drills, warm up & cool
down/stretch.
Yoga
Poses flow from one to another, breath
synchronizing, and body heat will improve flexibility
and muscle strength. Accomodates all ages.
Zumba®
Shed some pounds with the Latin Dance exercise
craze that Is fun for dancers and non-dancers alike.

18TH ANNUAL STREAMWOOD STRIDE
5K/10K/Kiddie Run
Saturday, June 11
Route will be held on wide, local neighborhood streets. Water stops are located throughout the course.
Kiddie Races • 3 - 7 years old
A high-energy, flat run with lots of motivation from the crowd.
Pancake Breakfast
Pancakes, sausage, juice, milk, coffee, and fruit await race participants. Family members are welcome to take
part in the breakfast for a charge of $3 for children (3-10 years old), and $5 for attendees 11 years and older.
Breakfast will be served in the Park Place Gymnasium immediately following the race.
Awards & Premiums
5K & 10K race awards will be three-deep in the following age groups:
14 and under, 15-19, 20-29, 30-39, 40-49, 50-59, 60-69, 70+
All participants will receive a goodie bag and pancake breakfast. Kiddie Run Participants will receive a t-shirt
and medal. 5K/10K Participants will receive a technical t-shirt.

AQUATIC CENTER RULES
•Children aged ten (10) years and younger must be
accompanied by a paying adult/guardian aged 18 years
or older at all times.
•Children under the age of five (5) must have an adult
aged 18 years or older within arm’s reach at all times.
•The Illinois Department of Public Health requires that all
swimmers wear proper swimming attire. Swimsuits are
required at all times. Cut-offs, gym shorts, sportswear, and
undergarments are strictly prohibited. Patrons not wearing
the proper attire will not be allowed in the pool.
•For your protection, all flotation devices must be U.S.
Coast Guard approved and labeled as such. All children
wearing flotation devices must remain within arm’s reach
of a swimming adult.
•Members or any pool patrons shall abide by all State,
County, Village, and Park District rules and regulations,
and shall conduct themselves in a safe, considerate, and
reasonable manner so as to not cause a disturbance or
to damage equipment and property. Members or any
patrons will display respect, consideration, and courtesy
at all times. The Park District reserves the right to
discontinue any indivudual’s use or membership due to
disruptive or unsafe behavior.
•Parents/Guardians are responsible for the supervision of
their children at all times. Lifeguards are on duty to
enforce rules and regulations in case of emergency.
•Parents will children aged five (5) years and younger are
asked to use the Family Locker Room.

LIFEGUARD CERTIFICATION
Acquire skills and knowledge to prevent and respond
to aquatic emergencies in this SAI StarGuard Certified
course. Certification includes: Lifeguard CPR, AED,
and First Aid. Prerequisites consist of the following:
swim continuous 50 yards without stopping, swim 50
yards with rescue tube, tread water for one (1) minute
without use of hands, retreive a 10 lb. weighted object
from the bottom of the pool.

SWIM LESSONS GENERAL NOTES
•If entry into a class is unavailable, we recommend that
you join the waitlist, as classes may be added due to
changes in enrollment.
•In order to make your child’s swim lessons more effective,
we ask that parents watch all swim lessons from a distance which permits clear observation without distraction.
Seating locations are on the bleachers and concession
areas. In addition, we ask that all siblings of participants
not enrolled in swim lessons be monitored at all times so
that we may maximize learning for all participants.
•On the first day of lessons, we ask that you arrive ten (10)
minutes early with your child’s last swim report card to
check in with the Swim Instructor. Swimmers without a
previous report card, a swim test will be given to
determined their swim level.
•Cancelations will not be honored for missed or
forgotten classes. To request a refund, a Customer
Request for Refund form must be completed and
submitted with any required documents to the Front
Counter or Registration Desk.

Learning to swim is fun, provides health benefits
and could save a child’s life. Sadly, drowning is
the third most common form of accidental
death in children, so learning to swim really is
an essential life skill.
Children absolutely love swimming, with one in
three saying it’s their favourite family activity.
But it's not all about having fun, there are other
great reasons to get your child swimming:
• It keeps your child’s heart and lungs healthy,
improves strength and flexibility, increases
stamina, and even improves balance and
posture.
• Your child will have plenty of opportunities to
make friends and grow in confidence.
• Swimming opens the door to other sports and
activities, including: swimming with dolphins,
scuba diving, rowing and sailing and even
becoming the next Olympic or Paralympic
champion.
• It’s a skill that once learned is rarely forgotten
– there are even swimming events for those
aged over 100.
• Swimming provides challenges, from working
through the ASA Awards to perhaps one day
swimming the English Channel.
• You can't always be there. It may save their
life one day.

FISH SWIM LESSONS
Minnows Swim Lessons emphasizes water
adjustment, preparatory activities for swimming, fun,
and enjoyment. Students will not be learning stroke
work. They will learn basic concepts of swimming
which will prepare them for the Fishbowl. This class is
for the more developed child who does not need
parental assistance in the water. Instructor/student
ratio is 1:5.

FLYING FISH SWIM LESSONS
PRIVATE SWIM LESSONS
Levels 4 - 6
Private Swim Lessons are designed for those who need
the one-on-one instruction. Adults who never learned
to swim, are uncomfortable in group settings, or those
who like more attention on strokes are ideal
participants. Students will share the pool with others
in Private Lessons. Instructors are assigned based on a
student’s abilities. Instructor/student ratio is 1:1.
4 - 15 Years Old
A coninuation of Fish Swim Lessons, Flying Fish
teaches a variety of skills. All children will be tested
and grouped with children of similar or comparable
skills on the first day of class. It is not uncommon for
children to spend more than one term in Fish Lessons in
order to complete the skill levels.
Students must have completed Fish Lessons or it’s
equivalent in order to qualify to Flying Fish.
Level 4 - Stroke Development
Provides a solid reinforcement of the foundation strokes.
Breaststroke is introduced and sidestroke is reinforced.
Intermediate survival skills and water safety are a focus.
Level 5 - Stroke Refinement
Includes review of backstroke, breaststroke, and sidestroke.
Butterfly stroke will be introduced. More advanced survival
skills and water safety are also included.

Children in the Building Block Preschool Program will
learn through play, as they are taught skills to help
develop their creativity as well as their educational
and emotional well-being by qualified and caring
instructors in a friendly atmosphere.

The program gears its curriculum to make sure that
students are taught the skills they will need to enter
Kindergarten, with a student/instructor ratio of 10:1.
Enrollment Restrictions/Requirements
•Children must be the appropriate age (3 or 4 years old)
prior to September 1, 2016 and must be toilet trained
(exception only for the 2.5 year old class). No age
exceptions will be made.
•Parents are required to bring a birth certificate or an
official document with the child’s birthdate and proof of
residency at the time of registration.
•Appropriate immunizations will be required for children to
attend preschool.
•A completed Program Information Form (PIF), child’s
immunization records, and a current photo of your child
must be completed and turned into the park district prior
to the child starting class.
Program Information Forms and the Parent Handbook will be
available on line March 21.
FEES & DEPOSITS
A $100 deposit is due at the time of registration. $25 of
your deposit is a non-refundable service fee. If
payment is made in full, a discount of $20 will be
applied towards your fee.
No refunds will be granted after January 27, 2017.
PAYMENT OPTIONS
The Streamwood Park District offers a payment plan
option. The $100 deposit must be paid for at the time
of registration. Monthly bills (payable online or at
registration) will be generated July 1, 2016 March 1, 2017.

FRIDAY FUNDAYS

first day. This is to allow enough time to notify the school and
change the bus route. All Saturday registrations will not be
able to start until the Wednesday of the following school
week.

KIDS OUT CLUB
Held on no school days for School DistrictU-46 (except
for national holidays), an exciting field trip is planned
and taken on those days. Fee includes
transportation, the cost of the trip, and lunch.

GIRLS SPRING VOLLEYBALL
5th - 8th Grade Girls
Girls will learn fundamental volleyball technical skills, as
well as teamwork, participation, and sportsmanship, all
while emphasizing fun.
Practices start the week of March 21 at Park Place in
the gym, and games will be played on Saturdays
starting on April 9 and finishing on May 14.
The season includes eight (8) to ten (10) practices and
six (6) games against Carol Stream, Bartlett, and
Hanover Park, with an end-of-season tournament.

This inter-village league typically runs from April

9 - 14 Year Old Girls
Girls already enrolled in the Streamwood Park District
Girls Spring Softball League will compete in up to 3
tournaments, which translates to a minimum of 12
games and up to 30 maximum.
Practices will be held once per week during the
regular season, and up to three (3) times per week
after the regular season ends.
A minimum of ten (10) girls must make the team at
each level for this program to run successfully.
Tryouts
Assessments will be made on pitching, fielding, and
hitting. Registration will not be available until all tryouts
have been completed, and girls have been selected
to teams.

FALL OUTDOOR SOCCER LEAGUE
4 Years Old - 8th Grade Boys & Girls
The Fall 2016 season runs from late August through
early November. Once volunteer coaches are
chosen, they will contact team parents in mid-August.
Youth Soccer participants are required to supply their
own cleats, shin guards, and soccer ball.
U6 & U8 Division
The U6 & U8 divisions are strictly instructional. Games
are held at a Streamwood Park District soccer field on
Saturdays throughout the season. U8 teams will
practice one (1) night per week. U6 teams will have
their practice roughly 30 minutes prior to their games.
U10 - U14 Divisions
Both the girls and co-ed divisions are part of the
Inter-Village Soccer Program, which requires limited
travel to neighboring towns. Teams will practice once
or twice per week, pending availability.

GOLDEN LION ACADEMY
MARTIAL ARTS
Golden Lion Academy Martial Arts is operated through
various Park Districts. The focus of the Golden Lion
Academy combines the disciplines of Tae-Kwon-Do,
Hapkido, Judo, and Kung-Fu.
Students six (6) years and up learn the rudiments of
self-defense from day one. They build upon these
features by adding more advanced techniques from
the various styles, while also learn the Korean
language and the history of martial arts.
As students progress, they earn opportunities for
advancement through various ranks in a safe,
friendly, and enjoyable environment. Dress in
comfortable clothing. Uniforms are available through
the instructor.
Golden Lion Academy is ongoing throughout the year.

ILLINOIS SHOTOKAN KARATE
Illinois Shotokan Karate Club is Chicagoland’s premier
karate school taught under the instruction of John
DiPasquale, a four-time National Champion, US
National Coach, and President of the US National
Karate Federation. Beginner students will not be
permitted to register into the current session after
the second class has met. Registration for Continuers
through Advanced students will be accepted after

good mix of strength training with cardiovascular and
flexibility exercises. Children increase their
coordination, agility & poise, as well as learn lasting

Summer Dance Camp is a fun alternative to the
traditional day camp. Kids get the camp experience
with games, field trips, and swim days while also
learning new dance skills and fun choreography.
Each camp will put on a showcase on the last Friday
of each session for parents, family, and friends.
Please remember to send your camper with a water
bottle, lunch, and gym shoes each day. Campers
must also have appropriate dance shoes for their
camp (i.e. ballet camp = ballet shoes, jazz camp =
jazzz shoes, etc.) and dance clothing.
Before and After Camp Care is available through the
Streamwood Park District Summer Camp Program. See

Our instructors, who are graduates in music education,
professional musicians, and experienced educators,
are eager to bring the joy of music into your child’s life.
Opportunities exist for students to receive instruction in:
voice, piano, guitar, and violin.

are invited to join us in one of our 47 parks where they

and pull pesky weeds as we prepare for another
beautiful summer in Streamwood.

Our 9-hole disc golf course is located at Sunny Hill Park
(1440 W. Irving Park Road; just west of the Route 59 &
Irving Park Road intersection). Hole One is located
near the top of the hill in the southeast corner of the
park.
FREEDOM RUN DOG PARK
Through an intergovenmental agreement with
Hoffman Estates Park District and Elgin Parks &
Recreation Department, Streamwood Park District
provides our residents with an off-leash membership
dog park.
Freedom Run is open daily year-round.

Our 18-hole disc golf course is located in the beautiful
setting of Shady Oaks Park; located at 777 S. Bartlett
Road (behind the Comunity Center & Admin
Buildings). The first hole is located in the southwest
corner of the park, directly behind the Parks
Maintenance garage.
Both courses are open to the public free of charge all
year round. Free Shady Oaks scorecards are available
online.

VANDALISM HOTLINE
The Streamwood Park District is committed to keeping
our parks and playgrounds clean.
As of recently, public playgrounds have become
the targets of arsonists. When a loss occurs, an entire
playground worth tens of thousands of dollars can be
lost to children of the community within a matter of
minutes. Other park amenities such as shelters, garbage cans, and benches have also been damaged
or destroyed.
Please join is in our ever-going mission to continue to
keep our parks and playgrounds clean. If you see any
vandalism, please call the confidential Vandalism

DOG OBEDIENCE
Our novice dog obedience course is designed to
educate the public on being responsible dog owners
and to recognize and control problems before they
become unmanageable. You will teach your dog to
heel, sit, down, stay, stand for veterinary exam and
come when called by the end of the session. Along
with learning basic commands, the topics of
nutrition, immunizations, and grooming will be
discussed. The first class is held without the dog.

Park Place Family Recreation Center has many
amenities and party themes to suit you and your
child’s ideal birthday party wishes.
A Park Place Party includes:
•Three (3) hours in the Party Room
•Thirty (30) minutes before/after your party to set-up,
decorate, and clean up. Trash bags/ trash removal is
included.
•A dedicated Party Host/Hostess to help set-up your party;
periodic check ups on you and your guests during your
time in the party room, and for post-party clean up.

•Plastic table covers in the color of your choice.
•Ice, ice bucket with scoop.
•You are welcome to bring in your own decorations, juice,

The Palomino Shelter is located up the path on the

PASSPORT TO ADVENTURE

Looking for fun ways to learn about history?
Participate in the Passport to Adventure Program. The
program runs from June 1 to August 31, and is a great
way to visit your local museums and nature centers
throughout the Summer.
The Hoosier Grove Museum will have an activity for
your child to complete during the passport program.
GENERAL INFORMATION
General Guidelines
At the museum you can explore the building, touch
artifacts and learn about life in a one-room
schoolhouse. There also will be a free take-home craft
offered by the museum every Saturday.

• The Hoosier Grove Schoolhouse was built in 1904 on
Schuenman’s Farm. A typical school day was from 9 a.m. - 4 p.m.
• The schoolhouse was not only a school, but was also used for
elections, picnics, celebrations, and public meetings.
• The schoolhouse was struck by lightning in 1907.
• The Hoosier Grove Schoolhouse was part of School District 43 1/2.
The “1/2” being that half of the school was in Hanover Township,
and the other half in Schaumburg Township.
• When the schoolhouse closed in 1954, there still wasn’t any
running water.

NWSRA exists to provide outstanding opportunities through recreation for people with disabilities. Celebrate ability throughout the
Northwest suburbs. Join the celebration! The team of full-time Therapeutic Recreation professionals and trained part-time staff at the
Northwest Special Recreation Association (NWSRA) offer safe, fun, and quality year-round opportunities to individuals with disabilities
of all ages and ability levels. Inclusion assistance provides individuals with disabilities the opportunity to participate in their home park
district program. When registering, the parent of the individual should inform the park district (and NWSRA, if desired) of any
accommodation needed in order for the individual to successfully enjoy the program. The park district staff will then contact NWSRA for

the programs listed within our brochure, our District
also provides specialized services for people with
disabilities through the Northwest Special Recreation
Association. Call NWSRA at (847) 392-2848, for information. If you have any questions about the Park
District’s policy regarding ADA, or believe you have
been unfairly discriminated against the provision of
programs, services, or activities of the Streamwood
Park District, please contact Superintendent of Recreation, Kurt Reckamp, at (630) 483-3091.
Your Cooperation Is Needed
Parks are for everyone to enjoy and should not be
used as a dumping ground for litter, garbage, or
waste. Keep your parks clean and free from debris
and waste. If you walk your pet in or around a park,
please be sure to pick up after it. Keeping your
parks clean will enhance the beauty of the park. We
appreciate your cooperation.
Control of Pets
Pets, including dogs, cats, and other
domesticated animals kept as pets, may be brought
into any park, but only when leashed in accordance with Village of Streamwood ordinance(s),
licensed as necessary, and controlled by the person
bringing such pet into the park. It is expressly prohibited for any pet, even though on a leash, to enter
or be on a park playground. Defecation by any pet
while on Park District property shall be immediately
and completely removed by the person in control of
the pet. No pets shall be allowed in any Park District
facility or building unless brought in for a Park District
program or event involving such pet. In such case,
the pet must be leashed, licensed and controlled
as aforesaid. The provisions of this section shall not
apply to service animals in the performance of their
duties. All Streamwood Park District ordinances are
enforced by Streamwood Police Department.
Smoking Policy
In compliance with state and federal laws, the
Streamwood Park District has set the following
policy: No cigarette, pipe, or cigar smoking will be
allowed in any of the Park District facilities. The policy includes Park District activities as well as private
rentals. Your cooperation is appreciated.
Child Car Seat Requirement
All parents and guardians of children participating
in a Park District trip or program where they are
being transported by a Park District vehicle, must
provide a child restraint system for any child 7 years
old or younger or sign a waiver giving permission for
the child to be transported without such system. All
participants must wear seat belts. For trips using a
school bus, there are no restraints for adult passengers or children.

Emergency Shelter
In case of a disaster situation as identified by the
Emergency Services and Disaster agency, Park
Place will be used as an emergency shelter. If this
occurs, it may be necessary for some programs to
be postponed or canceled.
Lightning Prediction System
The Streamwood Park District has installed three
lightning prediction systems. One located on the
roof of Park Place Family Recreation Center, the
second at Hoosier Grove Park, and the third at
Rahlfs Woods Park. These systems gather atmospheric information to accurately predict when
conditions are conducive for a lightning strike. When
conditions are “right,” the warning will sound (a
single fifteen (15) second horn blast) followed by an
orange strobe light located outside on the roof at
all locations and inside of the building at Park Place.
At this point, participants in outdoor activities need
to cease all activity and seek shelter immediately.
When the hazardous weather has passed, the “all
clear” will sound (a series of three-5 second horn
blasts), and the orange strobe will turn off. Only
at this point should outdoor activities resume. It
is important to remember that even though the
system may not sound when weather conditions are
threatening, you should exercise good judgment.
Proceed to stop all outdoor activities and move
to a safe location until the dangerous conditions
have passed. Horns from the Park Place location
can be heard at Dolphin Park baseball fields. These
warning systems will be active during the months
of April through October. Further, the District will be
checking the systems manually, on the first and third
Tuesdays of every month (April-October) at 10:30
a.m. Please contact Parks Manager, Amy Vito at

Indoor Facility and Outdoor Park Rentals The
Streamwood Park District’s Hoosier Grove Barn,
Park Place rooms, gymnasium and pools, Outdoor
Picnic Shelters and Gazebos, and the Splash Pad
are available for rental to the public. For information or to schedule an appointment, contact
Hoosier Grove Barn Office, 700 W. Irving Park Rd.,
at (630) 483- 3135. Community Center, Fields and
Super Cooker For information on renting our rooms
or gymnasium at the Community Center, please
contact Recreation Manager, Jake Larson, at (630)

on renting our super cooker or ball fields throughout
the Park District, please call Recreation Manager,

Camera Restrictions
The use of any visual recording device in any protected areas is prohibited. Visual recording devices
include, but are not limited to, any camera, camcorder, or videotaping device, and cellular phone
cameras. Protected areas are defined as any locker
room, dressing room, bathroom/rest room, lavatory,
shower, or bathing facility located in any District
facility. Any person caught violating this section of
Park rules may be banned from Park District facilities
and subject to a fine.
Brochure Changes/Errors Disclaimer
Due to the large amount of information available in
the Streamwood Park District brochure, errors and
changes before or after publication may occur.
We apologize for any errors in this brochure and will
advise you of any changes as quickly as possible.

when these situations arise.
Privacy Policy
The personal and private information collected by
the Streamwood Park District from any individual(s)
is intended for the sole use of the District, and such
information will not be sold or distributed to other
persons or agencies without the written permission
of the individual(s) providing it, except as required
by law. If information is provided to any other person
or agency pursuant to any legal requirement, the
District will advise the individual(s) of the disclosure.
Resident and Business Rates
A Streamwood Park District resident, via property
taxes, makes a significant financial contribution to
the operation of the District, whether or not he or
she takes advantage of the services offered. The
nonresident fees are required of nonresidents to apportion an equalizing fee to contribute to the overall
financing of the park system on an equitable basis
with the resident.
Resident Fees: A “resident” is defined as anyone
living with in the geographic boundaries of the
Streamwood Park District. Residents of the Streamwood Park District have priority when registering for
most programs.
Business Rate: All people that work at a business
within the Streamwood Park District boundaries
may receive resident rates on memberships (proper
identification required along with current pay stub,
program participants will be randomly sorted in the
nonresident lottery).

DISTRICT POLICIES
Refund Policy
To request a refund on a program, a Refund
Request Form must be completed and submitted
to Registration. Administrative approval must be
received before a refund will be issued.
1. A full refund will be given when the Park District
cancels a trip/program or a refund is requested as a
result of being placed on a waiting list.
2. A refund minus a service charge will be given
when a refund request is received a minimum of
four days before the start of the registered program.
3. A prorated refund with a service charge may be
granted when a refund request is received with less
than four days before the start or after the start of
the registered program and the reason is approved.
Documentation may be requested with your refund
request in order to be approved. The prorated
refund will be based on the date the request was
received, not the last attended class. A refund will
not be approved if the session is more than half
over. No refunds will be issued for individually missed
or forgotten classes.
4. No refund requests will be granted that are made
more than two (2) business days after a one day
program or special event.
5. Refunds for trips will not be given unless one of the
following occurs:
A. Park District cancels the trip.
B. A replacement is found to take the trip.
6. Deposits for certain programs and trips are identified as nonrefundable and therefore the refund
policy does not apply.
7. The cost of purchased individual supplies or
equipment such as t-shirts, uniforms, trophies,
costumes, etc. will be deducted from your refund.
No reimbursements will be given for required
equipment or supply purchases made by the participant or parent.
8. Please allow 4-6 weeks for your refund to be
processed. If originally paid by check or cash, a
check will be mailed to you. If paid by VISA, Master
Card, or Discover Card, we must credit the card
with which payment was made.
9. Any programs with special policies void the
preceding. See page 9 for membership and

Service Charge Policy
All refunds issued which are not a result of a Park
District cancellation will be assessed a service
charge. The service charge for programs with fees
of $10 or more will be $5; programs with fees less
than $10 will be charged 50% of the program fee.
Class Cancellations
Every effort is made to avoid canceling classes.
However, a minimum number of participants must
be registered. Notification of class cancellation may
be made with as little as 24-hour notice. In cases
where a program is canceled, you will be notified
prior to the start of the program and a full refund will
be issued.
Transfer Policy
All transfer requests will be honored if there are
openings in the class requesting to be transferred to
and the participant is the appropriate age for the
program. If additional payment is due, this must be
made at the time of the transfer request. For refund
see #7 under Refund Policy.
Program Scholarships
The Park District recognizes that there are people in
the community who are often unable to participate
in recreation programs due to economic hardships.
If any resident individual or family would like to apply
for a scholarship, pick up an application packet at
Park Place Registration Office or call the Registration
Counter, (630) 483-3025, or Recreation
Manager, Cindy Babicz at (630) 483-3730 or
cbabicz@spdcares.com to have one mailed or
emailed. All requests will remain confidential.
Important Information regarding Waiver and Release
STREAMWOOD PARK DISTRICT
The Streamwood Park District is committed to conducting its recreation programs and activities in a
safe manner and holds the safety of participants in
high regard. The Streamwood Park District continually strives to reduce such risks and insists that all participants and parents/guardians of minors registering
for programs/activities must recognize that there is
an inherent risk of injury when choosing to participate. You are solely responsible for determining if
you or your minor child/ward are physically fit and/
or skilled for the activities contemplated. It is always
advisable, especially if the participant is pregnant,
disabled in any way or recently suffered an illness,
injury, or impairment, to consult a physician before
undertaking any physical activity.
Warning of Risk
Recreational activities/programs are intended to
challenge and engage the physical, mental, and
emotional resources of each participant. Despite
careful and proper preparation, instruction, medical
advice, conditioning and equipment, there is still a
risk of serious injury when participating in any recreational activity/program. Understandably, not all
hazards and dangers can be foreseen. Depending
on the particular activity, participants must understand that certain risks, dangers, and injuries due to
inclement weather, slipping, falling, poor skill level
or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate
or defective equipment, inadequate supervision,
instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational
activities/programs exist. In this regard, it must be
recognized that it is impossible for the Streamwood
Park District to guarantee absolute safety.
Behavior Policy
Appropriate behavior is expected from all participants and observers in programs and in facilities
to protect the enjoyment and safety of all patrons.
Behaviors that will not be accepted include, but are
not limited to, the following:
• Disrupting a program or creating a disturbance at
a facility.
• Continuous refusal to follow program and/or facility rules and guidelines.
•Use of verbal harassment, profanity, vulgarity,
obscenity, or racial slurs.
•Blatant disrespect of staff and program or facility
rules and guidelines.
•Damage, vandalism, or theft of private or Park
District facilities, equipment or supplies.
• Possession or use of illegal substances or medication, including the use of alcohol or smoking where
prohibited.
• Inappropriate or offensive dress, including, but not
limited to dress which is too revealing and clothing which displays obscene or offensive words or
pictures.
• Possession or threat of a weapon.
•Gang activity including display or possession of
symbols, hand signals, soliciting membership, intimidating or threatening any individual, wearing or
displaying colors or items of dress, etc.
• Inappropriate display of affection, including but
not limited to prolonged kissing and touching of any
private area of the body.
The following outlines steps that may be used with
patrons who are not behaving appropriately. Membership or program fees will not be refunded for any
person removed from a program or facility due to
violation of this behavior policy. Some programs or
facilities may have stricter or more detailed behavior policies.

participant is involved in one severe incident, as
determined by District staff, the participant may no
longer participate in the program until a meeting
is scheduled and held. The participant, a parent/
guardian if the participant is a minor, the program
instructor, the program supervisor and any other
persons whom the District considers necessary for
a resolution of the inappropriate behavior, shall be
present at such meeting. At the meeting, the question of continued participation in the program will
be considered and, primarily for minor participants,
a Behavior Contract may be prepared as a condition of continued participation. Such a Behavior
Contract will be personalized to the circumstances
and the participant, and will include specific expectations and consequences for failure to meet such
expectations, including possible suspension and/or
termination from program participation. Inappropriate behavior by adults may result in more rapid
progress toward suspension or termination from
program participation.
Facilities
All facility participants are expected to follow the
rules of the facilities. Specialty areas of the facilities
(example: fitness center and pools) have posted
signs stating special rules specific to that area. Disregard or abuse of facility rules may result in removal
by staff or police from the facility for a designated
length of time or permanently.
Observers
Observers of programs are also expected to
behave appropriately. Parents, friends, and any
observers are
held to the same standards of behavior as participants. If inappropriate behavior of a program observer is reported to or observed by a staff person,
the observer will be asked to leave by the staff or
police, if necessary.
Appeals
Any participant who is suspended or terminated
from program participation shall have the right to
appeal to the Executive Director within fourteen
days of any notice of suspension or termination.
The Executive Director shall thereupon schedule a
meeting concerning such appeal, and the appellant shall be entitled to present his or her version
of the facts at such meeting. Within five days after
such meetings, the Executive Director shall notify the
appellant in writing that the suspension or termination has been affirmed, modified, or reversed.
The Executive Director shall notify the Board of Park
Commissioners of the meeting and the decision. It is
a vision of the Streamwood Park District to provide
safe and wholesome programs in well-maintained
facilities for our residents and guests. Rules must be
followed and policies such as this must be established to protect this vision for all involved. Our goal
is not to remove patrons from programs or facilities,
but to work together with participants and/or parents/guardians to the best of our ability to correct or
deal with a problem. The Streamwood Park District
asks for your cooperation in behaving appropriately
and also teaching your children the importance of
appropriate behavior and following rules.

Program Participants
All program participants are expected to behave
appropriately and follow the rules of the program
and facility. A participant will receive written notice
for inappropriate behavior, and in the case of a
minor, a copy of the notice will be provided to such
