Services Delivered in Outlying Areas (Rick Williams / Heather Moore)
While social services have a strong presence in the towns (Bracebridge, Huntsville, Gravenhurst), there is a very small
presence in the townships (Muskoka Lakes, Lake of Bays and Georgian Bay).
This project will bring some services to the outlying townships and villages in Muskoka, customizing services for each
area depending upon the need identified.
The goal is for all citizens to have access to social services and to have ongoing updates to the program based on
feedback from the communities.
Services will be offered out of a variety of public spaces such as municipal offices, community centres, libraries, etc.
Home At Last (Mary Camley, VON)
i. services to support the transition from hospital to home, with transportation and a Home
at Last Attendant
In operation since 2008 - servicing all 7 hospitals in Simcoe / Muskoka; and partners with many other organizations such as ‘A
Friend in Need’, ‘Friends’, ‘Muskoka Seniors’ (for transportation), etc.
Challenges include servicing a large geographical area (small towns, etc) with low resources.
Home At Last contacted in any number of ways – agencies, discharge nurses at hospitals, families, individuals.
Services are free. (Funded through Provincial Government). Typically service starts on same day requested.
PSW assists during transportation home, and getting settled at home. Will make stops on way home from hospital for groceries,
medicine, coffee, etc. PSW will also help with settling in at home – light cleaning, making bed, stocking fridge, some personal
care. (PSW uses a ‘settle-in checklist’). PSW checks for safety (i.e.: grab bars in bathroom, scatter rugs)
PSW ensures senior understand medication, and will, on occasion, accompany senior to the Pharmacy to understand the
pharmacists directions.
On average, PSW spends approximately 2 hours with the senior. Sometimes much longer, or even until family member can get
home from work, etc.
PSW follows up within 24-48 hours.
65+ qualifies, or under 65 years of age with chronic illness. Not usually people in their 40s.
Over last 2 years, have surpassed target numbers; the program is well-subscribed in the Muskoka area.
Future: Need more education about Home At Last for discharge nurses (many are not aware of service).
High satisfaction rate.

Seniors Transportation Committee and District Municipality of Muskoka Transportation Committee have merged. Heather will
continue to attend these committee meetings. 
Meeting with Hammond Transportation to discuss accessibility bussing. Since the vehicle is not used very often, the cost is
prohibitive for most people. Looking for economies of scale – with more people utilizing the bus, perhaps the cost could come
down? 
Hammond Transportation may be able to provide town to town transportation.

Round Table
Sarah Clipsham (CHS): There is a new regional director for Peterborough / Muskoka, plus a Program Manager for Muskoka.
Kellly Sawyer (The Friends): Current vacancy in Oakwood Heights (RGI, disabled, accessible unit).
Debbie Wight (NSMCCAC): CHRIS (Client Health Records Information System) will now sow all community support services. Working on
common communication.
Alda Tee (RVH): Currently 6 facilitators for ‘Living with Stroke’ program trained in Muskoka. (Assist with reintegration after stroke.) Alda
to provide more information when available.
Christine Gutmann (A Friend in Need): Starting program for seniors before palliative care is needed.
Heather Moore (DMM): Building tour following meeting. Office space in front entrance that can be used for agencies. It will be set up
with wireless internet (as will the Parlour). A process will be created whereby upon request of use of the office, a pass code to the
building will be given.
