ADMISSIONS APPLICATION PROCESS
Dear Student:
Welcome to DVC San Ramon Campus! Your first step will be to apply online to the college.
Once submitted, your application will be completed within 24 hours. The online application
can be accessed at www.dvc.edu. Click on the following link to begin:
Select “APPLY NOW” > Select “Step 1” Click “OpenCCC”
If you have not received an email with your student ID# within 24 hours of applying, contact
the district help desk by email at it-help@4cd.edu or by phone at 925-229-6888 to obtain your
student ID#.
If you have previously attended Diablo Valley College, you will need to submit a new
application. Instructions for completing the online application are included in this packet.
Due to privacy laws, OpenCCC will not accept online applications for persons under the age of
13. Paper applications can be obtained at the Admissions and Records Office.

High School Special Admission Application/Registration Information
Welcome to DVC San Ramon Campus!
 Your registration is not complete until all forms have been processed by the
DVC/SRC Admissions Office. Not enrolled = NO CREDIT.
 Please bring a picture ID (HS ID, CA ID, Driver’s License, or passport) when visiting
the Admissions Office.
 High school students are not eligible to enroll in classes on-line.
Start the process by applying to the college online at www.dvc.edu
SPRING 2016 - 11th & 12 graders
*September 15th - DVC starts accepting online applications for spring 2016
*Prior to January 5th – Online application and prerequisite forms (if required by course) should be
submitted and on file with the Admissions office.
*January 12th – In-person registration begins. Special admissions forms are only accepted when a student
is officially enrolled or waitlisted into a class. Instructor signature is required for all PE courses
regardless of the student’s high school grade level.
SPRING 2016 - 10th graders and below

First day of instruction – The special admissions form is only accepted when a student is officially
enrolled into a class. Instructor’s signature is required for enrollment (add code alone is insufficient for
10th graders and below).
All Students – courses with prerequisites
Official transcripts with proof of completion of the prerequisite course must be turned in to the
Admissions office, along with a completed DVC Prerequisite form, to enroll. Prerequisites may also be
satisfied by taking the assessment test. To enroll in Math 120 or below the online self-assessment test
(ISSA) may be taken. Please check the DVC Catalog or schedule of classes for prerequisite requirements.
Transcript request
In order for your grades to be sent to the high school, please submit your transcript request online.
Transcripts are sent only upon the student’s request. You may submit the online request after final grades
are posted. The first two regular process copies are free – charges will apply after that.

Students in the 11th or 12th grade must register in person beginning on the first day of special admit/concurrent registration.
Students below the 11th grade must indicate a course section number and obtain the instructor’s signature and register the first day
of class.
All students enrolling in a P.E. class must indicate a course section number and obtain the instructor’s signature.
The principal or designee must write in the box below the approved courses. Form must be completed with courses identified prior
to submission. Incomplete forms will be confiscated.
Home schooled students must attach a copy of their Dept. of Education Private School Affidavit.
The original form must be returned to Admissions and Records at the time of registration.
Courses with a prerequisite must be cleared prior to enrollment. The Prerequisite Equivalency Form and transcript (official or
unofficial) are to be submitted to the Admissions Office. Students are notified by email in 2-3 business days if the prerequisite has
been approved or denied. Assessment test and ISSA results may also clear some prerequisites. Students receiving permission to
register for waitlisted courses are required to return to Admissions to register in person.

The Concurrent Enrollment Program is open to any K-12 student who, in the opinion of the Superintendent/President or
designee, can benefit from advanced instruction.
Students may be admitted upon recommendation of the principal and parent. All students under the 11 th grade must
obtain the DVC Instructor’s permission to enroll in their course on the first day of the class. Additionally, all concurrent
students, regardless of grade level, must receive the permission of the DVC instructor on the first day of class prior to
enrolling in P.E. classes.
College level courses will only be available to high school students who do not have the course equivalencies offered in
the student’s home K-12 school district.
The College reserves the right to exclude or limit enrollment into programs where health, safety, instructional
methodology, facility constraints, or legal requirements are deemed inappropriate for concurrently enrolled students.
Concurrently enrolled students below the 11th grade will be permitted to enroll in no more than two courses, or 7 units,
per term.
Special Admit students are exempt from enrollment fees if enrolled in less than 12 units. However, Special Admit
students must pay the student union & activity fees. Special permission is required for students wishing to enroll in more
than 11 units. Contact Admissions for details. All other fees are required and must be paid at the time of
registration.
All coursework at DVC becomes part of your permanent academic history. Grades received or excessive drops may have
implications involving financial assistance, athletic eligibility, probation, etc.

Concurrent students are treated like every other college student. The rules regarding student records are governed by the
Family Education Rights and Privacy Act (FERPA). Academic information is only released with the written consent of
the student.
Please note: Your son or daughter will be exposed to a diverse population in educational programs designed for the adult
learner, which may involve sensitive topics that might be considered controversial or offensive to some. Your signature
on the reverse side of this form acknowledges your receipt of this information and stipulates your permission for your
child to enroll in a college level course and participate in all required activities that may include field trips off campus.
All courses taken will be taken for college credit and become a part of their official college record.

Complete an online Application for Admissions prior to in-person registration.
Home schooled students must provide verification of an Affidavit for Home Schooling, filed with Department of
Education, at the time or registration.
Submit an official or unofficial High School transcript for verification of prerequisite course completion, along with a
Prerequisite Equivalency Form. Prerequisites are strictly enforced and will not be waived.
Students below the junior level or students enrolling in PE courses must register in person after obtaining the DVC
instructor signature on the first day of the class. Registration is not permitted before the first day of class.
All concurrent students must register in person with a completed Special Admissions Recommendation form. Check
registration calendar for dates.
Incomplete forms will be kept by the Admissions Office, and a new form will be required to register.
How Do I Apply Online

time applying. Click “Sign In” if you have an
account.

After you sign in click on “Start A New
Application” to begin your application.
*IMPORTANT
It is recommended but not required to provide your Social Security Number when you apply. If you provide your SSN
on this application you may find it easier to access your student records when you need them if you forget or misplace
your DVC student ID number. If you do not provide your SSN or do not have one, you will always need to use your DVC
student ID number to access your student records. This information is emailed to you when you apply online. You will
need to come in person to the Admissions & Records office or Enrollment Lab and bring a photo ID to obtain your student ID number and InSite WebAdvisor User Name if you misplace this information. All OpenCCC institutions are required by law to keep your SSN private and secure and not to reveal it without your permission.

It is recommended that you take the DVC Assessment to determine your level of English and math skills. English and
math course work used to satisfy a prerequisite requirement will also satisfy assessment and placement.
This form will not be processed if incomplete or submitted without documentation.
HOW DO I CLEAR A PREREQUISITE REQUIREMENT? Submit this form along with documentation that you have satisfied the
prerequisite at another institution or submit this form if you completed the prerequisite course work at DVC prior to
summer 1999. (Please note: CCC and LMC coursework completed after 1977 need not be documented.)
Your prerequisite must be cleared prior to your registering for classes.
Prerequisite coursework must be completed with a “C” grade or better.
Documentation may take the form of
 an official or unofficial transcript
 a report card with final grades
 a letter from instructor/school on official letterhead stating the course was completed and grade.
These documents must include your name or social security number and the name of the school. Even if you have already
submitted official transcripts from another institution you must still attach a copy to this form.
CAN I USE IN-PROGRESS COURSEWORK? In-progress coursework cannot be used unless you are taking the course at CCC,
DVC or LMC.

Effective June 1, 2010 we will only be testing students who are enrolling at DVC in either the present
or future semester. We will not test students who are planning on attending another school.
Accordingly, we will not send test scores to other schools.
If you have previously taken an assessment test at DVC and would like to use your scores at another
school you will need to print your Test Summary from the WebAdvisor system to send to the school.
An interpretation of these scores are available on our Test Score Results page.
NEW INFO ABOUT THE MATH TEST The math assessment test is designed to determine your math
skills in order to place you into the appropriate math classes at DVC. Currently we offer two different
options for placement testing. Which math test should you take? Can you answer these questions?

NO - If you can't answer the questions above AND/OR you haven't completed Algebra II or higher
within the last two years, then take the ISSA at http://www.dvc.edu/issa/. This can be done from
your personal computer.
Informed Student Self Assessment (ISSA)
In addition to assessing your skills and background in math, the ISSA explores your overall time
commitments and activities outside of math. This test is available online and may be taken multiple
times a semester. Students do not come to the Assessment Center to take the test, as it is designed
to be taken at your convenience at any computer of your choice with internet access.
This test is used for placement into:
Math 75 - Arithmetic and Pre-algebra
Math 110 - Elementary Algebra
Math 114 - Geometry
Math 120 - Intermediate Algebra
YES - You can take the test at DVC Pleasant Hill or San Ramon Campus. If you have taken Algebra II or
higher within the last two years AND you are able to successfully answer the questions on the College
Level Math sample test then you should take the test.
