I’d like to personally welcome each of you to the 43rd Annual Review of
Dentistry conference, held in beautiful Destin, Florida. It’s an exciting
time for the Alabama Academy of General Dentistry as our community
collectively grows, adapts and responds to the ever-evolving practice
of dentistry, within our increasingly complex healthcare environment.
Our organization is in the midst of a period of many local, regional, and
national challenges. Our industry, the world of dentistry, is an exciting and rewarding area
in which we can each work, study, and find enjoyment. As a collective, we will benefit from
joining together in meetings like this, to gather and inspire each other, to ensure the Academy of General Dentistry remains at the cutting edge of science and practice.
I would value the opportunity to give you an idea of what you can expect to see and hear
and what we hope to achieve during our 2016 summer meeting. Within the conference
agenda, you can expect to see:
• Education and updates on dental materials, periodontal science, oral surgery, use of lasers within clinical practice, and endodontics;
• A certification course for oral conscience sedation;
• A hands-on course for Six Month Smiles;
• Courses on embezzlement and money management; and
• An innovative approach to mandatory CPR and infection control training.

Dr. John Coke received his undergraduate degree from the
University of Colorado-Boulder and then graduated with a DDS
degree from Loyola University School of Dentistry in Chicago.
Immediately following, he completed a one-year General Practice
Residency at Portsmouth Naval Hospital, Portsmouth, VA. After
two years as a Naval Dental Officer in Guantanamo Bay, Cuba,
he completed a two-year certificate program in Oral Medicine
and Oral Diagnosis from the University of Illinois--Chicago. Dr.
Coke has been involved with GPR hospital-based programs in
Chicago, IL, Denver, CO and Gainesville, FL. Dr. Coke retired
from UAB as GPR/AEGD Program Director in 2014 and is currently a part-time Adjunct Professor in Hospital Dentistry.
Dr. Coke is board certified by the American Board of Oral Medicine, Diplomate of the American Board of Special Care Dentistry
and Fellow of the American Association of Hospital Dentists.
He has been active with the American Dental Association Commission of Dental Accreditation serving as a Site Evaluator
since 1984 and is past Chairman of the Advisory Committee for
Advanced Education in General Practice Residency. He has
given over one hundred CE presentations nationally on common sense pharmacology for the general dentist, medical risk
management, infection control and adult minimal sedation for the
general dentist. He is the author and co-author of twenty eight
refereed publications. In 2012 Dr. Coke was awarded the UAB
President’s Award for Excellence in Teaching.
Dr. Mike Edwards graduated with a DMD degree from the
University of Alabama School of Dentistry in Birmingham. He
completed an externship with the United States Public Health/Indian Health Service in St. Ignatius, Montana. Dr. Edwards began
private practice in 1979 and has received both his Fellowship
and Mastership in The Academy of General Dentistry.
Dr. Edwards is a past President of the Alabama Dental Association. Dr. Edwards has practiced over 36 years incorporating
enteral and parenteral sedation in both practice and teaching
environments. He presents lectures nationally on the subject of
sedation and emergencies. Dr. Edwards is a faculty member of
the new ADA Simulator Course “Recognition and Management
of Airway Emergencies in Sedation” presented in Chicago and
San Francisco. Dr. Edwards served two terms on the American
Dental Association Committee on Anesthesia (once as chairman)
and is a member of the ADA Council on Dental Education and
Licensure.
Course Objectives:
By the end of this course, dentists will be able to:
• explain the spectrum of sedation from minimal to general
anesthesia.
• describe sedation agents commonly used in dentistry and
the basics of their pharmacology and specific indications in
dentistry.
• be able to evaluate patients to establish their medical risk assessment and identify medical conditions that will modify their
minimal sedations.
• use proper documentation required, including consent forms,
monitoring forms for sedation and patient information packages; how to utilize your staff in the documentation process.
• discuss proper monitoring techniques for the sedated patient
and the use of your dental team.
• discuss different sedation protocols and office set-up specific
for individual patients and specific for dental procedures; how
to effectively incorporate dental staff into these protocols.
• identify and explain potential medical emergencies specific
to oral sedation; recognition and management by the dental
team.
• demonstrate the basics of airway ventilation and management.
By the end of this course, the Dental Team will be able to:
• know how to utilize the dental team in the documentation
process.
• identify the basics of monitoring, both external monitors and
use of eyes, ears and mouth.

Dr. Karl Koerner is currently Director of Oral Surgery for Intermountain Dental Associates in Utah. He is also an Adjunct
Professor in Oral Surgery at Roseman University of Health Sciences, College of Dentistry in Utah. His clinical dental treatment
is limited to oral surgery which he performs in several offices. He
is not an oral surgeon but received surgery training in the U.S.
Army where he completed a general practice residency. He has
a Masters Degree in Education.
Dr. Koerner is the author of many articles on various aspects of
oral surgery for the general dentist and has authored or co-authored four books on oral surgery. In addition, he has made several DVDs on surgery with Dr. Gordon Christensen. He is past
president of the Utah Dental Association and Utah Academy of
General Dentistry. He has received the National AGD’s Weclew
award for his contributions to dentistry and dental education and
also the Utah Dental Association’s Distinguished Service Award.
Dr. Koerner has presented hundreds of didactic and participation oral surgery courses in the U.S. and around the world. He
speaks about twice a month at dental meetings both large and
small. In the last few years, he has organized 15 dental humanitarian trips to China and enjoys studying and speaking Mandarin
Chinese.

Many extractions look easy but can soon become difficult, time
consuming, and lead to problems. This course reviews the best
techniques and instruments to remove “surgical” extractions easily and quickly while conserving bone. It makes exodontia more
enjoyable and predictable so it can be performed with greater
confidence and less stress.
Course Objectives: As a result of attending this course, the dentist will be able to:
• More easily do “surgical” or difficult extractions that frequently
occur in general practice
• Be more proficient at ridge preparation for immediate dentures
• Prevent and or manage complications that can occur during
oral surgery such as bleeding, sinus problems, etc.
• Perform predictable socket bone grafting.

**Pre-requisite: Yesterday’s Course
It is one thing to hear about new techniques but yet another to go
back to the office and start doing them on patients. Performing
new procedures on models bridges the gap between didactics
and patient care. In this workshop session, participants manage
soft tissue and use instruments and techniques they have seen
demonstrated the day before to remove “surgical” extractions--including removing broken roots. They perform single and multiple
extractions with alveoplasty and continuous-lock sutures. Other
types of sutures are also replaced by participants.

Dr. Howard S. Glazer is a Fellow and Past President of the
Academy of General Dentistry, and former Assistant Clinical
Professor in Dentistry at the Albert Einstein College of Medicine
(Bronx, NY). He has been a visiting clinician at several universities around the country. He is a Fellow of the American College
of Dentists; International College of Dentists; American Society
for Dental Aesthetics, the American Academy of Forensic Sciences, and a Diplomate of the American Board of Aesthetic
Dentistry. Dr. Glazer is an Attending Dentist at the Englewood
Hospital (Englewood, NJ). Additionally, Dr. Glazer is the Deputy
Chief Forensic Dental Consultant to the Office of Chief Medical
Examiner, City of New York.
For the past several years, Dr. Glazer has been named as one
of the “Leading Clinicians in Continuing Education” by Dentistry
Today, and most recently was named as one of the Top Dentists
in New Jersey by New Jersey Monthly & 201 Magazine. He
lectures throughout the United States, Canada, and overseas,
on the subjects of dental materials, cosmetic dentistry, forensic dentistry and patient management. Additionally, Dr. Glazer
is a frequent author of dental articles and has been published
throughout the world. Currently he publishes a monthly column
in AGD IMPACT entitled, “What’s Hot and What’s Getting Hotter!”

Dr. Pat Little is a graduate of Virginia Commonwealth University,
School of Dentistry. He launched two successful practices from
“scratch” and practiced fourteen years before disability ended his
clinical career.
After leaving clinical practice, Dr. Little decided to pursue his
interest in practice management. He enrolled at Kennesaw State
University where he obtained his accounting and general business education.
Dr. Little subsequently joined Prosperident as a Senior Fraud
Examiner and earned the Certified Fraud Examiner credential.
Through Prosperident, Dr. Little advises dentists on matters
related to fraud and embezzlement.
While maintaining his relationship with Prosperident, Dr. Little
joined Dental Team Performance as a Practice Optimization Consultant where he works with dentists and their teams in identifying key production drivers and optimizing practice performance.
Dr. Little’s goal through his lecturing and consulting is to help
dental practices achieve long-term success while maximizing

While the vast majority of dental teams are hardworking, loyal and
honest, it only takes one desperate individual to financially ruin a dental
practice. Many doctors fail to discover fraud and embezzlement until
significant damage has occurred. While internal controls are useful and
important, they have limitations and may not be as effective as the doctor thinks.
Participants will learn how to mitigate fraud and embezzlement by recognizing important warning signs associated with embezzlement and by
understanding the characteristics of an embezzler.

Dr. Bruce Cassis is a 1980 graduate of the West Virginia School
of Dentistry. He maintains a successful dental practice in Fayetteville, West Virginia, and is a clinical instructor for certification
level training utilizing the Waterlase MD er,cr: YSGG laser and diode wavelength systems. He has successfully trained thousands
of dentists in implementing laser assisted dentistry into their
practices. Dr. Cassis has earned his Mastership Certification
from both the World Clinical Laser Institute and the Academy of
General Dentistry. He has served as Chair on the Program Approval for Continuing Education (PACE) Council for the Academy
of General Dentistry, and is currently the chairperson for the AGD
Communications Council.
Dr. Cassis contributed to research conducted by Shareef Madavi,
of SM2 Consulting, and the publication, “Case Studies on Waterlase in Today’s Dental Practice, Investing in Your Patients and
Your Practice.” The article was a stand-alone publication and
was also featured in Dental Economics. Dr. Cassis also helped
in the development and clinical testing of the WaterlaseMD turbo
laser hand piece system.
Dr. Cassis is a member of the Academy of General Dentistry,
American Academy of Cosmetic Dentistry, American Dental
Association, Dental Organization for Conscious Sedation, West
Virginia Dental Association, and The West Virginia Academy of
General Dentistry. He is also active in his church and community.
He is a life-long member of the St. George Orthodox Cathedral
and is a member of the choir. He is active in the Fayetteville
Lions Club having served as a past-president in the club.

With a changing technological society, we have the goal to increase production while conserving time. We must give attention
to the tools that help us accomplish this goal. The dental laser is
definitely a product that can help achieve this goal.
Dr. Cassis will present a general review of the history of lasers,
the physics behind laser dentistry, laser safety, and review some
of the lasers available to general dentists. He will discuss how
you can successfully incorporate a laser into your dental practice. Ideally, this presentation will follow a TELL, SHOW and DO
format.
General Course Objectives:

3. Describe laser energy production and its uses in dentistry
4. Define the acronym for LASER
5. Describe the hydrophotonics phenomenon
6. Name at least four dental procedures enhanced or improved by the use of a laser
7. Safely demonstrate the use of an Er, Ce: YSGG laser on extracted teeth, pig jaws, or suitable substitutes
8. Demonstrate how to incorporate a dental laser into your

“Kill ‘em Quick, Dead and Often: Periodontal
Medicament Carriers and Oral Biofilm
Management”

Dr. Duane Keller originally developed a customized tray to
deliver medication deep into periodontal pockets for his own
patients who struggled with gum disease. Their results were so
convincing that he asked the FDA to clear the tray so he could
market it to help other dentists and their patients. FDA clearance
was issued in 2004. At the outset, Dr. Keller prescribed peroxide
gel and antibiotics for tray delivery, believing that the antibiotic
without peroxide rarely healed as well as when both medications
were used. Duane started to read microbiology journals with
renewed interest to find out why. He paid particular attention to
articles on biofilm written by Dr. Bill Costerton, a pioneer microbiologist who helped coin the term biofilm. One afternoon, Duane
called Bill and a few days later they sat together, brainstorming
research studies for Bill’s microbiology teams. With Bill as a colleague, Duane had access to new insights and information, and
dental microbiology became the basis of his periodontal treatment protocols. The paradigm shift in treatment focused away
from the symptoms to the cause of disease underpins all of Dr.
Keller’s treatment recommendations. Duane Keller graduated
from Washington University’s School of Dental Medicine in 1976
and has maintained a full time private practice ever since.
“Kill ‘em Quick, Dead and Often: Periodontal Medicament
Carriers and Oral Biofilm Management”

Periodontal disease is a classic biofilm disease that presents
significant treatment challenges. Biofilms have defensive capabilities and can adapt to treatment, making your treatment less
effective. One approach that offers better biofilm management
is the delivery of oxidizing and oxygenating agents deep into
periodontal pockets with a periodontal medicament carrier. This
is important because toothbrush, rinses, and floss cannot reach
deep into pockets and biofilm regrowth occurs rapidly. Research
shows statistically significant reductions in bleeding and pocketing when prescription tray delivery is combined with scaling.
The presentation reviews the latest research and case studies
to explain how doctors incorporate the special tray delivery into
treatment plans.
After completing this course, you will understand:
• the benefits and limitations of antimicrobial and mechanical therapies for biofilm control
• how and why antimicrobials with oxidative/reduction reac- tions effectively manage biofilms
• how to incorporate localized antibiofilm therapies into exist-
ing treatment plans
• when infected cells must be treated with surgery
• how the management or oral biofilms benefits general health and wellness

Dr. Jerome Cymerman received his DMD degree from the University
of Pennsylvania School of Dental Medicine in 1980. He did his general practice residency at the University of Miami School of Medicine/
Jackson Memorial Hospital in Miami, Florida and he completed his
Endodontic residency at the Nassau County Medical Center in 1983. Dr.
Cymerman is a Diplomate of the American Board of Endodontics. He
has lectured extensively on various topics in endodontics. His areas of
expertise include immunopathogenesis of periapical lesions, treatment
of traumatic dental injuries, microsurgical root canal therapy and the use
of cone beam computerized tomography in Endodontics. Dr. Cymerman
is an Affiliate Member of the Amercian Academy of Oral and Maxillofacial
Radiology.
Dr. Cymerman has been in private Endodontic practice since 1983 in
Stony Brook, New York. Dr. Cymerman is the Division Director of Endodontics and Program Director for the Advanced Specialty Education
Program in Endodontics at the Stony Brook University School of Dental
Medicine. His research activities have included such varied topics as
ultrasonics, immunology of periapical lesions, pain management, all of
which have been published in the Journal of Endodontics.
As a member of Real World Endo’s Board of Advisors, Dr. Cymerman is
certified to conduct lectures and seminars on Precision-Based Endodontics.
Restorative Endodontics,
A Modern Standard of Care for Long Term Success

Description: Two Part Presentation (Lecture)
Restorative Endodontics is based on minimally invasive root canal
preparations and bioceramic bonded obturation that enhance the long
term biomechanical success of the endodontically treated tooth. It (RE)
is based on a biologic approach that emphasizes debridement and
disinfection rather than shaping for obturation. The Real World Endo
presentation is highly practical and will describe (in detail) numerous facets of clinical endodontics, utilizing a “Case Based Learning Approach.”
Clinical cases will be discussed in detail that will demonstrate both the
Basic and Advanced instrumentation and Obturation Technique, as well
as some technique variations to handle more challenging caes. All participants will have the opportunity to validate these concepts during the
afternoon hands-on session.

incorporating a few new simple steps during clinical care.
Attendees will understand the importance of proper straight-line access and
the factors necessary to accomplishing the task.

Please bring a file sample from your current rotary system to compare to ESX (a size 35 is ideal.) In addition, any accessed, extracted
teeth you may want to work on.

Mr. Scott Herndon, for the tenth year in a row, will bring his
experience and enthusiasm to the 43rd Annual Review of Dentistry, instructing American Heart Association CPR classes.
Scott has over 31 years experience in occupational safety and
health and emergency response as well as experience instructing. In addition to basic life support courses, Scott has been
certified as a Fire Service Instructor and is an authorized OSHA
Outreach Trainer, routinely serving as instructor trainer at the
OSHA Training Center at the University of Alabama.
