The Embassy Suites Chicago-O’Hare/Rosemont
is an all-suite hotel located just across the street
from the Donald E. Stephens Convention Center.
All rooms are beautifully decorated with a private
bedroom and separate living room, refrigerator,
microwave, coffee maker and a queen sleep-sofa.
Additionally, the hotel offers complimentary
transportation to and from O’Hare
International Airport.
For the convenience of attendees, a limited number
of rooms have been reserved until March 4, 2009 at
a discounted rate of $105 for a King bed or $115 for
2 double beds, plus tax, currently at 13%. This rate
is not guaranteed outside the conference dates or
after March 4, 2009.

fax. If you mail the form, please keep a copy
for your records. Registration confirmations
will be issued before the meeting.
Registration fees include admittance to all
scientific sessions, conference materials,
processing of certificates of attendance or
continuing medical education, and all event
functions as specified in the program.
