Welcome to eForms
Using eForms
You can access eForms from Xpress
under the Student Records heading
using either the Financial Aid or
eForms option.
Make sure you allow “Pop-ups”
before accessing eForms
Note: First Time users please look at the
Students_First_Time_Logon.pdf for
information on your initial password and
where to find your Student I.D.
The Using_eForms.pdf provides answers to
the most commonly asked questions.
Enter your Xpress Login ID and eForm
Password you set after initial logon and the
last four digits of your Student I.D.
When this screen appears, you
have logged into your eForms
account.
DoceFill is the software provider of
eForms.
To exit eForms, click on the word
logout.
DO NOT exit by using the “X”
button to close the page or tab!
If you exit by any means other
than clicking the word “logout”
and try to get back in the system,
you may see the following image
rather than the logon screen.
If this is the case there are 3 fixes:
Open a new browser (Firefox,
Chrome, etc.)
Close and reopen your
current browser
Clear the cache of your
current browser
To access the Library
of Forms,
Click on the “Book
Shelf Icon”
When you click on the “Book Shelf Icon” the
Library of Forms is listed on the left.
Forms are broken up into Form Groups.
To complete an eForm, click on the name of
the form
The form you selected appears on the right side of the screen.
Certain fields are pre-populated on the
form such as your name, student ID and
today’s date.
Complete the form by following the
directions on each form and providing
the requested information.
There is a COMMENTS box provided on each eForm
Use this area to share any additional information
that is relevant to evaluating your request or
processing the form.
On the bottom of certain forms, there
may be a routing box. Choose the
destination that corresponds to your
campus (Peosta or Calmar).
Online Only students, please select the
Calmar campus
If you wish to submit additional documentation, you may do so by clicking on the Attachment
button at the bottom of the eForm (must be a PDF file).
You may also submit additional documents by:
Emailing as an attachment to peostafa@nicc.edu or calmarfa@nicc.edu Attachments must
be a PDF file.
Faxing to 563-557-0347 (Peosta) or 563-562-4369 (Calmar)
Bring the document to the Financial Aid office on your campus (Peosta or Calmar)
Mail the document(s) C/O Financial Aid to:

o 1625 Hwy 150 South P.O. Box 400 Calmar, IA 52132 (Calmar Campus)
“Draft Icon”
Click to view any
form saved as a draft.
Save Draft Saves a form as a draft
document to complete later
“Inbox/Received Documents Icon”
Contains all eForms that have been returned for
your review or sent to you as a follow-up document
“Submitted Icon”
Displays forms that you have submitted,
but are still in a processing phase.
You may check the current status of the
form by looking at the “History” box
located at the bottom of any form
opened from the “Submitted Icon.”
“Completed Icon”
Saves a copy of every eForm you have
submitted that has been archived.
You may check the processing history of
the form by looking at the “History” box
located at the bottom of any form
opened from the “Completed Icon.”
FAQ’s about eForms
WHAT IS AN eForm?
eForms are electronic forms available for student use. Some eForm fields are pre-populated with the
student's information.
DOES THE STUDENT PRINT THE
eForm?
No. eForms are electronic forms and designed to be completed and routed electronically. There is a print
option if you would like to keep a paper copy for your records but you do not need to print the form for our
office.
HOW IS THE eForm APPROVED BY
THE DEPARTMENT?
Once the student completes the eForm, the student clicks the 'SUBMIT‘ or ‘SEND” button. Once the Financial
Aid office receives ALL of your requested forms, your file will be reviewed.
IS ANY SPACE PROVIDED ON THE
eForm FOR COMMENTS?
Yes. eForms provides an area that allows the student and the department to comment, ask questions, or
provide additional clarification.
IS THE eForm THE SAME AS A
PAPER FORM?
No. eForms are designed to be used on a computer and utilize series of skip logic, drop-down boxes, radio
buttons, and forced-fill areas to guide the student through the form and help simplify completing the form.
CAN STUDENTS STILL USE THE
PAPER FORMS?
Once a form has been redesigned to an eForm, the 'paper' form is eliminated and students use the eForm.
HOW DOES A DEPARTMENT LOOK
AT THE eForms?
The department receives an exact copy of what you completed.
HOW IS THE eForm FILED?
Once the eForm is completed, it is routed to the student's file. There is a nightly batch job run that moves all
completed eForms into the student's file.
