ENCLOSED (in order):
1. Cherokee Nation Citizenship Application – If you have NEVER had a Citizenship card (a.k.a.
‘Blue Card’), you need to immediately apply for one using this form. The Certificate of Degree of
Indian Blood (CDIB) (a.k.a. ‘White Card’) is NOT citizenship. Without citizenship, you will be
ineligible for almost all Tribal services including Tribal scholarships and IRS requirements for the
Affordable Care Act exemption from health insurance mandates as an ‘Indian person.’
2. Request for Replacement Card or Change of Address Form – Request for replacement
Citizenship card (a.k.a. ‘Blue Card’) or Certificate of Degree of Indian Blood (CDIB) (a.k.a.
‘White Card’). The same form is necessary if you need to notify the Tribe of your change in
address which includes 911 addressing changes. If you have not received mail from the Nation
ever or in some time, please complete this form and update the Nation with your physical and
mailing address. The same form may be used to request a modification to your citizenship
including name changes (for example, marriage or divorce) and blood quantum changes including
corrections. If you need an ‘Indian Preference Letter’ (IPL) for employment or a scholarship, you
would use this form.
3. Cherokee Nation Voter Registration Form – Cherokee Nation voting is separate and
independent from voting in the State of Oklahoma elections or any other elections. ALL
Cherokees regardless of where they live are eligible to register to vote in Tribal elections as long
as they are 18 years of age at the time of the upcoming election and register before the Cherokee
Nation Election Commission’s deadlines for the election where the Cherokee becomes 18 years
of age.
4. Cherokee Phoenix (Tribal Newspaper) Subscription Form – Cherokee Nation’s Tribal
newspaper is printed monthly and will be mailed to the address of your choice for an annual
subscription fee of $10.
5. Cherokee Nation Tribal Council 2013-2015 Map and Contact Information – Understand
who your Tribal Councilmember is and how to reach them!
6. Tulsa & Rogers Counties Fridge Flier – Cara Cowan Watts has compiled and maintains a onepage flier for you to keep on your fridge, so you know where to go and how to contact your local
Cherokee Nation offices and resources. You never have to search a website, again!
7. Death Affidavit – If your loved one has passed away (even years ago) and you never notified the
Nation in writing, please stop and do so immediately using this form. Otherwise, the Nation
does not know and your loved one may still be listed as an active voter and more.

jobs, graduate fellowships, community and school grants and more for K-12 students, teachers, parents, grandparents,

Join the Cherokee Women’s Grassroots Leadership Group (CWGLG) email listserv on Yahoo Groups for news,
events and opportunities focused on strengthening our Cherokee women and their families. CWGLG should begin
meeting quarterly, again, in the coming year.

For those who take their local newspaper, Cara writes a weekly column she submits to all of the Rogers County

For Tahlequah offices and personnel, call the Cherokee Nation. The operator should be able to transfer you to any individual
or service team by name. Office hours are 8am to 5pm Monday through Friday.
For Human Services and programs such as Burial Assistance, General Assistance, Imminent Need Programs and other
social service programs, Brittany Denny is the contact for Tulsa and Rogers County. Ms. Denny can be reached at the Catoosa
office Monday through Wednesdays at 918-266-5626, ext. 7778. This is apt to change should Ms. Rider be called away on tribal
business, so it is not set in stone. Office hours are 9:30am to 3:30pm Monday through Friday.
For Career Services and Job Assistance, please contact the Claremore Career Services office. Office hours are 8am to 5pm
Monday through Friday.
