Revision Officer (RO) Placemat Who can vote? A Canadian citizen who is at least 18 years of age, and lives in Brampton, or owns or rents property in Brampton or is the spouse of a person who owns or rents property in Brampton According to the voter’s name and current address: Check the Voters’ List – is the voter on the List for this location? A voter must vote at the voting location assigned based on where he or she lives or owns property, as of the date of voting Is the voter’s information correct? Yes No Yes Assist voter to complete Application to correct any of: Address (moved from one address to another within Brampton) Name (name change or spelling) School Board support information **Confirm identification (already on the list so only require proof of qualifying address and name) No Verify that all fields are completed (including school board support). Is the voter on the list for another location? No Is the voter at the correct location? Yes (use Voters’ List and/or street guide to determine) Yes Assist voter to complete Application to add and check their identification for voter eligibility confirming: 1. proof of Canadian Citizenship (see list on opposite side) 2. proof of age (18+) 3. proof of qualifying address for voting location Have applicant read Declaration of Applicant, confirm verbally that information is true, and have applicant sign Application. There are NO EXCEPTIONS to this requirement. The applicant must provide proof for all 3 eligibility requirements, DO NOT sign the Application until these requirements have been met. When satisfied everything is complete and correct, Revision Officer will sign and date under “For Election Official Use Only”. Note: You can only add a person to the Voter’s List if he/she is qualified to vote at that voting location (person’s address is assigned to your voting location) – otherwise direct them to their designated location. No Applicant wants to delete his or her own name or deceased spouse’s name Assist applicant to complete Application Have applicant read Declaration of Applicant, confirm verbally that information is true, and have applicant sign Application. A person can only delete his/her own name, unless the person to be deleted is deceased (accept the Application to delete if person advises their spouse is deceased – no death certificate required). When satisfied everything is complete and correct, Revision Officer will sign and date under “For Election Official Use Only”. Provide original Application to a DRO to place in Envelope E.
