Vignette: " Borderline Bob is the first to say something when the
donuts for the office do no arrive. 'Well, that's the last time I ever
order from Dirk's Donuts,' he growls. 'The food isn't even very
good!' His friends are silent remembering that only last month,
Bob was praising Dirk's for having the best donuts in the city. Since
Bob has borderline personality disorder, he often engages in this
type of black-and-white thinking, or splitting. Everything is either
all-good or all-bad, and Borderline Bob swings wildly between the
two. As a result, many of his relationships with people tend to be
unstable. He's actually been known to go out and get a beer or two
with Dirk, but is now ranting and calling him a 'donut-shilling dirt
bag!"
Definition: People with this disorder tend to see things "Black and
White." It's either one way or the other. (Ex: Good/Bad) They have
long term patterns of turbulent/unstable emotions.
Symptoms:
 Fear of being abandoned
 Feelings of emptiness and boredom
 Frequent displays of inappropriate anger - anger at wrong things
 Impulsiveness with money, substance abuse, sexual relationships,
binge eating, or shoplifting
 Intolerance of being alone
 Repeated crises and acts of self-injury, such as wrist cutting or
overdosing


Theories of Development: The causes unknown. Genetic, family,
and social factors are thought to play roles. Increasing risk include:
being abandoned as a child, sexual abuse, poor communication w/
family, and disrupted family life.

Vignette: A 27-year-old banker who had already earned his first
million had developed a false story about his upbringing in a
foreign country. The story evolved from a series of non delusional
“sliding away from the truth,” which are frequently used by
narcissistic patients to support their grandiosity. Fully aware of its
falsehood, he elaborated on the story to friends and
acquaintances, as well as to the woman with whom he lived and
planned to marry. On introducing her to his family, however, his
duplicity was (predictably) revealed, and he entered treatment
ashamed and panic-stricken after the loss of his admiring
girlfriend. Although he acknowledged that her anger with him was
justified, he was shocked with self-righteous fury when she
actually decided to leave him.
