Smith Center for Women’s Cancers at Dana-Farber provides comprehensive care for women with all types of breast cancer, including ductal carcinoma in situ, infiltrating (invasive) ductal carcinoma, lobular carcinoma in situ, infiltrating (invasive) lobular carcinoma, inflammatory breast cancer, Paget’s disease, and phylloides tumor. We offer specialized expertise for patients with metastatic breast cancer or breast cancer during pregnancy, as well as an Inflammatory Breast Cancer Program, and the Program for Young Women with Breast Cancer which addresses the unique needs of women under 40 who are diagnosed with breast cancer. Experts in the Cancer Genetics and Prevention Program can also create a personalized plan for women at higher risk for developing breast or ovarian cancer. Patients receive care from a multidisciplinary team of health care professionals with extensive experience and expertise in the unique needs of women with breast cancer. This includes medical, surgical, and radiation oncologists specializing in breast cancer, as well as radiologists, pathologists, reproductive medicine specialists, plastic surgeons, oncology nurses, genetic counselors, and more. Clinical research is a vital aspect of our mission at the Breast Oncology Program. We believe that outstanding clinical care is directly linked to an active program of clinical research. Physician-researchers take the questions that arise from their experiences in the clinic, answer them through clinical trials, and take this new knowledge back to the clinic, where it can benefit patients. At any given time, dozens of research studies involving hundreds of patients are under way within the Breast Oncology Program. These studies are broad in scope: investigations of prevention and early-detection strategies, new therapies, novel combinations of existing treatments, quality-of-life studies, and laboratory analyses of tumor samples to better understand the relationship between tumor types and patient outcomes. Our team is also leading the way in treating breast cancer as a complex family of diseases rather than a single disease. All tumors are assessed for molecular markers that guide therapy and help determine eligibility for clinical trials exploring innovative new therapies. Our team provides consultation and second opinions, evaluation and management, prevention and early detection counseling, genetic counseling and risk assessment, screening of high-risk individuals, and histopathologic diagnosis and review. The Cutaneous Oncology team also works closely with the Melanoma team in the diagnosis and treatment of pigmented lesions. Patients are cared for by a multidisciplinary team of specialists and have access to specialized treatments, including Mohs surgery and other skin surgeries, immuno- and chemotherapy, radiation therapy, and photopheresis. In addition, patients may have the opportunity to participate in the numerous clinical trials evaluating new therapies. Research performed by our faculty is at the cutting edge of oncology, cell biology, and immunology as they relate to skin. Many of our doctors are involved in the Specialized Program of Research Excellence (SPORE) in Skin Cancer grant, awarded by the National Cancer Institute to promote interdisciplinary translational research and to facilitate the exchange of information between the scientists who study skin cancer and the physicians who treat it. Additionally, the program leverages the resources available through the Harvard Skin Disease Research Center, one of six such centers nationally that investigate a variety of skin diseases, including cutaneous oncology. In this program, specialists who are researching exciting new ways to kill tumors or halt their growth conduct small, carefully designed studies of experimental drugs that are in early phases of development. These trials include Phase 1 dose escalation studies, as well as studies confirming proof-of-mechanism of novel agents and preliminary efficacy studies in patient populations with tumors harboring specific genetic alterations. Patients are cared for by a multidisciplinary team of specialists who use advanced, innovative techniques to diagnose and treat gastrointestinal malignancies. Our team is a leader in delivering a personalized treatment approach for many gastrointestinal malignancies. All colorectal cancers are tested for KRAS and BRAF mutations, with therapy tailored according to the tumor genotype. In addition, treatment for patients with esophageal and gastric cancers is provided based on each patient’s individual HER2 amplification. The Center offers genetic counseling and testing to patients with a family history of gastrointestinal cancers. This program also conducts research to understand the genetic causes of cancer and to evaluate new diagnostic tests for early detection, novel endoscopic techniques, and new medical therapies to prevent gastrointestinal cancers. Specialists evaluate and treat all types and stages of head and neck cancers, from early lesions to rare and challenging cases. Our team has extensive experience in managing the complex treatment required for head and neck cancer patients. Care is coordinated by a team of clinicians specializing in neuro-oncology, neurosurgery, radiation oncology, psychosocial oncology, nursing, neuropathology, neuropsychology, social work, and genetic counseling. It is focused on optimizing people’s function and comfort in the shadow of a limited life expectancy in all of the domains that make us who we are: physical, emotional, spiritual, sexual, social and financial” (Ferris FD, et al. J Clin Oncol 2009; 27:3052-8). Palliative care is not limited to care at the end of life — that is hospice care. Palliative care is appropriate for all cancer patients, from diagnosis on, and we focus both on helping them and providing support for their families. Our inpatient and outpatient adult palliative care teams include expert physicians, nurse practitioners, physician assistants, nurses, social workers, pharmacists and chaplains. Because most patients require therapy that includes a combination of chemotherapy, radiation, and surgery, care is coordinated among a specialized team that includes thoracic surgeons, medical oncologists, radiation oncologists, pulmonologists, radiologists, pathologists, nurses, physical therapists, respiratory therapists, dieticians and nutritionists, and social workers. We perform routine genomic testing of tumors from all patients with advanced non-small cell lung cancer using the latest next generation sequencing methods to evaluate for changes in over 300 genes. We offer a wide variety of therapeutic approaches and clinical trials of novel therapies based on the genomic makeup of the individual patient’s cancer. We have special expertise treating patients and families with all forms of non-malignant blood disorders, including anemia, bone marrow failure syndromes, bleeding problems, blood clots (thrombosis), platelet disorders including idiopathic thrombocytopenic purpura (ITP), sickle cell disease, and white blood cell disorders.
