The Service for Adults with Attention Deficit Hyperactivity
Disorder/Autism Spectrum Conditions (ADHD/ASC) is an
established service within the Specialist Services Directorate of
South West Yorkshire Partnership NHS Trust, currently serving
the Wakefield, Kirklees, Barnsley and Calderdale Localities.
The Service accepts referrals for adults from the age of 18
upwards that do not have an additional Learning Disability. The
team comprises a Consultant Psychiatrist (Clinical Lead),
Specialist Doctors, Nurses, Occupational Therapists, Social
Workers and Senior Pharmacist. The Service is based at
Manygates Clinic in Wakefield with additional clinic sites across
localities.
The Service has established diagnostic and intervention
pathways including the Kirklees Adult ASC Social Care
pathway. The social work role includes being part of the ASC
diagnostic assessment process and responsibility for the care
co-ordination of social care assessments and interventions,
specialist advice and support.
You will demonstrate a commitment to partnership working to
enable effective social care for adults in Kirklees with a
diagnosis of ASC and ADHD. In return for social work skills and
experience we can offer training and development opportunities
within a dynamic team and friendly environment.
You must hold a HCPC recognised Social Work Qualification
and be registered with the HCPC. You will have experience
working with people with mental health problems, gained
through paid/voluntary work or practice placement. You will hold
or be prepared to work towards an Approved Mental Health
Practitioner (AMHP) qualification.
Travel is an essential part of this job and therefore it would be
extremely difficult for you to do your job effectively if you did not
use a car for all or most of the time. It is essential that you hold

Based at Perseverance House in Huddersfield, you will
manage a group of key projects helping KNH to retain its
rating as one of the top performing housing organisations in
the country. The projects you will manage will provide you
with an excellent opportunity to develop your skills as a
Project Manager, as well as directly improve KNH services.
