Philippine Academy of Family Physicians
The Publications Committee is contemplating on shifting to an electronic format
for our scientific journal, the Filipino Family Physician. In order to minimize the
cost of printing and at the same time to keep abreast with current journalistic
practice of publication, the PAFP recommends using the electronic format.
We would like to know your thoughts about this. Please complete the survey form
below. This is to assist us in our decision as to the best way to protect the interest
of the organization and carry out needed reforms to address current financial
concerns without sacrificing our services to the members.

units. PAFP Members should fill up and submit it in designated counters.
This is a requirement for the issuance of certificates.

Members and pre-registered delegates need not fill up any form for
registration. They have to go directly to these respective counters.

GENERAL GUIDELINES FOR ALL

No I.D., no entry will be enforced in all activities. A replacement fee of
Php500.00 will be charged for lost I.D. cards.
6.
Original receipt of pre-registered participants can be claimed at the
designated area.
2.
The convention strictly enforces a no-smoking policy.
7.
As our contribution to environmental protection, a drop box shall be available
for returned convention ID plastic holders.

CHILDREN ARE NOT ALLOWED TO ENTER THE CONVENTION
SITE (based on PICC policies).

HOSPITALITY AREAS WITH CORRESPONDING BOOTH PASSPORT.

for a permanent ID that will be issued for CME accreditation and registration

For the luncheon symposia: FIRST COME, FIRST SERVED BASIS,
SEATING CAPACITY ONLY, NO RESERVATION POLICY will be
implemented.

Only registered participants with proper convention IDs shall be allowed
entry to the Luncheon Symposia Rooms.
5.
Once you have entered, OCCUPY your seat, No seat reservation shall be
honored. All unoccupied seats shall be considered vacant.
6.
Once lecture presentations have commenced, doors shall be closed and
meal ticket shall be distributed to the seated guests.
7.
After the lecture, meals shall be served to meal ticket holders.

GUIDELINES AT BOOTH EXHIBITS

Pharmaceutical Hospitality Areas and Booth Exhibits are located at Forum
1 and Forum 2 of the Philippine International Convention Center. Entry to
the exhibit area shall be allowed only on the following schedule:

Passports to Pharmaceutical Hospitality Areas and Booth Exhibits are already
incorporated in the souvenir program. Completely marked passports will be
eligible for raffle on the third day of the convention. A tambiolo will be
provided in the Exhibits Area.
4.
LOST PASSPORTS will not be replaced. Photocopied Booth Passports will
not be honored.
5.
When you visit the booths and hospitality areas, you are allowed to bring
only one booth passport.
6.
Please understand that you will be given one give away per booth/ hospitality
area. PLEASE DO NOT ASK FOR MORE.

Children and accompanying persons, even if they are registered

30, 2014. Late payment automatically cancels Pakikiramay Benefits. Said
benefits will only be reinstated 90 days from the date late dues were settled.

IV. SPECIAL ASSESSMENTS
May we ask you to help in the collection of the Special Assessment Fee of
(Php3,000.00 which may be paid in three installments (Php1,000 per year) or
using any payment scheme your chapter implements. (starting 2012)
Senior members are exempted from this special assessment.
Members who are 70 years old and above are given 20% discount on this
assessment.
