8 Grade Celebration th REACH FOR THE STARS Friday, April 17, 2015 6:00 pm - 9:00 pm WoodCreek Junior High Information Packet This packet includes all information and required forms. The WoodCreek Junior High Class of 2015 is recognizing their success with a celebration – Reach for the Stars – on Friday, April 17, 2015, from 6 - 9pm at WoodCreek Junior High. Please read all attached forms carefully. The attached forms: Guidelines Dress Code Permission Slip/Payment Form - Fee per student: Early Bird Special $20 (Jan. 20 - Feb. 27) Receive a FREE CELEBRATION T-SHIRT with the Early Bird Special! Full Price $25 (Mar. 16 - Apr. 2) Early Bird Special ticket sales begin January 20, 2015. When you purchase an Early Bird Ticket (Early Bird sale ends Feb. 27) your student will receive an EXCLUSIVE 8th Grade Celebration T-Shirt for FREE! Payment and permission slip may be dropped off at the WCJH front office in an envelope clearly marked “8 th Grade Celebration.” We will be in the cafeteria during lunch periods to receive payment and permission slips on select Tuesdays and Thursdays (Feb. 10, 12, 17, 19, 24, 26, Mar. 24, 26, 31, Apr 2.) Students will NOT be able to purchase a ticket without a permission slip. An email receipt will be sent to you. An entry pass to the dance will be given to the student at a later time. Regular priced ticket sales will take effect after February 27, 2015. Ticket sales will end April 2, 2014. This information is also available on the PTA website at www.wcjhpta.com. Additional Information Packets will be available at the WCJH front office. 8th GRADE CELEBRATION GUIDELINES To ensure that the dance is a successful and safe event, the following guidelines have been set by Mrs. Finnesand, Mr. Stroud and the 8th Grade Celebration Committee. Permission Slip/Payment Form and Fees: In order to attend the celebration, the student and one of his/her parents or guardians MUST sign the attached permission slip, which states that you have read, understand and agree to follow the guidelines. Permission slip and fee are due at the same time. Both must be turned in no later than Thursday, April 2, 2015. If turning in to the WCJH front office, please place in an envelope clearly marked “8 th Grade Celebration”. Once payment and permission slip are received and recorded, you will receive an email receipt from a Celebration Committee Member. Invitations: All students who have turned in their signed permission slip and paid their fee will receive a numbered entry pass to the celebration. Drop Off and Pick Up: LIMOUSINES WILL NOT BE ALLOWED ON THE SCHOOL GROUNDS. DRESS CODE COMPLIANCE WILL BE CHECKED IMMEDIATELY UPON ENTERING THE CELEBRATION. The celebration will end promptly at 9:00 pm. You must arrive on time to pick up your student. Security: During party hours, your student may leave only in the company of his/her parent or another parent that is designated in advance. A signed note must be on file with our Check-in Committee. Show of ID will be required. Photos: The Decorating Committee is requesting a 4x6 sized baby photo of your student. Please write his/her name on the back of the photo. Turn this in with your payment and permission slip. Photos may not be returned. 8th GRADE CELEBRATION DRESS CODE This policy will be strictly enforced. If there are any questions regarding the dress code, please contact Mr. Stroud, 8th Grade Assistant Principal, prior to the night of the celebration. No strapless dresses unless covered with a shawl or jacket Spaghetti straps will be allowed Halter-top dresses or tops are allowed, but CAN NOT be bare back The front and back neckline must not come lower than the top of the armpits when arms are extended straight out See-through mesh will NOT be allowed Dresses may NOT be shorter than mid-thigh length and any type of slits in the dress may not be above the mid-thigh NO midriffs are to show, which includes any two-piece outfits that would reveal the midriff when arms are raised NO cutouts are allowed in the midriff area All pants must be worn at the waist. Cannot be excessively baggy No jeans or shorts will be allowed Shirts or blouses may not be oversized or immodestly undersized Suit and sports coats are not required Please take the time to consider all of the above as students who come dressed inappropriately will NOT be allowed to enter the celebration. PERMISSION SLIP AND PAYMENT FORM Return this page with payment & photo The student named below has my permission to attend the 8th Grade Celebration on Friday, April 17, 2015, at WoodCreek Junior High School. We have read, understand and agree to follow the celebration guidelines. If my student does not comply with the set guidelines, he/she will be asked to leave the celebration and I will be required to pick up my student immediately from the celebration. I understand photos will be taken during the event and will be made available online via QR code. I understand that both a signed permission slip and the fee (Early Bird Special $20 by February 27, 2015, or $25 by April 2, 2015) must be received by Thursday, April 2, 2015, or my student will not be allowed to enter the celebration. In addition, I understand that fees are non-refundable. **Please attach a 4x6 sized baby photo of your student with his/her name written on the back. Photos may not be returned.
