SCHOLARSHIP APPLICATION FORM
PURPOSE: To provide scholarships to deserving Wylie High School graduating seniors
intending to pursue higher education at a college or university.
AWARD COMPONENTS: Two (2) $2,500 scholarships awarded to two (2) students
(one male, one female) selected by the representatives of BulldogsLiveOnline.com along
with Wylie guidance counselors and coaches. (BLO reserves the right to extend the
number of students should the quality and quantity of applicants deem it necessary).
CRITERIA:
1. Applicant must be a Wylie High School student active in the athletic department.
2. Applicant must be a graduating high school senior in the year of the award.
3. Applicant must be a enrolling full-time in an accredited college or university.
4. Applicant must demonstrate scholarship—in the complete sense—consistent with
capacity and circumstances.
5. Applicant must demonstrate both leadership and ethical responsibility.
6. Applicant must demonstrate financial need.
Deadline for application is Friday April 3, 2015. Applications received with a
postmark of April 4th or later will not be considered.

Should any of the chosen recipients of the scholarship fail to attend an accredited college
or university the following fall term, then the applicant who was judged as an alternate
will receive the award if he or she has enrolled in an accredited college or (confirmation
of college or university enrollment will be requested prior to funding the award).

WORK EXPERIENCE

ESSAY
Attach a 300-500 word essay as a separate page that explains why you desire to attend
college and how receiving this scholarship will impact your ability to attend school. What
are your career interests/goals and what has motivated you toward this direction? What
do you hope to accomplish in your life? Add any information about yourself you feel is
important for the scholarship selection committee to know about you. If there has been a
major obstacle in your life you have had to overcome, you might consider discussing this.
Consider this essay as a way to let the committee know what type of person you are and
why you deserve this award. Essay must be signed by applicant to indicate it is the
applicant’s original work. Integrity is essential to success.

(coaches, teachers, youth pastors’, community service supervisors, etc.)
