Also limited to only 2 food items. (Ex. Kettle corn & water, lemonade & popcorn, etc.) Only five spots available that will be given on a first come first serve basis, Total Emitted: NOTE: _______ 1) Temporary Food Establishment Permit: All food vendors are required to fill out a temporary food permit through the health department and, if required, pay the application fee. It is the vendor’s responsibility to contact the health department at 828-287-6104 at least 2 weeks prior to the show. (For our car show the deadline is Friday July 31st, 2015) Please notify the Forest City Merchants Association after you speak with the health department. 2) Event Fire Prevention Permit: All food vendors are required to fill out an Event Fire Prevention Permit and return it. The fire department will conduct a safety check at each booth using these permits during the setup of the show. FOR YOUR CONVENIENCE BOTH APPLICATIONS ARE ATTACHED TO THIS APPLICATION. 3) You must list every food item that you are going to sell. If you show up the day of the event with a food not listed on this form the Forest City Merchants Association has the right to refuse sells of these items. Forest City Merchants Association also has the right to refuse vendors special events permit. 4) Vendors Fees are NON-REFUNDABLE. The Car Show is held RAIN or SHINE! 5) Vendors are responsible for supplying their own tents, tables, chairs, signage, garbage bags, etc. 6) Set up time starts at 5:30am and all booths MUST be set up by 9:00am for Health Department and Fire Department Inspection. If your booth is located on Main Street you must be in your booth location by the time the street closes at 7:00am and be ready for inspection by 9:00am. 7) For the safety of show participants no Main Street or Food Court Vendor is allowed to leave until after the 7:00pm Trophy Ceremony. By signing below I hereby agree to abide by the rules and regulations set forth by the Forest City Merchants Association. I understand and agree that there will be no refunds and that decisions of the Forest City Merchants Association are final. Furthermore, I hereby release and forever discharge the Forest City Merchants Association and their agents and representatives, from any responsibilities, personal liability, loss, claims, or damage arising out of or in conjunction with this event. THIS, AND THE FIRE PERMIT, ALONG WITH PAYMENT FOR THE 24TH ANNUAL HOT NIGHTS COOL RIDES CAR SHOW MUST BE MAILED TO THE FOREST CITY MERCHANTS ASSOCIATION AND MUST BE RECEIVED BY FRIDAY JULY 31ST, 2015. DUE TO HEALTH DEPARTMENT RESTRICTIONS, APPLICATIONS RECEIVED AFTER THIS DATE WILL NOT BE ALLOWED INTO THE SHOW.
