When you purchase a qualifying Poulan PRO mower between March 2 – March 28, 2015
Consumers that purchase select Poulan PRO tractors, zero-turn mowers or walk behind mowers between
March 2 and March 28, 2015 will recieve a $30, $100 or $200 reward from Poulan PRO. Qualifying models include
and are limited to: PB22VA48 Tractor, PBGT2654 Tractor, P54ZX Zero-Turn Mower, P54ZXT Zero-Turn Mower and
PR700AWD Walk-Behind Mower. Simply fill out this form, including the model information and follow instructions to recieve
a reward from Poulan PRO. Reward will be issued in the form of Poulan PRO Visa® Prepaid Card(s) mailed to the Consumer
listed below. Submission must be postmarked by April 28,2015.
To qualify for the reward:
1. Complete and submit this form with all fields completed.

ttach a copy of your sales receipt for the qualifying units purchased. Please verify the purchase date is within the offer start and end
dates shown above and is clearly visible on the receipt. Please circle the qualifying units on the receipt.

ttach proof of product registration with Poulan PRO. This can be submitted as either a screen shot of the product registration
completed on-line or a copy of the registered warranty card.
4. Mail form, product registration(s) showing proof of purchase and sales reciept(s) no later than April 28,2015.
