HOPE can GET YOU ON YOUR WAY! When the crisis is over, and an individual is being discharged from the Crisis Response Center, they may have questions like: What do I do next? Where will I go now? How do I get there? Where will I live? Can someone help me to understand the discharge process? Located in the CRC, the S.P.A.N. staff can help! For more information, please call (520) 301-2249 or (520) 770-1197. www.hopetucson.org info@hopetucson.org HopeIncTucson The S.P.A.N. team, located in the Crisis Response Center (CRC), provides peer and family support services for individuals discharged from the Crisis Response Center during their after-crisis care. Recovery Navigators offer assistance in discharge and recovery planning and offer peer and family support services. To offer support and connection to resources in our community to people as an alternative to court-ordered evaluation and hospitalization for a mental health crisis To promote person-centered recovery planning through peer support To emphasize safe re-entry into the community for those experiencing crisis Residents of Pima County who have been discharged from the CRC Individuals in need of transition services Individuals and family members seeking peer support Call (520) 301-2249 or (520) 770-1197 At the Crisis Response Center, ask to speak to SPAN Staff HOPE, Inc. receives funding from the Community Partnership of Southern Arizona, Pima County, Mercy Care Plan, Old Pueblo Community Services, and from contributions made by members of our community.
