2015 Jeff Milano-Johnson Regional Lacrosse Tournament Registration Package Contents Include: Tournament Details Team Registration Form Team Waiver/Release Form Tournament Rules About the JMJ Tournament The Jeffrey Milano-Johnson Regional Lacrosse Tournament is an event celebrating the life of Jeff Milano-Johnson and supports Rochester Kids, the non-profit organization that sponsors the tournament. Rochester Kids provides scholarships and support to city kids including support for inner city lacrosse teams. The JMJ Regional Lacrosse Tournament was established to honor Jeff MilanoJohnson who died on April 11, 2007 at the age of 14 from a cerebral aneurysm while warming up for a Pittsford Varsity Lacrosse game in Spencerport, NY. The Tournament aims to keep the spirit of this vibrant young man alive. Jeff’s zest for life and dedication to the sport of lacrosse inspired his teammates, coaches, friends and all those with whom he came in contact. The JMJ Memorial Lacrosse Tournament for 7/8 and 9/10 grade boys lacrosse teams that have completed their spring seasons is traditionally scheduled for the last weekend in May or the 1st weekend in June of each year is sponsored and managed solely by JMJ Fund/Rochester Kids and not by any other community lacrosse program OR by any member of the Section 5 athletic organization is one of the primary fundraising activities for JMJ Fund/Rochester Kids \ Additional details about the tournament and Rochester Kids can be found at jmjfund.org or rochesterkids.org JMJ Tournament Details Date & Time: June 7, 2015 (8:00 am to 4:00 pm) Location: Unity Health Systems Total Sports Experience 800 Elmgrove Road, Rochester, NY 14624 Level: Boy’s 7/8 and Format: The tournament will be a 10-on-10 format. Each game will be 25 minute running time. Each team will be guaranteed four games with the winner of each division awarded a trophy. Cost: $330 per team 9/10 Grades -Deposit of $100 due with team registration form no later than 5/1/14 -Balance of $230 due no later than 5/19/14 Payment: Make checks payable to: JMJ Memorial Fund Send payment: Chuck Lamb 5 Locke Drive Pittsford, NY 14534 Logistics: 1. Standard NYS Boy’s High School Lacrosse rules apply along with the following modifications listed below. 2. Format is 10 on 10 (3-3-3 + Goalie). 3. Each team will play 4 games. The winner of each section will be awarded a trophy based upon points. a. Won-Loss Record b. Head to Head Record c. Goal Differential (Total All 4 Games – max 7 per game) d. Goals Allowed (Total All 4 Games – lowest #) e. Coin Toss 4. Each team must have an adult coach (21 years of age or older) who will be in charge of team discipline and act as the team's representative. 5. Each game consists of two 12.5 minute running time periods. Time is kept by the referee on the field. 6. Penalties are called and served as they are in a regular field lacrosse game. The referee releases penalized players when their serve time has expired. Penalty time does not start until the penalized player takes a knee, out of bounds, at the midline, on the bench side of the field. 7. A game ending tied is decided by a single 4 minute sudden-death overtime, which begins immediately after regulation time with a faceoff. If the game is still tied at the end of the sudden death overtime period, the game will count in the standings as a tie. 8. Each period begins with a faceoff. 9. After a goal is scored the ball is awarded to the scored upon goalie who puts the ball in play from his crease area. A referee must whistle to indicate play has recommenced. If a team falls behind by 6 or more goals, they are awarded the ball at midfield after they've been scored upon. 10. All player substitutions are on the fly. Players must touch sticks at the intersection of the midline and the sideline before entering the field of play. 11. Each team is allotted one timeout per half and one in overtime. Timeouts are one minute long. 12. Team members must wear matching jerseys or tank tops which are numbered. 13. ANY PLAYERS OR COACHES INVOLVED IN A FIGHT WILL BE EXPELLED FROM THE TOURNAMENT. No second chances. The JMJ Tournament has ZERO tolerance for fighting. 14. Flagrant fouls: 1st offense - expulsion from game; 2nd offense (occurring anytime during tournament after first offense) - expulsion from tournament. ACKNOWLEDGE, agree and represent that I understand the nature of Playing LACROSSE and that I am qualified, in good health, and in proper physical condition to participate in such Activity. I further agree and warrant that if at any time I believe conditions to be unsaid, I will immediately discontinue further participation in the Activity. 2. FULLY UNDERSTAND THAT (a) Playing Lacrosse INVOLVES RISKS AND DANGERS OF SERIOUS BODILY INURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, AND DEATH (“RISKS”); (b) these Risks and dangers may be caused by my own actions or inactions, the actions or inactions of others participating in the Activity, the condition in which the Activity takes place, or THE NEGLLIGENCE OF THE “RELEASEES” NAMED BELOW; (c) there may be OTHER RISK AND SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSEES, COSTS, AND DAMAGES I incur as a result of my participation or that of the minor in the Activity. 3. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE Rochester Kids/Jeff –Milano Johnson Memorial Fund, Inc., their respective administrators, directors, agents, officers, members, volunteers and employees, other participants, any sponsors, advertisers, and if applicable owner or lessors of premises on which the Activity takes place (each considered one of the “RELEASEES” herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE “RELEASEES” OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my behalf, makes a claim against any of the Releasees, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage or cost which may incur as the result of such a claim.
