Active, full-time associates working at least 30 hours per week are eligible for benefits. Cadence Bank offers a comprehensive benefits lineup that provides associates the opportunity to design their own benefits program to keep benefits in sync with their needs as well as the needs of their families. The information contained in this document is meant to give you an “At-A-Glance” look at available benefit plans and essential details to get you started in determining the best fit for you. Health Insurance Plans Cadence Bank offers three PPO health plans for you to choose from. They are all administered by Blue Cross and Blue Shield of Alabama. BCBS CDHP Premier Plan BCBS CDHP Core Plan BCBS Base Plan A Consumer Driven Health Plans (CDHP) offer lower premiums than the Base Plan, but deductibles and co-insurance amounts are higher. For a comparison of the three available plans, please refer to the chart below. Every calendar year Every other calendar year Lenses: Single vision, lined bifocal, and lined trifocal lenses, Polycarbonate lenses for dependent children Premium progressive lenses 50% of UCR** $10 The Cadence Bank EAP program is a confidential assessment, short-term counseling and referral service for all regular associates and their eligible dependents that need help in areas including: Marital and Family Issues, Alcohol and Other Drug Dependency, Stress Related Issues, Financial/Legal Referrals, Emotional Problems, and Personal Growth. All counseling offered through the EAP is provided at no cost to you up to 6 sessions a year. Flexible Spending Accounts (FSA) Life Insurance Administered by Liberty Mutual Coverage is effective with date of hire. Cadence Bank provides associates with Basic Life and Accidental Death & Dismemberment insurance at 2X their annual salary, rounded to the next highest $1,000. Voluntary Term Life Insurance is available for you to purchase for yourself, your spouse and/or your children. Disability Insurance Cadence Bank provides associates with Basic Long-Term Disability Insurance. The maximum monthly benefit is equal to 60% of salary, up to $10,000. If 60% of your monthly salary exceeds $10,000, you may be eligible to purchase additional LongTerm Disability coverage to increase your monthly benefit up to a combined maximum of $15,000. Short-Term Disability insurance is available for you to purchase and it provides a maximum weekly benefit of 60% of your salary up to $1,000 per week for a maximum benefit period of 13 weeks. Administered by TASC Coverage is effective the first day of the month following your date of hire. A Flexible Spending Account allows you to set aside pre-tax dollars to pay for eligible health care and dependent care (daycare) expenses. In 2015, you may contribute up to $2,550 into a Health Care FSA and/or $5,000 into a Dependent Care FSA. TASC also offers the following: TASC Visa Prepaid Card enabling access to FSA funds at the point of purchase TASC mobile app allowing the user to check their FSA balance, request reimbursements, and check the status of reimbursement all from your smart phone Health Savings Account (HSA) Coverage is effective the first day of the month following your date of hire. Associates electing a Consumer Driven Health Plan (CDHP) are eligible to contribute to a Health Savings Account. An HSA is similar to a Health Care FSA but offers so much more. Payroll contributions are withheld on a pre-tax basis but the following features are only available through an HSA: Can be used as a long-term savings vehicle for medical expenses in retirement You own the account – it’s fully portable (like an IRA) 2015 Annual Contribution from $3,350/single to $6,650/family with additional $1,000 annual catch-up contribution allowed for participants age 55-65 Year-end balances carry over from year to year Cadence will be contributing seed money (pro-rated based on date of hire) to an HSA for those associates electing a CDHP in 2015. Cadence Bank 401(k) Plan Administered by Fidelity Associates (age 21 and older) are eligible to participate in the Cadence Bank 401(k) Plan. Associates can contribute from 1% to 50% of eligible compensation. The maximum dollar amount that can be contributed is $18,000.00 for 2015. Cadence will match 100% of the first 3% in eligible compensation deferred and 50% of the next 2% in eligible compensation deferred. The match is 100% vested immediately. Associates that are age 50 or over can contribute an additional $5,500.00 annually as a catch-up contribution. Cadence Bank Benefit Time Information Vacation Policy Full-time associates earn vacation based on the following guidelines: Calendar Service Year Non-Exempt Exempt Up to 5 years 10 Days 15 Days 5 to 15 years 15 Days 20 Days 15 or more years 20 Days 20 Days Part-time associates who are scheduled to work 20 or more hours weekly are eligible to earn one-half of the stated guidelines. Cadence Bank Holiday Schedule Cadence Bank observes the following holidays: New Year’s Day Martin Luther King, Jr. Day President’s Day Memorial Day Associates at the Vice President level and above are eligible for 20 days of vacation per calendar year. Associates hired prior to September 1st are eligible to receive vacation based on each full month of employment worked. Associates hired after September 1st are not eligible for vacation until January 1st of the following year. Independence Day Labor Day Columbus Day Veteran’s Day Thanksgiving Day Christmas Day Sick Time Accrual Full-time associates who are scheduled to work 30 or more hours per week will be awarded one day of sick time per month, with a maximum accrual of 65 days. Part-time associates scheduled to work 20 or more hours per week will be awarded ½ day per month, with a maximum accrual of 32.5 days. In addition to the above holidays, Cadence Bank also grants one floating holiday per calendar year to all associates who are employed prior to January 1st. Cadence Bank recognizes that we have a responsibility to the communities we serve. Many of our associates are leaders in these communities, serving on boards, working with non-profits, volunteering much of their personal time to call attention to causes or support civic organizations. The Cadence Cares program encourages associates to perform local volunteer work – one day per calendar year, with pay – to support an organization or cause that contributes value to the community. Eligible associates include full-time and part-time associates who have been employed at least 90 days (Cadence Cares day is pro-rated for part-time associates).
