LAST WEEKS OF SCHOOL AND CLEARANCE DETAILS FOR SENIORS Advisory – Work from previous trimesters/years should be turned in immediately to allow ample time for review and grade changes to be processed. The later outstanding Advisory work from previous trimesters is turned in, the longer it will take to clear! Scholarship Information – For seniors who have received scholarships, the information is due to the Career Resource Center by May 13 to ensure it is included in the Awards program as well as the Graduation program. Announcements/Invitations – may be purchased through Jostens. If you have questions regarding announcements, please contact Jostens at 952-974-0800 or email Nicole.mareck@jostens.com. Behavior – Any student planning to participate in commencement who becomes involved in planning and/or participating in any inappropriate, disrespectful, illegal and/or unsafe activities on the school premises and/or at school activities, may lose the privilege to participate in commencement. Cap, Gown & Tassel – are provided by OSH free of charge for seniors participating in the 2015 OSH commencement ceremony only and are theirs to keep. These items will be distributed to eligible seniors on their last day, Tuesday, June 2 from 1:05 to 2:00 PM in the Forum. Seniors must present their completed clearance form to receive their cap and gown package and are released at that point. NOTE - no caps/gowns will be disbursed prior to the scheduled distribution time of 1:05 PM, so please plan accordingly. Commencement Participation Fee – a fee of $20.00 (approved by the School Board) is collected from all seniors planning to participate in commencement. Seniors will pay this fee (CASH only) beginning on May 13 at the Greeter/Information Desk. Seniors must pay this fee before they will be cleared to pick up their cap and gown package on June 2. Detention/Fines and Fees – All detention must be served and all fines and fees must be paid before a student is cleared to pick up their cap and gown. Final Transcript Release Form (included with this document) – is required before OSH will sign off on the final clearance form. Information on how to request your final transcript is included in this information. The release form is also available in the Career Resource Center and on the OSH Website under Quick Links. Graduation Assembly/Clearance Forms – On May 28 and May 29, during Advisory, we will conduct MANDATORY SENIOR ASSEMBLIES in the auditorium. Seniors will be divided into two groups by Advisor and must attend on their assigned day as the required clearance forms will be handed out at the end of the assembly. PSEO students may choose which day they attend. The meeting will cover all details of the commencement ceremony. Also included in this flier is a copy of the Senior Information Sheet that will be distributed to seniors at the assembly. Please review this important information with your senior student. The Senior Clearance form (sample on page 6) will be handed out at the end of the assembly by Advisors – the form will have the students name preprinted when they are handed out. If a senior misses the assembly, they will not receive the required form until they makeup the assembly (as it is mandatory). Seniors are to fill out all information except the “teacher signature” and “comment” columns, then present it to each teacher for their signature. In addition, seniors should present the clearance form to the CRC (Career Resource Center), to verify that the senior has submitted their post high school plan and a final transcript release (two sheets on color paper included with this Flier). Senior locker cleanout is Monday, June 1 and students will have their Advisory teacher sign their clearance form to verify the locker is cleaned out. The student’s counselor will be the last person to sign off on the clearance form. Counselors will be available during all lunches on June 1. Seniors will present the completed clearance form during 6th period on June 2 to receive their cap and gown. After they have picked up their cap and gown they will either need to leave the building or go to a prearranged waiting area until 2:00 PM – they will not be permitted anywhere else in the building. PSEO students: You are also required to attend one of the senior assemblies on May 28 or 29. Part time PSEO with regular Advisory: Follow the general instructions above. In addition, you must either print off your grade report for your college course(s) with a staff member, or, if your grades are not yet available online from the college, you need the signature of each college instructor on the clearance form.
