On your wave start date, you can log on to the Lockheed Martin Employee Service Center (LMESC) website for more details on your specific plan options and costs. You can enroll online or by phone. Wave 1: Oct. 6 - 22 Active employees in all states except California, Georgia, New Jersey and Texas Wave 2: Oct. 13 - 29 Active employees in California, Georgia, New Jersey and Texas Wave 3: Oct. 27 - Nov. 12 Retirees, disability medical participants, and special populations Correction Period for All Employees: Nov. 25 - Dec. 4 You will receive information on your coverage options and costs in upcoming communications. Also, remember the elections you make during Annual Enrollment take effect Jan. 1, 2016 and you cannot change them during the calendar year unless you experience a qualified status change or have an IRS-defined qualifying change in status. If you have a qualified status change, you must report the event and any changes to your elections to the LMESC within 30 days following the event. Note: You can view your current 2015 benefits under the My Benefits section on the LMESC home page. Do not make 2016 enrollment decisions based solely on this information because plan offerings and costs may be different. Expatriates may enroll during any wave period. How to Enroll Online How to Enroll By Phone 1. During your enrollment wave, access LMESC Online from the (external) Internet, at https://lmpeople.lmco.com. From the Internet (external), access LM People at https://www.lmpeople.com or www.lockheedmartin. com>employees> and log in using your NT Username and password. Call the LMESC during your Annual Enrollment wave at one of these numbers: 2. In the Pay and Benefits section, select “LM Employee Service Center.” Select the prompt for Annual Enrollment. Representatives are available 8 a.m. to 8 p.m. Eastern time Monday through Friday (except on holidays). You will need your LM People ID and LMESC PIN. If you do not have this information, the LMESC will reset your PIN then transfer your call to the Lockheed Martin Service Desk for further assistance. 3. Once your wave begins, access your Annual Enrollment from the LMESC Online home page banner and follow the steps to make your elections. 4. Print a copy of your election summary for your records. 5. If you haven’t designated a life insurance beneficiary online, or you would like to review your current beneficiaries, simply select “View Beneficiary Information” under the “My Life” tab. The Lockheed Martin Employee Service Center Online features a special “Live Chat Portal” that allows you to chat online real time with an LMESC customer representative who can answer specific questions about your benefits. You can also ask your question via email by clicking on “Contact Us.” Your Benefits Through LM HealthWorks, medical coverage is integrated with health and wellness programs to help you feel the best you can – at home and on the job. We provide a comprehensive suite of resources and information to make it easier for you to make positive choices for your health, including: • Medical, dental and vision coverage •F ree health coaching and condition management • Incentives for healthy lifestyle choices •T eladoc 24/7 telemedicine •T obacco cessation support •2 4-hour Informed Health Line (Nurseline) •F ree flu shots at many locations •A physical activity program to help you meet your personal fitness goals •F ree, confidential biometric health screenings •N utritious food options in cafeterias •T rusted, online health information available 24/7 •O n-site Wellness Centers, walking paths, fitness centers, and weight management groups at many locations While specific benefits will depend on your work location, the following pages detail some of the benefits that may be offered. You can view your 2016 options by logging on to the LMESC Online (LM People > Pay and Benefits > LM Employee Service Center) when Annual Enrollment starts for you. Medical Coverage Medical coverage can offer you the care you need, help you remain healthy and provide a safety net that guards against the high cost of health care. The LM HealthWorks Plan covers preventive care at 100 percent, includes prescription drug benefits and has an out-of-pocket maximum to help protect you financially in the event of a serious injury or illness. And, you can earn credits for your HealthFund by completing Healthy Actions. Coverage Tiers for You and Your Family When you enroll in health coverage – medical, dental or vision – you choose between four categories of coverage based on the dependents you enroll, if any. The coverage levels, or tiers, are: • Employee Only • Employee + Spouse/Same-Sex Domestic Partner • Employee + Child(ren) • Employee + Family Please note that it may cost you more to use out-ofnetwork facilities. The amount the LM HealthWorks Plan pays is based on the reasonably accepted charges for a particular service in your specific geographic area. You can maximize your benefits by using in-network providers and facilities whenever possible. For a list of in-network facilities, log in to Aetna Navigator at www.aetna.com, and use the DocFind feature. You can also call Member Services at 877-458-4975. Express Scripts Prescription Benefit Program Express Scripts is the prescription drug administrator for the LM HealthWorks Plan. Take advantage of the mail-order service for additional savings. On the Express Scripts website you can also look up medications and compare costs, as well as confirm the coverage levels of your medications (www.express-scripts.com). Remember, covered medications change periodically, so it’s a good idea to check that your prescriptions are still covered. Life and Accident Lockheed Martin offers Employee Term Life, Group Universal Life, Special Accident and Dependent Optional Term Life insurance. Depending on the option you choose, benefit amounts may be a multiple of your annual base pay or a choice of coverage tiers. Special Accident insurance is additional coverage that pays a benefit if a covered individual dies or suffers certain losses in an accident. Group Universal Life insurance provides benefits above the Employee Term Life coverage plus the chance to save additional funds in a cash accumulation account. The cash accumulation account is a great way to build cash value as part of your life insurance plan. The cash accumulation account currently pays four percent interest and can be used in a variety of ways: • Use it to build cash value to help pay premiums at older ages. As you age, your premium level will increase. Help to keep premiums level by accumulating cash value in the account and then using it to offset premium increases in later years. (continued) PAGE 5 Life and Accident (continued) • Use cash value as a way to save for future needs such as college tuition, weddings or home improvements. You can borrow or withdraw funds from the account without paying income tax, up to certain limits. • Use it to increase the amount that you leave to your beneficiaries. Since Group Universal Life is a life insurance product, any cash in the fund will pass to your beneficiaries as life insurance proceeds -- that means they will not need to pay income tax on those proceeds. Dental Coverage A wide range of medical problems including diabetes, heart disease, bronchitis and headaches can be traced to teeth and gum disease. Prevention is powerful, so see your dentist and keep your mouth healthy. Vision Coverage Vision coverage is available to help with the cost of eye exams, prescription lenses, frames and contact lenses for you and your covered dependents. Flexible Spending Accounts Did You Know? If your spouse/same-sex domestic partner is also a Lockheed Martin employee, you cannot be covered as an employee AND as a dependent for any life plans made available through the company. You may only cover your spouse/same-sex domestic partner as a dependent if he or she is not enrolled in any other company life insurance plans. In addition, dependent children can only be covered by one employee. Disability Disability insurance replaces a portion of your income while you are unable to work. Lockheed Martin provides short-term disability coverage to you automatically at no cost, subject to eligibility requirements. You may also elect long-term disability insurance during Annual Enrollment, subject to eligibility requirements. Business Travel Accident Insurance This insurance pays a benefit if you die or are seriously injured as a result of an accident while traveling on company business. Lockheed Martin automatically provides Business Travel Accident Insurance to you at no cost, subject to eligibility requirements. Is Your Beneficiary Information Up-to-Date? Whether you need to assign a new beneficiary or update an existing one, now is the perfect time to make sure this critical information is accurate. It’s easy to do and can be completed through the LMESC Online > My Life > View Beneficiary Information. Employee Assistance Program LifeMatters, the company’s Employee Assistance Program, can help you manage many aspects of your life. LifeMatters offers confidential consultations for stress, relationship issues, financial concerns and more. Visit LifeMatters at www.mylifematters.com (password: LMC1). PAGE 6 Flexible Spending Accounts (FSAs) are a valuable consumer tool that help you save money on taxes. There are two types of accounts: • Health Care Spending Account • Dependent Care Spending Account Important Reminder: You must make an active enrollment election each year if you wish to contribute money to the flexible spending accounts. The default contribution amount is zero if you do not make an election. Savings Plan Most employees are eligible for a 401(k) Savings Plan. Consider contributing as much as you can to your future. Visit the Lockheed Martin Savings Plan Web Tool at LM People > Pay and Benefits > Savings Plan. Employee Discounts Program Lockheed Martin employees can receive discounts on products and services through offers from companies participating in our Employee Discount Program. Take advantage of discounts on computers, flowers, furniture, vacation packages, and much more. Visit LM People > Pay and Benefits > Employee Discounts Program. Commuter Assistance Program If you are commuting to work using mass transit, this program enables you to use a pre-tax payroll deduction to pay for eligible transit, vanpool and parking expenses or transportation passes. To enroll, visit LM People > Pay and Benefits > LM Employee Service Center > My Benefits > Commuter Assistance Program. Or, you may call the LMESC at 866-562-2363. (continued) Resources at Your Fingertips For Your Benefit Many of our benefit providers offer tools that give you convenient and secure access to your information, as well as resources for your care: www.MyLMBenefits.com Aetna/LM HealthWorks Convenience Care: LM HealthWorks Plan members have access to the care they need, when and where they need it, by using one of the available Convenience Care options. Rather than visit a costly emergency room, members can go to an urgent care center, call the free 24-hour Nurseline, consult with Teladoc (www.teladoc.com/aetna), go to a retail clinic or reference iTriage online at www.itriagehealth.com to search for doctors and facilities, and get advice on symptoms. iTriage and Teladoc also offer mobile “apps” available for download. DocFind: Use this online provider directory to find in-network providers who have agreed to charge special preferred pricing rates for LM HealthWorks Plan members. To find in-network providers in your area visit http://www.aetna.com/ docfind/custom/lmhealthworks/. Cost of Care Calculator: As a member of the plan, you can use the Aetna Cost of Care tools to get estimated average costs for medical treatments and procedures, tests and other services in your area. To access, log in to www.aetna.com > Care & Treatment > Estimate Cost of Care. Express Scripts Prescription Drug Program Refill any time, from anywhere! Search for “Express Scripts” in the app store or visit www.express-scripts.com. Did You Know? LMESC Online gives you direct access to many of our vendor partner websites right from the homepage “Quick Links” section. No extra password or User ID is needed. Employees and their family members now have access to tools and resources to support their use of company-provided products and services ranging from benefits to discounts to health and wellness support. In addition, some of the same information that is available to employees is now available to family members on their own secure website. It features articles, videos, tools and resources to help you and your family: • Choose the benefits that are right for your personal situation. • Use your benefits in ways that save you money, support your health and leverage wealth resources. • Improve your health and wealth with Lockheed Martin’s Total Rewards. Spouses/Same-Sex Domestic Partners are invited to register for an account by accessing www.MyLMBenefits.com and entering the code LMFYB. They will also be prompted to enter other data to validate their association with a Lockheed Martin employee. Healthy Actions Lockheed Martin is committed to employee wellness and offers incentives for taking steps to better your health. The LM HealthWorks Plan offers a HealthFund that employees and spouses/same-sex domestic partners can build by completing certain healthy actions. If you are an LM HealthWorks Plan member, don’t forget to take advantage of building your 2016 HealthFund before it’s too late. Tracking Your HealthFund Balance Keep track of your Healthy Actions and HealthFund balance by visiting your Aetna Navigator® home page at www.aetna.com, then click on “View Incentives” on the blue “I want to...” menu. PAGE 7 Healthy Actions The Healthy Actions incentive program isn’t changing for 2016. Take a step towards living a healthier lifestyle and employees enrolled in LM HealthWorks can earn up to $1,150; enrolled spouses/same-sex domestic partners can earn up to $750. Non-members can earn up to $600 into your paycheck, subject to federal and state income tax. Beginning on Jan. 1, 2016, the Well-Being Assessment (WBA) will have a new look and feel, including fewer questions and an easier-to-read report. It will also be easier to complete your WBA because the new format is smartphone-friendly and can be accessed on your iPhone, iPad, or Android device. LM HealthWorks members and their spouses/same-sex domestic partners can each earn $100 in HealthFund credits simply by logging on to www.aetna.com and clicking “Take the WBA” on the blue “I want to….” menu on the left-hand side of the screen. Remember, you have until 11:59 PM on Nov. 30 to take your 2015 WBA. You can continue to earn HealthFund credits for the other Healthy Actions through midnight on Dec. 31. PAGE 8 Healthy Actions Wellness Centers We offer a number of services to all employees – regardless of health plan – to help you stay well throughout the year. Lockheed Martin’s Wellness Centers play an integral role in managing employee health. There are 27 Wellness Centers that ensure access to health care support is convenient, confidential and free, regardless of your health plan. Wellness Center services include: Get Active For a Good Cause • Acute care (colds, cough, sore throats) There are many opportunities to commit to participating in an event in your community that will get you out and moving for a good cause, from the American Heart Association’s Heart Walk to the American Cancer Society’s Relay for Life. Watch for communications at your local site to learn more and to find opportunities in your area. • Blood pressure checks Virgin Pulse Physical Activity Program • Chronic health condition counseling Looking for ways to be more active? Get moving with Virgin Pulse — an interactive, web-based physical activity program that can help you increase your daily activity. Visit www.virginhealthmiles.com to track your activity or use the “Virgin Pulse” app which can be found in the app store to easily track and monitor your progress and manage all of your fitness information in one place. The program is free for employees and spouses/same-sex domestic partners, regardless of whether you are an LM HealthWorks Plan member, and eligible participants can earn up to $600 in rewards for being physically active! • Cholesterol screenings • Healthy eating education • Weight management support • Tobacco cessation education Flu Shots Every fall, more than 100 Lockheed Martin sites host events in which employees may receive a free seasonal flu shot to stay well throughout flu season. Employees without access to on-site flu shots may apply for a flu shot care card online, visit their doctor or make an appointment at their nearest on-site Wellness Center. Sign up today at LM People > Health and Wellness. Did You Know? Spouses/Same-Sex Domestic Partners (SSDP) who are enrolled in a company health plan are automatically eligible for the Virgin Pulse Physical Activity Program. If your spouse/spouse/same-sex domestic partner is not enrolled in your health plan, you can confirm eligibility by accessing the LMESC Online, clicking on My Life > Life Changes & Events > Physical Activity Spouse Elig and following the instructions. Sixty percent of our employees live within five miles of a Lockheed Martin Wellness Center. Quit For Life® Program If you or your eligible dependent is a tobacco user, you and/or your eligible dependents can enroll in our free Quit for Life® tobacco cessation program. You will get your own Quit Coach, a personalized Quit plan, medication or tobacco cessation aids, and access to online resources and a social support community. PAGE 9 Your Information Availability of Summary Health Information The Privacy of Your Health Information Choosing health coverage is an important decision. To help you make an informed choice, your plan makes available a Summary of Benefits and Coverage (SBC), which summarizes important information about health coverage in a standard format. Lockheed Martin is firmly committed to protecting the confidentiality of your personal health information and strongly supports the protection provided by the Health Insurance Portability and Accountability Act of 1996 (HIPAA). HIPAA requires privacy rights for personal health information, as well as protection from improper use and disclosure. It affects how your health information can be used by group health plans sponsored by employers including Lockheed Martin. The health benefits available to you represent a significant component of your Total Rewards package. They also provide important protection for you and your family. Please review the HIPAA Privacy Notice, which is available online in the LMESC Online > Library > Helpful Resources. SBCs will be posted in the Annual Enrollment section of the LMESC Online website. A paper copy is also available, free of charge, by calling 866-562-2363, 800-833-8334 (hearing impaired) or 201-242-4397 (international callers). PAGE 10 Health Care Reform The United States’ Patient Protection and Affordable Care Act (ACA) – commonly known as health care reform – was signed into law March 23, 2010. According to the U.S. Department of Health and Human Services, the ACA is working to make health care more affordable, accessible and of a higher quality for families, seniors, businesses and taxpayers alike. Effective Jan. 1, 2015, employers with 50 or more full-time employees – including Lockheed Martin – are subject to the ACA’s employer shared responsibility provisions. These provisions require Lockheed Martin to offer affordable health coverage that provides a minimum level of coverage or be subject to a penalty. Lockheed Martin plans are currently compliant with health care reform laws and exceed the required minimum level of coverage. In line with these provisions, Lockheed Martin must also adhere to information reporting responsibilities, which require employers to send health insurance information returns to employees and to the Internal Revenue Service (IRS). The IRS has created new forms for this purpose, which employees will need when completing their 2015 federal income tax returns. Most employees will be able to elect to receive the new form electronically through https://www. lmpeople.com. Those who do not elect to receive their forms electronically will receive a printed copy in the mail in early 2016. Please note that individuals, including those with access to affordable employer-provided health care plans like Lockheed Martin’s, can choose to purchase insurance through Health Insurance Marketplaces available in their area. However, if you choose to purchase health insurance other than from Lockheed Martin, Lockheed Martin will not make any contribution to your premium or out-of-pocket costs. All health care costs will be your responsibility and you should consider all information available before making any decisions on coverage.
