It aims to provide options for participation as well as support for seniors if required. The directory provides a summary of what is available in Mandurah and surrounds and does not aim to list every group or service. The City of Mandurah also has an online directory that lists local community services, facilities, events, clubs, groups and organisations. Home Care Package Provides a coordinated package of services tailored to meet specific care needs when people are no longer able to manage at home with basic support. Home Care Packages use a Consumer Directed Care approach (CDC) giving people the opportunity to decide what services they get and how they are provided with the allocated funds. An Aged Care Assessment Team (ACAT) will assess the level of care needed. Talk to your doctor or local hospital for a referral to an ACAT. Respite Commonwealth Respite and Carelink Centre 1800 052 222 Respite funding is designed specifically to give the carer a break from the caring role, for carers who have been caring for someone for 6 months or more. • In home Respite ‐‐ A support person will come into the home and spend time with your loved one whilst giving the carer a break to attend to tasks outside the home. • Centre Based Day Respite ‐‐ Your loved one attends a day centre. • Respite Facility ‐‐ A stay in a residential aged care facility (see page 43) or respite cottage for up to a few weeks ‐‐ Speak to your doctor about getting an Aged Care Assessment Team (ACAT) assessment ‐‐ Book in advance where possible. Residents requiring extra support will need to arrange independently. Lifestyle villages offer communal facilities and activities for residents. Contracts generally provide for the purchase of a transportable home and the lease of the land on which it stands. 44 Accommodation & Housing Retirement Villages A retirement village is a community of homes for residents 55 years or older. Most offer independent living units or serviced apartments for residents who require some assistance. Retirement villages offer several different ‘ownership’ options such as: • Leasehold • Loan and licence agreement • Strata title • Company title • Unit trust • Rental Residential Aged Care Facilities (Nursing Homes) There are two levels of care provided in aged care homes. Low level care for people who are semi-dependent and high level care for people who need greater assistance and daily nursing care. They provide accommodation, meals, cleaning and laundry, assistance with showering and dressing, medication management and social activities.
