Complete the application form
Provide confirmation of your regular income
The application form needs to be completed in full and
signed by all applicants. It is essential you fill in all your
details carefully and correctly, so please pay particular
attention when completing the following sections:
• Section 2 ‘Details of the Accounts you would like to open’
• Section 3 ‘Financial Background’
• Section 6 ‘Offshore PhoneBank’
Finally, sections 5 and 8 must be signed by all applicants.
We will need one of the following original or certified copies:
STEP 2
Provide confirmation of your identity
We need you to send us a copy of your current passport:
• It should be a clear certified black and white photocopy.
• Remember to also enclose copies for any joint applicants.
• Your passport(s) must be valid (not out of date) and
show your date of birth, place of birth and signature.
Your signature should be the same as that shown on
your application form.
Please read the leaflet ‘Certifying your passport copy and
other documentation’ very carefully to ensure certification is
done correctly.
STEP 3
Provide confirmation of your address
We need you to send us a recent copy of one of
the following:
• A gas or electricity bill.
• A telephone bill (mobile bills will not be accepted).
• A local authority rates bill.
The bill should clearly state your name and current
residential address.
For joint accounts:
• We need to see an original utility bill addressed to each
party, either jointly or individually.
• Or an original mortgage statement, bank or credit card
statement from a recognised bank addressed to each
party, either jointly or individually.
All documents must be less than 3 months old.
• Recent six months’ consecutive active bank statements.
• An original letter addressed to Lloyds TSB from your
employer on formal company headed paper confirming
your earnings.
• Latest three months’ formal payslips.
Or, if you are self-employed, one of the following:
• Recent six months’ consecutive active personal bank
statements - original or certified copies.
• An original letter addressed to Lloyds TSB from your
accountant on formal company headed paper stating
your present earnings.
Please read the leaflet ‘Certifying your passport copy and
other documentation’ very carefully to ensure certification is
done correctly.
STEP 5
Enclose your initial opening deposit
Please enclose an opening cheque or banker’s draft for
a minimum of £100, $100 or €100. If you open more
than one account, only one initial deposit is required.
Please make your cheque payable to your own name or
to Lloyds TSB. All cheques and drafts should be endorsed
(signed by you) on the back. For existing customers, you
may transfer funds from your existing Lloyds TSB account
by completing Section 7 of the application form.
STEP 6
Go through the checklist, and send your completed
application form and all the RELEVANT associated
documents to:

Lloyds TSB Private Banking Limited, which
