PHOTO BY MICHELLE LE ANTI ANTIBIOTIC Hospital construction on track, below budget ‘ANTIBACTERIAL’ GELS AND SOAPS ARE OVERKILL AT BEST, DOCTORS SAY, AND A PUBLIC HEALTH HAZARD AT WORST By Kathy Schrenk C old and flu season is in full swing, which means more people are pulling antibiotic gels from their pockets or purses to wipe off germs, or buying antibacterial soaps for home use. And if you’re a parent of a little germ-monger, you may be finding yourself in the pediatrician’s office more often than you’d like, and possibly getting a prescription or two for an antibiotic. Unfortunately, you may be doing more harm than good to your family, the population at large, and the environment. Antibiotic gels and soaps seem to be ubiquitous these days, but outside of hospitals, they’re overkill, experts say. Dr. Daniel Shin, infectious disease specialist at Palo Alto Medical Foundation’s Mountain View Center, and assistant professor of medicine at UCSF, uses antibacterial hand gel when he’s working at the hospital, as do most nurses and doctors, but at home he uses regular soap and water. He gets lots of questions from his patients about the gels and hand wash-ing. “Everyone is worried about germs,” he said, but it’s best to avoid over-thecounter antibiotic products and just use regular soap and water. That’s because antibiotic gels and soaps contain a substance called triclosan, which has been shown n to boost bacteria’s abilility to resist treatment. Antibacterial soaps and gels can be harmful to o the E l Camino Hospital administrators say the new facility is on track and below budget, and should be open for patients in fall 2009. Construction began on the $452 million building in 2006 to meet new state seismic regulations. The new hospital, spread out over about 460,000 square feet on five floors, will have private rooms and baths equipped with wireless technology for each of its approximately 240 patients, a larger emergency room and areas for families to stay on each floor. Ken King, vice president of facility services, said construction will finish in July 2009 as planned. The new hospital should open to patients in October 2009 after several months of preparations, including training staff and moving equipment. But a final opening date has not yet been set, King said. “After we complete construction, we have a whole series of things to do,” he said. With this increased awareness, however, we now have a new problem: information overload. Many patients I meet have read or heard so many different warnings about environmental threats within their homes that they simply don’t know where to begin to fix these potential problems. I counsel patients to not get frustrated and give up. Do what you can do. Bit by bit, each step of the way, you will create a more healthful home for your family. Q: A: How can I make the home environment safer for my family? We all want a healthful home that is clean and safe, especially during the holidays when friends and family come to visit. The qualities of a healthful home are: dry, clean, well ventilated, pest free, non-toxic, calm and comfortable. To achieve this, start with these simple tips: ■ Consult with a home professional if you notice roof or pipe leaks, a musty smell or an area of the home that always seems damp. A dry home will have cleaner air that is free of spores from molds that thrive in damp areas. It also will be less attractive to household pests. ■ Clean regularly to decrease dust that can trigger allergies and asthma attacks. ■ Open up windows to air out rooms. Without good ventilation, harmful particles can quickly build up in the air. ■ Ask smokers to smoke outside only. ■ Use non-toxic pest-control products to keep out pests such as dust mites, rodents and fleas that can spread of germs and allergens within the home. ■ Substitute cleaning products that contain toxic ingredients that may irritate the lungs and skin with home-made cleaning remedies made with baking soda, white vinegar, lemon juice and water. The Consumer Reports Web site at www. consumerreports.org promotes homemade cleaners and provides recipes, as do many other Web sites and books. Q: Drinking bottled water used to make me feel healthier, but is tap water actually better? A: It may be. New research has found that some types of plastic containers, including bottles and water cooler reservoirs, may leech chemicals into water. To be on the safe side, choose filtered tap water. Installed water filters, especially reverse osmosis units, may provide the purest water, but plain tap water is also fine. If you have old pipes, run cold water for about a minute before filling a drinking glass to remove metals that may be in the water. Q: A: If you could only change one thing about your home, where would you start? One of the easiest places to start to reduce home toxins is in the kitchen. To avoid pesticides on food, wash fruits and vegetables, peel non-organic produce, and buy organic when possible. Farmer’s markets are a good source of less expensive local, organic food. In addition, avoid using pesticides on fruits or vegetables you grow in your own garden. Q: A: I love a home that smells of pine or spices during the holidays. Are seasonal air fresheners safe? The best way to tell is to read the label on the bottle. Many air fresheners contain butane (lighter fluid) and acetone (nail polish remover), and some are even labeled “fatal if inhaled.” Experts also are concerned that synthetic fragrances that mimic the hormone estrogen could disrupt the delicate hormonal balance that controls our bodies’ growth, development and reproductive functions. A safe alternative is using natural citrus oil spray or baking soda to freshen the air. Q: A: I’ve heard that “offgassing” is a home danger. What does it mean? “Off-gassing” is the release of chemicals into the air through evaporation. If you have ever noticed how the smell of fresh paint or new carpet lingers in a home, you’ve experienced off-gassing. Remodeling your home can produce a lot of harmful chemical odors from plywood, chipboard, synthetic carpeting and furniture floor glues. Even new shoes or dry cleaning can release toxic chemicals in the air. Triclosan eventually gets into the water supply, and though it’s biodegradable, it produces potent carcinogens called dioxins when it breaks down. Besides, “You’re not going to catch something lethal from a grocery cart,” he said. When someone does catch a cold or other virus, that’s when it becomes really crucial to be careful of what medicines are getting prescribed, said Penny Takizawa, head of infection control at El Camino Hospital. Experts agree that antibiotics are used more often than necessary, Takizawa said. They only work when a bacterial infection, like most forms of bronchitis, is attacking the body. But they’re ineffective against the common cold or the flu. When they are overused, strains of antibiotic-resistant diseases, like diarrhea-causing MRSA or so-called “C-diff,” can develop, she said. C-diff is very difficult to kill outside of an autoclave, which is used to sterilize medical equipment. C-diff poses little risk to healthy adults unless they pick up the spores and then go on antibiotics, Takizawa said. Then, the medicine will kill the good bugs that usually live in the gut, and the C-diff will thrive, causing a new illness. Ear infections, for instance, don’t always require antibiotics, she said. Most, in fact, are caused by viruses, and can’t be cured with antibiotics. Pediatricians are now being advised not to treat ear infections with antibiotics, Dr. Shin noted. ing. But plain old-fashioned washing with soap and warm water is best. So while limited use of the gels when you’re out is OK, avoid them if you can — and don’t use antibacterial soaps at home at all. As for prescription antibiotics, have a discussion with your doctor if he or she wants to prescribe you antibiotics.
