PRESIDENT’S MESSAGE
Standing on the shoulders of giants
As you may remember, in last year’s Annual Report, I wrote about that old saying,
“Success has many parents,” and its application to orthopaedics. This year, I’d like
to share an equally appropriate thought. All of us who are privileged to call ourselves
orthopaedic surgeons are standing on the shoulders of those who came before us and
paved the way for our opportunities and successes.
No doubt, we’ve all worked hard to achieve our professional success. But we didn’t
do it alone, and today’s new investigators, whether young in age or new in experience,
can’t do it alone either. They are also standing on the shoulders of giants, namely the
thousands of generous donors who have made possible the work OREF does to advance
orthopaedics, one grant at a time. Our donors allow us to support the investigators
who are committed to answering the most urgent questions in orthopaedics today.
In this year’s report, as we look back on 2013, it is my honor and privilege to
recognize you! On behalf of the OREF Board of Trustees, CEO Sharon Mellor,
committee members, volunteers, staff and, of course, our grateful grant recipients,
I extend sincere thanks for your generosity and commitment to advancing the field
of orthopaedics. We recognize you as the giants upon which the future of
orthopaedics stands.
On a personal note, as my term as president ends, and OREF approaches its 60th
anniversary, I am more encouraged than ever that OREF, under the leadership of
its new president, John J. Callaghan, MD, and through the unwavering support of its
donors, will be providing critical early funding to new investigators far into the future.
I hope that you enjoy reading this year’s report and, as always, I invite your feedback

2013 I FINANCIALS
MANAGEMENT COMMENTARY | YEAR END DECEMBER 31, 2013
OREF financial statements are presented on an accrual accounting
basis with 2013 grants expense being funded by 2012 contribution
revenues. Donations received in 2013 will be used for 2014 grants.
OREF received 2013 funding from more than 1,992 individuals
and 35 corporate partners. OREF’s enhanced focus on research
resulted in more than $1.4 million in gifts directed to the new
research fund. Annual giving also increased 48% to $1.8 million. OREF secured support from 10 new, increased or re-engaged
corporate supporters, thereby increasing corporate support by
28% more than 2012.
Fundraising and general administrative expenses were lower
than prior years as a result of focused cost-controls and
efficiencies realized as OREF transitions out of programs not
aligned with its mission.
Investment earnings were 9.43%, a strong showing in light
of general investment markets.
OREF has a strong balance sheet reflecting increased
financial health.

On how the National Institute of Arthritis and
Musculoskeletal and Skin Diseases (NIAMS) supports
orthopaedic research and education: Part of the National
Institutes of Health (NIH), the NIAMS supports research into the
causes, treatment, and prevention of arthritis and musculoskeletal
and skin diseases; the training of basic and clinical scientists to
carry out this research; and the dissemination of information on
research progress in these diseases. Roughly 84% of our budget
goes to extramural research, mostly in the form of investigatorinitiated or training grants. We are committed to funding the best
scientific projects as determined by a rigorous, two-stage peer
review process (see http://csr.nih.gov/).
On how OREF grants can help clinician scientists
obtain NIH funding: As NIH funding is highly competitive, it
is important for researchers to have access to multiple sources of
support. Even a small grant can facilitate collection of preliminary
data, which may help a good application become a great
application, and thereby get funded by the NIH. For those who
already have NIH funds, programs such as the OREF Mentored
Clinician Scientist Grant (for NIH career development awardees),
provide supplemental support that better enables the grantee for
the next level of NIH funding.
On how, in addition to providing training and career
development opportunities, OREF can help orthopaedic
surgeons become more competitive for NIH funding:
An important NIAMS goal is to identify and fund trials that
are as timely and informative as possible, and that will lead
to improvements in clinical practices for disease prevention,
diagnosis, and treatment. Professional foundations such as OREF
can play an essential role by helping researchers form teams
that can identify the key research questions relevant to clinical
practice, and develop the highly meritorious investigator-initiated
proposals to answer them. More information about the grants

On what drew him to orthopaedics: I became interested in
orthopaedics because of my own high school injuries. I tore my
meniscus and had a wonderful orthopaedic surgeon who was a
clinician scientist and very inspirational.
On how he first learned about OREF: When I was a
resident, I was fortunate to work with clinical researchers who
were very involved with OREF. They suggested I apply for
the OREF Health Services Research Fellowship, which I did
successfully. That support allowed me to go to the Harvard School
of Public Health, earn a master’s degree in epidemiology, and it
stimulated an interest in clinical trials, outcome instruments, and
cost effectiveness decision analysis, which has really has set the
tone for my career.
On the importance of orthopaedic research: I trained at
multiple hospitals so I learned different techniques for the same
surgery—for example, several different ways to do a knee
or hip replacement. What was frustrating was that there weren’t
good data to support one technique over another. Finding those
answers is important and it’s what interested me
in clinical research.
On being a peer review committee member: My goal
as a peer reviewer was to identify promising investigators who
could benefit from research funding and career mentorship in
order to become established orthopaedic researchers. I think
that many promising orthopaedic researchers or clinicianscientists get off track early in their careers because of a lack
of funding, support, and mentorship. This is a big loss for our
field. Programs such as the OREF Career Development Award
support these investigators and can make a tremendous impact
on establishing their careers.

On what drew him to orthopaedics: I was drawn to the
personalities of residents and attending staff (Drs. J. Paul Harvey
and Mike Patzakis) at LA County USC Hospital, and to the range
of clinical problems addressed within the specialty—trauma,
infection, tumor, hand, etc.
On how he first learned about OREF: During residency at the
University of Washington, Dr. Dan Spengler, our spine attending,
pointed out to me OREF’s resident research grant program.
On why he supports OREF: I support OREF because it has
supported my career development, because it is our collective
responsibility to support our own research to advance the care
of our patients, and because OREF is critical for our specialty to
compete for federally sponsored research support.
On the importance of orthopaedic research: Without
high-quality research we cannot improve the care of patients
with musculoskeletal problems.
On becoming a member of OREF’s Visionary Research
Society: I became a member of the Visionary Research
Society because I want to foster the notion that, in terms of us
behaving in a professionally responsible manner, it is important
for the specialty of orthopaedics to be the number-one specialty
supported by research and education.

What drew him to orthopedics? Orthopaedics gives me the
ability to use my mechanical and surgical skills to help people by
easing their pain and restoring their function.
On how he first learned about OREF: I first heard of OREF
during residency but did not have any meaningful sense of OREF
until I met Dr. Ramon Jimenez. He enlightened me as to the value
of basic science research in furthering our ability to achieve our
professional and humanitarian goals.
On why he supports OREF: Unfortunately, basic science
research is not often glamorous. However, this does not diminish
its importance. Basic science research is the foundation upon
which we build our clinical research and, subsequently, our
treatment of patients.

On becoming a member of OREF’s Visionary Research
Society: When I spoke with Dr. Jimenez, he mentioned that if
not for basic science research, orthopaedics would be limited to
skeletal traction and debridement of osteomyelitis. I realized the
importance of basic science research and wanted to do my part to
support it.
On how he encourages colleagues to support research:
Isaac Newton once said, “If I have seen so far, it is because I
have stood upon the shoulders of giants.” Orthopaedics stands
out among other specialties for its giants and for how far they
have seen. We have seen much and yet have miles to go.
Without support for OREF, continued progress in our ability to
treat patients, ease suffering, restore function, and uphold our
Hippocratic oath is greatly diminished.

On why he became a clinician scientist: One role of the
surgeon-scientist is to find ways to integrate new basic science

On the importance of orthopaedic research: We need
to look to the future and ask where the greatest challenges are.
We’re constantly going to be faced with providing better care for
our patients and to have better outcomes. One way to achieve
this will be through innovation and translational research. It’s
absolutely critical that, as a profession, a commitment is made
to recognize and financially support these research efforts.
On the importance of OREF funding: I knew receiving
an OREF grant would be critical to the success of the research
project, but what I did not anticipate is how much of a profound
influence it would have on my career selection. I think there
is a small, critical window in a career where a surgeon can be
influenced to pursue the surgeon scientist track. By supporting
surgeon scientist research projects, OREF gives us a chance to
investigate scientific questions that can potentially significantly
change our surgical practice for the better. In the long term,
I hope this translates to discoveries that help our patients live
longer and better lives.

On what drew him to orthopaedics: Youth-related injuries

On the importance of orthopaedic research: Research
is important to seek the best solutions to orthopaedic problems.
There needs to be recognition of a problem and questions asked
to gain an understanding of the pathology or injury and to find
pathways that will improve our techniques to manage the problem.
On the importance of OREF funding: To me, OREF funding
is the fuel that supports the engine of change and improvement of
our delivery of health care. We boast that there is new information
that improves our specialty as we provide surgical education to
our peers and co professionals. We need to support this growth
and anticipate the needs of a profession that is dynamic and will
continue to look for the best solutions.
On being co-chair of OREF’s mentor campaign: The
co-chairs of the mentor campaign aimed to create an opportunity
to recognize a mentor and significant leader who has “moved the
needle” and made a significant difference in how we practice. My
role was to communicate with individuals with similar training and
share my belief that by contributing to OREF we recognize the
importance of people who have devoted their lives to making this
a more noble profession and helping the science of orthopaedics
at the same time.

On what drew him to orthopaedics: Sports and working
with Athletic Trainers drew me to orthopaedics. I am an honorary
member of the National Athletic Trainers’ Association.
On the importance of orthopaedic research: Our country is
facing a health care crisis in the sheer number of people impacted
by osteoarthritis and the costs of orthopaedic care. We need
continued research for finding economical and clinically effective
alternatives for this tidal wave of patients facing inactivity because
of pain and lack of function.
On why DJO Global supports OREF: OREF’s mission of
supporting orthopaedic research to improve function, eliminate
pain and restore mobility matches very closely with DJO Global

from OREF was critical. My research on brachial plexus birth palsy
is a new collaboration between our group, which has expertise
in neuromuscular biomechanics and computational modeling
of neuromuscular skeletal systems, and our collaborator in
orthopaedics who is not only a clinical expert in the treatment of
brachial plexus birth palsy, but also brings experience with the
animal model we’re using for the research. By working together,
we can tackle this in a more comprehensive way than we would
be able to working independently. We would not have been able
to get all of this off the ground without the Pediatric Orthopaedic
Society of North America/OREF Research Grant.

On what drew her to orthopaedics: My work in
graduate school was with a lab that was broadly interested
in musculoskeletal modeling. Most of the applications were
orthopaedic in nature. We were all using mechanical engineering
strategies to understand abnormal movement and disorders of the
musculoskeletal system.
On how she first learned about OREF: A number of sources
at my institution suggested OREF to me. Thomas L. Smith, PhD,
one of my collaborators from the Department of Orthopaedic
Surgery & Rehabilitation at Wake Forest School of Medicine, had
worked closely with OREF in the past and he’d had really good
experiences with OREF support.
On the value of applying for an OREF grant: The grant
funding we received was for a resubmission proposal. When you
submit a grant to OREF, you get thorough reviews that give you
very good, specific feedback. It’s worth your time to submit a grant
to OREF because the feedback you get can improve the science.
I’m very appreciative of OREF’s process and the time that the
reviewers spend because their input is really valuable.

CELEBRATING GRANT RECIPIENTS
In 2013, OREF provided $1.6 million in funding. In total, OREF has provided $140 million for more than 4,660 grants and awards since its founding in 1955.
Below is just a sampling of the research made possible by your gift to OREF.
Benjamin C. Bengs, MD, studied the role implant
design plays in achieving fixation for total hip
arthroplasty patients who have sustained proximal
bone loss.
Christopher K. Kepler, MD, MBA, researched the
molecular mechanisms associated with intervertebral
disk degeneration and diskogenic pain in order to develop
noninvasive treatments that can be targeted to individual disks.
Reuben Gobezie, MD, determined the pathways
involved in the development of osteoarthritis
and rheumatoid arthritis, which will help identify
pharmacologic interventions that will allow patients
to enjoy a more active life.
James P. Lawrence, MD, studied whether nimodipine can

to develop alternatives to revision surgeries associated with bone

David W. Shearer, MD, MPH, compared re-operation
rates for femoral fracture patients in Dar es Salaam,
Tanzania who are treated with intramedullary nailing to those
who are treated with open reduction and internal fixation with
a dynamic compression plate. Results will help determine
the most effective, locally appropriate treatment for femoral
shaft fractures.
Sevan Hopyan, MD, PhD, identified human
mutations and mechanisms that underlie congenital
femoral and tibial deficiency in order to develop
genetic screens for limb deficiency and future
regenerative strategies.
Austin V. Stone, MD, PhD, explored the molecular

On what drew him to orthopaedics: As a physics major in
college, I was interested in mechanical relationships. In addition,
while an undergraduate at Yale, I had contact with Dr. Wayne
Southwick, and he was largely responsible for my decision to go
into medicine. As a medical student, I was further influenced
by Dr. Harold Snedden, father of one of my best friends and an
orthopaedic surgeon. He asked me as a student to go with him on
several emergency cases.
On how he first learned about OREF: As chair of the Tripler
Army Medical Center orthopaedic residency program, I recognized
that someone in the program needed to have expertise in basic
sciences. To increase my own expertise, I began attending the
annual meetings of the Orthopaedic Research Society (ORS).
There, I saw firsthand the results of OREF’s work. I have continued
my association with the ORS and OREF ever since.
On why he has supported OREF for more than 20
years: I recognize the value of OREF to our practices and to our
patients. Without OREF, the massive expansion of orthopaedics
as a profession would not have occurred. Money given to OREF,
in my view, is the best leveraged money you can give, in terms of
contributing to happiness for the largest number of people.
On why orthopaedic research is important: When I
became an orthopaedic surgeon, total joint replacements were in
their infancy; the MRI, CAT scan, and image intensifier had not
been invented; the arthroscope was brand new; fracture fixation
was archaic; and spinal fixation was limited to Harrington rods.
Without research, 90% of what we do for our patients today
would not be possible. OREF contributions are essential for the
growth of our profession because orthopaedic research does not
compete well against research into cancer, cardiovascular disease,
and neurological disease. OREF provides a vital jump-start to
orthopaedic research and education. By doing so, it benefits every
orthopaedic surgeon and every patient who seeks orthopaedic
care—about 100% of people, I would guess.
