An FSAP Report should be submitted when an actual or potential event arises from an FAR deviation, SOP violation, failure to follow ATC instructions, or when a pilot wishes to report a safety of flight event confidentially. Each pilot must file their own independent report to be included in the program. If the electronic method of filing an FSAP is un available, notification of intent to file an FSAP may be made using the telephone hotline: 844-280-2727. An electronic report is required within 24 hours of the end of the duty period of the event for a domestic trip or 48 hours for an international trip. If this is not feasible, the report deadline can be extended if the incident is reported to the FSAP hotline within 24 hours of the end of the duty period that it occurred. For complete information regarding the Flight Safety Action Program (FSAP), refer to the FOM. Step Action 1 Begin by navigating to CCS. 2 Hover your cursor over the Reports Tab and select FSAP. 3 A new browser window will open and redirect you to the FSAP application. 12 Crew Communication System Pilot’s User Guide: Reports 4 Complete the information in the fields. Information, including your name, address, recent flights, and crew member names will automatically populate based on your profile. Follow the directions on the screen and complete as much information as possible. The Report has a 30 minute limit. Click Save and Continue often. 5 When the report is complete, select Submit. 6 You have successfully completed an FSAP Report. End of procedure. 13 Crew Communication System Pilot’s User Guide: Reports IOR IOR – Irregular Operations Report The IOR provides a standardized means of reporting incidents requiring a statement from the pilots. Only Captains are required to file IORs, but all pilots are encouraged to file a report regarding issues they want to bring to the attention of Flight Operations management or the Safety Department. See the FOM for a complete list of the Mandatory Report Submission Requirements. If an IOR is mandatory, this indicates an FAA or NTSB requirement for a report from the pilot. The Company files this report with the FAA or NTSB on behalf of the pilot. An electronic report is required within 24 hours of the end of the duty period that the event occurred for a domestic trip, or 48 hours for an international trip. If this is not feasible, the report deadline may be extended if the incident is reported to the FSAP hotline within 24 hours of the end of the duty period where it occurred. For complete information regarding the Irregular Operations Report (IOR), refer to the FOM.
