Verification of residency for new students We require at least three documents to verify residency. Category I - One document showing proper name and address Closing statement for purchase of residence. Closing date must be prior to the registration appointment. Signed current lease with current receipt. Lease must list all school age children living on the premises or a notarized letter must accompany the lease. The landlord’s name, address and phone number must be on the lease. Steps to complete registration: CONGRATULATIONS ON YOUR MOVE TO LIBERTYVILLE! LHS 1. You need to have taken possession of your new residence. Residency is easily defined by where you lay your head at night, not where you will or could lay your head. 2. Three documents to verify your residency within LHS attendance boundaries. See the back page for specific requirements. 3. Birth certificate We are excited for your child to be a part of our school and we hope you are excited too! LHS has a great tradition of academics, athletics, fine arts and service. Moving to a new school can be a difficult and stressful time for a family. We hope that your transition to Libertyville is smooth and that your are ready for the new year. If you have any questions or concerns about your transition to LHS, please do hesitate to give us a call. We will do our best to work with you through this exciting, but often challenging time. 4. Academic records. Hand carried academic records to assist the counselor in helping you choose the most appropriate classes. If you transfer during the school year this should include grades to date of leaving. If your student receives special services you should also bring a copy of the Individualized Education Plan. 5. ISBE Good Standing Form for students transferring from another Illinois public school. Obtain from previous Illinois public high school. 6. Basic registration fees will be collected at your appointment. 7. All freshman and new students transferring from out of state must have a new physical examination on file prior to the start of the school year. 8. Make an appointment with the counselor on duty. Call 847-327-7000, after you have taken possession of your new residence to make an appointment to register your student. All new families meet with a counselor to register. Allow an hour for your appointment. APPOINTMENTS DURING THE SUMMER. In June, a counselor will be available Tuesdays and Thursdays. In July and August a counselor will be available Tuesdays, Wednesdays and Thursdays. The offices are closed on Fridays during the summer. You are assigned a counselor by your last name, but your appointment will be with the counselor who is on duty. During the first weeks of school, faculty from the Student Services Department will contact new families for a New Student Orientation event. Stay tuned! APPOINTMENTS DURING THE SCHOOL YEAR.
