From our President/CEO and Board Chair Porter Hills is honored to serve the needs of residents and family members and blessed to do so with a talented and dedicated staff. We seek opportunities to collaborate in the greater community, to give choices to seniors, fulfilling our mission of being “Trusted Experts Serving Seniors with Grace”. Our vision “to be nationally recognized as an unparalleled leader and partner in senior services” continually challenges us to provide unique and creative services for today’s seniors in our communities and for those in the next 15 years. Innovation We are proud, not only of our outstanding residential communities, but of our ability to assist individuals empowering them to live their best lives, wherever they call home. TANDEM365 TANDEM365, became fully operational in 2015. Through the collaborative efforts of Clark Retirement Community, Holland Home, Life EMS, Porter Hills and Sunset Retirement Communities & Services, seniors with complex medical conditions are assisted by a team to safely stay in their home. The only program of its kind in the country, TANDEM365 currently serves 250 individuals. In the next three years we are anticipating serving 2,000 individuals who have very chronic, life altering conditions. TANDEM365 challenges, and can soon revolutionize, how health care will be delivered. TANDEM365 is achieving 95% satisfaction and has the potential to save hundreds of millions of dollars annually! (continued) From our President/CEO and Board Chair (continued) LifeCircles-PACE Program On Our Campuses Our LifeCircles-PACE program, the Program for All-Inclusive Care for the Elderly, is a full risk, managed care program that is growing again! In existence for six years, LifeCircles currently serves 225 individuals during the day, returning them to the comfort of their home in the evening. They would otherwise be in a long-term care center. Various Federal and State approvals will now give us the opportunity to serve 525 individuals! A new on-campus initiative, a Health Navigator, has been implemented to assist residents in managing the complex health care system when the need arises. Avenues by Porter Hills Our future is bright and our blessings many as we journey together to fulfill these approaches, providing community health supports for every elder in our communities. Avenues by Porter Hills, one of less than 40 similar models nationwide, gives healthy individuals the choice of maintaining their independence in the comfort and convenience of their own home. The program provides a lifetime of continuing care with the security of knowing that if health status changes, services will be available and paid for by their Avenues by Porter Hills membership. Our membership is growing strong with the anticipation of doubling over the next 12 months. We are also excited to be able to extend our service area to the Lakeshore. 3 Two major projects are also in the planning stages that will assist Cook Valley Estates and Porter Hills Village and will implement new models of services for residents on those campuses. Yachcik President/CEO Highlights 2014-2015 July 2014 Cook Valley Estates and Meadowlark Retirement Village scored in the top 10% nationwide in the annual resident satisfaction survey! Nicole Maag, Vice President of Residential Services, was accepted into the Leading Age Leadership Academy. TANDEM365 entered into an agreement for a pilot program with Priority Health. September 2014 Sharon Buursma became Chair and Jay Wisentaner 1st Vice Chair of the Porter Hills Foundation Board. Ann Murar and Daniel Caldwell from LifeCircles–PACE presented at the National PACE Association Conference. Best of October 2014 Marywood Health Center received licensing for the 17 remaining beds expanding their ability to serve those in need of quality rehabilitative services. LifeCircles received formal State approval from the Department of Community Health to increase their participant level from 225 to 350. 4 Porter Hills 2015 Annual Report 2014-15 Readers Poll Porter Hills was named Best Retirement Community in Grand Rapids by the readers of Grand Rapids Magazine. December 2014 LifeCircles closed on the purchase of the Muskegon building. Highlights 2014-2015 January 2015 LifeCircles broke ground in Holland, and is approved by the State for 175 PACE participants. February 2015 Teresa Toland named President/CEO of TANDEM365. March 2015 Marcy Emmelkamp, Social Services Coordinator, received Field Instructor of the Year from Grand Valley State University. May 2015 For the 13th year in a row Porter Hills is named one of West Michigan’s 101 Best & Brightest Companies to Work For. 5 Porter Hills 2015 Annual Report June 2015 The Porter Hills Foundation celebrated its 20th Anniversary at Where In the World…The Prince’s Ball. Guests experienced the transformation of dreams to reality. Together they supported the Angel Fund, providing the “magic” for future endeavors to grow and take flight. P Buite resented by LLC n & Associates Fiscal Year 2015 The plans to add assisted living to the Cook Valley Estates campus continued to move forward including the acquisition of properties surrounding the campus. Financial Summary YEAR ENDED JUNE 30, 2015 At the close of this fiscal year ending June 30, 2015, the financial position of Porter Hills continues to be solid. Total revenue continues to increase primarily from expansion of our home and community based services, LifeCircles-PACE program, and improved census in most areas. Improvements in the economy are positively impacting Porter Hills creating a high demand for independent living. The aging of our society and the desire to avoid skilled nursing is also creating high demand for assisted living. Legislative changes over the last several years are decreasing reimbursement rates in all areas of health care. When insurance reimbursement does increase, it is not adequate to cover inflationary increases of wages and benefits. Despite those reimbursement challenges, the operations continue to benchmark well against the market in financial performance while showing improved outcomes. Cash reserves appear healthy in total but still lag compared to best practices. During fiscal year 2015, Porter Hills provided about $450,000 in assistance to residents who have outlived their financial resources. In fiscal year 2015, Porter Hills continued to provide investment into TANDEM365. Several other recent start-ups, Avenues by Porter Hills, Emmanuel Hospice, and Marywood Health Center are now self sustaining. We also had a very focused effort on developing a design to update the center portion of the Porter Hills Village campus and add assisted living to our Cook Valley Estates campus. Our debt was refinanced through a multi-year approach, requiring some up front costs but resulting in a longer term and lower interest rates. Amortization rates remained the same. We continue to work diligently to strengthen our financial position to ensure that Porter Hills is always positioned to serve seniors. The Investment Committee actively monitors the performance of our investment managers. We are honored to continue our unwavering support of Porter Hills as trusted experts serving seniors with grace. We fulfill our mission of funding for Porter Hills through three critical funds. Porter Hills Angel Fund Our Angel Fund provides funding for the start-up and launch of new Porter Hills programs designed for social impact and financial return. To date, the Foundation has invested $1,400,000 in three Porter Hills projects: •Emanuel Hospice, a collaboration of Porter Hills, St. Ann’s, Clark Retirement Community, and Sunset Retirement Communities & Services to assist individuals who are in the final transitions of life, and their families. Porter Hills Builder Fund Our Builder Fund invests in capital projects with an expected payback. Our first project will launch in spring 2016. The Transforming Cook Valley Estates Project will provide funding for the new construction of assisted living apartments and memory support suites on the Cook Valley Estates campus. Porter Hills Benevolent Fund Our Benevolent Fund continues to assist eligible residents of Porter Hills who have outlived their financial resources. Ott Chaplaincy Fund The Future is Here! Porter Hills was launched nearly fifty years ago with the generous support of forward-looking individuals who helped build the longterm stability of the Porter Hills we know today. The organization is named in tribute to Donald and Ruth Porter who donated the 35 acre parcel of land on which Porter Hills Village lies. The Isabella Foundation generously invested $300,000 to seed the start up of Porter Hills during the earliest days and later created Donors a named endowment fund for benevolent care and over time contributed more than $1 million for various capital projects and vital programming needs. The commitment of these leaders to the future success of Porter Hills cemented their legacy of fostering self-respect, independence and cultural growth among residents. Clark, Porter Hills, St. Ann’s, and Sunset Goodwill Industries of Greater Grand Rapids Certified Nurse Aide Training (CNA) The Certified Nurse Aide training course is located at Porter Hills Village. This training includes classroom instruction as well as laboratory and clinical experience, while working with residents of Porter Hills Village. Grand Rapids Community College & Partners in Action Emmanuel Hospice Personal Assistant Certification Course A collaboration between three faith-based providers, Emmanuel Hospice offers holistic, end-of-life care provided by an inter-disciplinary team. It is a compassionate, caring model for individuals wherever they are at the end-of-life. The Personal Assistant Certification course is offered exclusively to our employees. In addition to providing care, the graduates are skilled in coordination of daily home management and personal lifestyle activities. Dominican Sisters ~ Grand Rapids The GREEN HOUSE® Replication Initiative Marywood Health Center Porter Hills’ GREEN HOUSE® Homes Marywood Health Center offers holistic, restorative healing that focuses on clinical and spiritual care for people recovering from illness or injury. Both Porter Hills and the Dominican Sisters are committed to providing respectful care and comprehensive, innovative services to older adults in the greater Grand Rapids area. The Robert Wood Johnson Foundation awarded a grant to NCB Capital Impact’s Community Solutions Group to support the development of fifty projects across the United States through THE GREEN HOUSE® Replication initiative. At Porter Hills our mission is to provide a range of retirement options for every financial profile. Porter Hills, in conjunction with the Department of Housing and Urban Development, offers several rent subsidized senior living communities.
