In the ever-changing world of healthcare, it is becoming evident that providers of skilled services must create an environment of collaboration and communication in order to improve the quality of life for their patients and remain viable. Unique patient-centered multi-disciplinary approaches are on the rise. Utilization of these types of programs is critical in order to maintain high-quality care standards to maximize patient outcomes and satisfaction. This workshop is designed to cover a wide range of topics in order to provide the participant with an improved understanding of the “big picture” of home health rehabilitation. Participants will learn to overcome common barriers to successful rehabilitation along with development and implementation of dementia and fall prevention programs that work. This workshop will also include practical strategies for documentation across all levels and settings to ensure reimbursement.
