I understand that all the staff engaged in the programme have been subject to CRB checks. I understand that the Summer Passport scheme incurs a single payment of £10 per young person, which is payable at the time of first activity session attendance. The Summer Passport is relevant to Exhibit:A(rts) activities and to all other Thurrock Youth Service activities between 23/07/07 and 31/08/07. I understand that certain activities will incur additional expenses and that these will be stated in advance. I understand that any continued inappropriate, disrespectful or disruptive behaviour on the part of the young person named overleaf, will result in one official verbal warning to both the young person and the parent/guardian, leading to an instant dismissal from the programme if the behaviour then continues. A dismissal will be signed off by two members of staff who have witnessed the behaviour. No refunds will be given.
