We reflect on this past year with pride as we were again able to serve more than 5,000 children and adults with disabilities and those in crisis throughout the Fox Valley. This was achieved despite the $1 million in state funding cuts and another $1 million in state grant funds unpaid due to the lack of a state budget. We had to make many tough decisions this past year in order to continue services and remain financially stable. Now more than ever we are extremely grateful for our many generous donors, volunteers and dedicated staff. Despite the many challenges faced in 2015, several new projects moved forward. Water’s Edge Apartments in South Elgin offers affordable housing to many AID clients. Behavioral Health Outpatient services expanded by taking on both staff and clients from another local agency that closed its children’s mental health program. Supportive housing opportunities expanded for individuals with developmental disabilities and behavioral health challenges; though more than 1,000 individuals remain on the waiting list locally. We hope that a three-year Dunham Fund Challenge Grant will incentivize families, staff, friends and supporters to increase giving to AID this year. Thank you to all who gave so generously this past year! We hope we can count on you to help us reach our 2016 campaign goal. Additionally, we ask that you continue to advocate for the needs of those we serve and those who are not served. This advocacy should be at all levels of government, local, county and state. We need to make our voices heard and remind our elected officials that we are all voters. Your continued support and confidence in AID is deeply appreciated. All services are customized to meet the needs of each child and his or her family. Respite: Assists families of individuals with developmental disabilities, who live at home, by providing short term relief. A voucher system allows families to choose their own respite workers and the services provided to the family member. Audiology Evaluations: Upon referral from a primary care physician, an experienced audiologist evaluates the hearing abilities of infants (6 months or older) and children, using a variety of tests and techniques. School Transition: The State Board of Education approved School Transition Program provides students, ages 18-22, a more supported, meaningful and successful transition to adult services. Community Support Services: Client and Family Support: Offers individualized assessment and linkage with appropriate services both within AID and to other community based resources. Provides temporary, emergency support and services on a “scholarship” basis for individuals who reside in Kane and Kendall counties and Hanover Township in Cook County in need of AID services. Adult Home Based Support: Provides service facilitation and support for individuals with developmental disabilities to obtain/maintain appropriate services and to develop/maintain independent living skills. Families may purchase AID services based on the individual needs of clients. AID ANNUAL REPORT | 2 Employment/Community Integration: Developmental Services and Community Integration: Provides skill training, paid work opportunities, community integration and employment to individuals in order to enhance and maintain their independence in community living. The program provides support, assistance and advocacy for individuals with developmental disabilities. The program provides activities to assist participants in attaining the following outcomes: • Enhance daily living skills through functional educational opportunities • Increase socialization and communication skills through interactive experiences • Improve health and wellness through dietary and physical fitness opportunities • Secure vocational skills • Learn of local resources through inclusive community interactions • Develop relationships and sharpen job skills through community based service projects STARS Program: A collaborative effort between AID and the Fox Valley Special Recreation that focuses on enhancing the quality of life for individuals with disabilities by building life skills through active participation in community based therapeutic recreation. Autism Program: A specialized curriculum that includes communication skills, social and coping skills, functional life skills, community integration, vocational skills and volunteer opportunities. Vocational Training: Assistance for individuals in obtaining and maintaining job training through subcontracts or community job sites. Community Jobs: Employment Specialists assist individuals in developing positive relationships with community employers. Services include: career planning; job development; job seeking skills; assistance with résumé writing; application processes and interviewing skills. Supported Employment: Designed to assist individuals with most significant disabilities to obtain and retain competitive employment in an integrated setting with effective ongoing support services. All individuals earn minimum wage or better. Residential Developmental Disabilities Services: Supervised Community Living: 24-hour home setting for individuals with developmental disabilities who need and want to develop independent living skills. Supported Community Living: Intermittent services based on the individual needs of each resident. Services Provided: • Daily living skills including cooking, self-care, laundry, accessing community services, budgeting, housekeeping and more. • Counseling Services, behavior management and therapy services • Transportation services • Community participation • Medication management monitored by nursing personnel PROGRAMS AND SERVICES Crisis/Community Outreach: Crisis Intervention Services: Walk-in or face-to-face crisis services to help people in distress. Services include assessment, counseling and referral/linkage to community services. Crisis Line of Fox Valley (630-966-9393): Provides assistance to persons with a wide variety of needs and concerns including depression, anxiety, loneliness and substance abuse. Sunshine Calls: Trained volunteers offer phone support to people in need. Regular, scheduled phone calls provide people with social interaction, medication reminders, crisis counseling and referral to community resources. Victims Services: 24-hour assistance to victims of violent crime and/or trauma. Services include: crisis intervention, counseling, advocacy, education, information/referrals. Outpatient Mental Health: Individual and Group Counseling: • Managing the symptoms of mental illness • Couples Therapy • Family and Parenting Issues • Trauma • Managing Emotions • Anxiety • Co-occurring Disorders • Child and Adolescent Counseling AID ANNUAL REPORT | 4 Outpatient Mental Health (cont.) : Case Management: • Assistance with obtaining Medicaid, Social Security and other benefits • Linkage and referral to other community services Psychosocial Rehabilitation: Groups to help people develop skills they need for daily living and for recovering from a mental illness. Examples of groups include: • Stress Management • Weight Loss and Healthy Living • Mindfulness • Living in the Community • Social Skills Psychological Evaluations: Psychologist gives numerous tests and assessments, including face to face interviews to help diagnose a mental health illness and/or developmental disability. Testing also helps determine how and why a person thinks, feels, and behaves the way they do. Psychological tests evaluate strengths and weaknesses in the way a person thinks and behaves as well as assesses intelligence, personality, and behavior. Neuropsychological testing can also be completed to assess and treat behaviors directly related to brain functioning. After testing is complete our psychologist will go over the testing results and make recommendations to take advantage of personal strengths and problem solve weaknesses. Psychiatric Services: Medication management for adults and children with a mental illness. Residential Mental Health Services: Supervised Living: The purpose of this program is to help people with a mental illness live more independently in the community. People live in their own apartments with 24 hour staff support. Services include: • Individual and Group Counseling • Case Management • Help with Medications • Assistance in the Community • Skill Building - such as budgeting, shopping, cooking, cleaning and laundry • Crisis Resolution • Healthy Living Supported Living: This program is the next step in community independence. People live on their own and receive staff support at least once a week in their home. Services are the same as those in Supervised Living but less intensive. 5 | AID ANNUAL REPORT AID SERVED 5,234 CLIENTS IN FY15 CLIENTS BY AGE: 3959 118 436 357 CLIENTS BY RACE 2% Caucasian - 3092 African American - 699 Hispanic - 1125 4% 21% 19% Asian - 86 Other/Unknown - 232 364 13% 14% 59% 6% CLIENTS BY PROGRAM Children and Transition Services - 232 11% Community Support Services - 347 Employment/Community Integration Services - 719 Residential Developmental Disabilities Services - 281 Residential Mental Health Services - 249 Behavioral Health Services - 4391 4% 4% 71% 4% • AID served 5,234 children and adults in fiscal year 2015. • Employment services placed 90 clients in community jobs. • 56 clients participated in the Structured Therapeutic Adult Recreation Services (STARS) program; a recreation/community integration focused program in collaboration with Fox Valley Special Recreation Association (FVSRA). • AID initiated a Micro-Business Group to encourage and support clients in developing their own businesses. Clients with different skills and talents from all programs have the opportunity to market and sell directly to the public in an organized “Client Craft Sale” held several times a year at multiple AID locations. • Clients volunteer at a variety of organizations: Elgin Re-Store, Huntley Animal House, Animal Shelter, Hawthorne Woods Park, Batavia Rehabilitation Center, Batavia Interfaith Food Pantry, Bountiful Blessing Food Pantry, the Cancer Society, Church Food Bank in Huntley, Elgin Recreational Centre, Feed My Starving Children, First Presbyterian Church, Hanover Township, Kenneth Young Center Resale Shop, Marie Wilkinson Food Pantry in Aurora, and others. Volunteering offers hands-on preparation for community employment while bolstering community investment and philanthropy. • The Autism program continues to expand, with a growing waiting list, both in Elgin and Aurora. Clients continue to excel in small, structured environments with visual and positive behavior supports. • Opened 3 new homes: Carl (Elgin), Laurel (Aurora), and Mill Street (Geneva). • Intermittent Community Integrated Living Arrangement (CILA) program, which added 6 new clients this year, focused on nutrition, fitness, healthy meal preparation and making healthy life choices. • 83 children received Audiology Services. • The In Home Children’s program served 38 children. Three clients aged out to transfer to the Adult Home Based program. 7 | AID ANNUAL REPORT • Expanded the Behavioral Health Supportive Living Program to include Northern Kane County (Elgin area); increasing the number of people supported to 140. • Assisted clients to secure residential units: 4 clients to Statewide Referral Network units at Mill Street Station in Oswego, and 17 clients to apartments at Water’s Edge in South Elgin. • Provided Adult and Youth Mental Health First Aid trainings to agencies in the community. Four staff members became Youth Mental Health First Aid trainers. These trainings were also provided to approximately 50 employees at the Juvenile Detention Center in St. Charles. • Entered into partnership with District 131 through a grant designed to improve school and community knowledge of mental illness and improve access to mental health services.
