Basic economic and federal fiscal issues will be discussed, along with the implications for health care reform. The speaker will also review the state-by-state decisions on Medicaid expansion and the impact on providers. Learning Objectives: 1. Have you heard of the POLST or the new Assisted Living Community changes in Georgia? Why is all of this important? After time, these laws often get misquoted as fewer and fewer of us see them ourselves and we depend on hearsay. This presentation will show you the actual laws and apply them to real-life cases. Some of it should be a “refresher” course while we have added some new content/updates for 2013. You will either leave with new knowledge or empowered in your current knowledge by seeing the laws firsthand. Learning Objectives: 1. State two different parts in the Georgia “Next of Kin” Law 2. In particular, this presentation will examine how wording can influence a caregiver’s ability to allow a loved one to die peacefully without heroic efforts. Finally, this presentation will educate the audience on what a natural death is, including looking at proponents for and opponents against using AND (Allow Natural Death). Learning Objectives: 1. Case managers must have an extensive knowledge of local, state, national and international resources to meet the increasing needs of their patient population. In this session, the panelists will discuss a patient scenario that required the development of a creative discharge plan. Discussion will include the multiple barriers to discharge that were encountered and how they were addressed. Learning Objectives: 1. Describe the role of the case manager in the discharge planning process for the case study patient 2. Identify the barriers to discharge for the selected scenario 3.
