Hospital clinicians can see a patient’s primary care history from within the hospital record. This includes demographics, diagnoses, treatments, medications, allergies, results, disease registers, co-morbidities and family history. Puts the hospital data onto the GP’s desktop. GPs can view hospital records for the patient they are consulting without having to load a new system. This includes demographics, referrals, attendances (inpatient/ outpatient, A&E), waiting lists, medications, alerts, allergies, pathology results and radiology reports. ITY UN MM LTH CO HEA Medway CareCentricTM is a version of the CareCentric shared care record software from Graphnet Health, developed specifically for users of the Medway EPR and embedded within the heart of Medway. Medway CareCentric gives Medway users immediate access to a wider integrated digital shared care record. It: In addition, the system is easily expandable. It can be extended to bring in other data sources and create a local shared care record including acute, mentalhealth, primary care, community and social care data. Extensive links between health and social care allow clinicians to look at the care allocated to a patient in their home prior to discharge. Medway CareCentric can also be used as a single source of shared data where clinicians are operating services across multiple Trusts. So for example, clinicians seeing a patient as an outpatient in one Trust and operating on that patient in another will be able to see records from both Trusts. Because of the close integration between Medway and CareCentric, acute clinicians can call up a patient’s details from within Medway, without having to log in again, remember new passwords, or consult an index. The easier and faster a shared care record software is to use, the more information is entered into it, and the richer and more useful the record becomes. You just click on the CareCentric Embedded button in the portal and ‘bang’ - you’ve suddenly got the GP in the room with you….It gives clinicians assurance and back-up. Some of the Benefits Using Medway and CareCentric, a Trust can reduce admissions from A&E and bring down A&E waiting times. This is due to the availability of critical information at the point of care. Patients who are admitted can be treated more quickly in A&E with reduced risk as the shared record is able to provide medication and treatment history. Reducing re-admissions. More integrated discharge management and better information availability for community staff means re-admissions may be reduced, with an impact on 30-day targets. An Out of Hours GP service is able to access details of recent hospital treatment as well as GP notes, so they are able to treat a patient appropriately at home rather than admitting to A&E or asking the patient’s GP to provide a follow-up visit. Reduction in phone calls and copying. Shared records mean that staff in primary and secondary care no longer need to spend time copying records and dealing with telephone enquiries for patients needing treatment elsewhere. The shared care record can be used to coordinate working across disciplines and agencies for anticipatory and end of life care planning. The patient’s wishes can be made known to all care providers and ambulance staff. The shared care record is ideal for multi-agency and multi-disciplinary care pathways for long term conditions, such as dementia, diabetes, asthma, and COPD. Assessment and data collection forms and work-lists can be set up, with alerts and baton-passing to ensure that checks and tasks are not missed. Improved medications management. The availability of medication history across care settings supports faster and safer clinical decisions. Improved patient safety. Decisions are made more quickly, backed up by up-to-date information on medications, tests, and treatment across all care settings. Eradication of paper. Shared information leads to a reduction in printing and paperwork. This cuts time printing and transporting paper, and cuts telephone costs. CareCentric is a datastore custom-designed for healthcare, built for maximum performance, and created with clinical logic to ensure data integrity and context. CareCentric solves the problems created by multiple information silos by bringing information together into one place, and making this data available for viewing. It is a powerful, robust, and proven foundation for sharing healthcare data, preserving data security, but bringing valuable care information to the people who need it. Building the Medway CareCentric Shared Record Medway CareCentric is an out of the box shared care record solution tailored for Medway users. It has been specially preconfigured to accept data from Medway PAS and clinical applications and to present that data to GPs and other care workers. It has also been fully integrated within Medway so that Medway users can look up GP data for the patient they are currently looking at without having to log in to a separate system or reselect the patient record. It has also been preconfigured to take data from GP systems such as EMIS Web and InPS Vision. Other health and social care systems can be added by connecting them using CareCentric’s standard interface formats. There are a range of standard connectors available including RiO and the Liquidlogic adult and children’s systems. CareCentric GatewayTM then presents users with a unified view of the data. It is a web portal, which acts as the gateway to the single care record, allowing multidisciplinary teams from single Trusts or multiple care settings to work together to deliver a coherent service to patients.
