Introduction and Purpose of RFQ:
The Children & Families Commission of Fresno County (Commission) also known as and
referred to in this document as First 5 Fresno County (F5FC) requests quotations for managing
the purchase, shipping and delivery of a quality diaper bag to be given to new moms in Fresno
County.
Please quote prices for 10,000 and 15,000 of the following type of diaper bag:
Precious Cargo ® Diaper Bag (or similar)
• 100% lead-free material
• 2 side open pockets, 2 front open pockets
• Adjustable and detachable shoulder strap
• Color: Black
• No changing pad or stroller straps
• Dimensions: 11.5”h x 12.75”w x 4.375”d; approximately 640
cubic inches
• Customized print on flap: First 5 Fresno County logo (color)
and the following tagline - “Give your baby the healthiest start
in life: Breastfeed.”

Qualified vendors interested in providing this product for F5FC are encouraged to submit a
Quotation. The vendor selected will have demonstrated understanding in providing similar
services, and must be capable of rendering the services in a professional, timely and costconscious manner.
Information about First 5 Fresno County:
California voters passed Proposition 10, the California Children and Families Act in November
1998. The Act provided for a 50 cent per pack tax on cigarettes. The monies collected are to
be used by First 5 county commissions to fund early childhood education programs that
promote early childhood development from prenatal through age 5.
F5FC receives
approximately $11 million per year and is responsible for developing a strategic plan to guide

local funding decisions that are consistent with the intent of the California Children and Families
Act.
Minimum Qualifications:
Vendors must have minimum five years of experience in providing similar services

Questions on the Request for Quotation:
Any questions about this RFQ must be e-mailed to Steve Rapada, Operations Officer at
srapada@first5fresno.org before 5 p.m. Monday, October 13th 2014. Questions and responses
will be posted on the F5FC website by 5:00 p.m. on Tuesday, October 14th, 2014.
Proposal Submission Requirements:
The quotation must be submitted in the following format and meet these requirements. The
entire quotation must not exceed seven (3) typed written pages, including the cover letter with
the brief narrative described below:
1. Include Attachment “A”, the Quotation Standard Form No. 1 – completed and signed.
2. Quotations must be accompanied with a cover letter on agency letterhead and should
contain and certify the following:
•
The company name and address

The name and telephone number of the person or persons authorized to
represent the vendor regarding all matters related to the quotation
•
The vendor has thoroughly examined the RFQ requirements and proposed fees
cover all the services that are indicated
•
The vendor acknowledges and accepts all terms and conditions included in the RFQ
•
The vendor agrees to provide management consultant services in a manner acceptable
to F5FC and as stipulated in the RFQ
•
The vendor and key professionals do not have or anticipate a potential conflict of interest
with the F5FC or the Commission
•
The cover letter should also contain the following statement:
“I/We have read the Diaper Bag - Request for Quotation (RFQ) and fully understand
its intent. I/We certify that we have adequate personnel, equipment, and facilities to
provide the Commission’s requested services that we have indicated we can meet.
I/We understand that our ability to meet the criteria and provide the required services
shall be judged solely by F5FC Staff or Commission.”
A person authorized to bind the business/firm to all commitments made in its quotation
shall sign the cover letter.
3. The quotation must include a brief narrative outlining the consultant’s qualifications and
capacity to provide the requested services. This response must include the following:
•
An overview of your firm’s services approach. The brief narrative should
demonstrate the vendor’s ability to customize services to the client’s needs.
•
Provide a proposed timeframe for the project.
•
Provide names and contact information (address and phone) for three (3)
references. Briefly describe the nature of the engagement/project for each.
•
If applicable, provide names of public sector clients, including state agencies,
authorities, and municipalities. Include at least one such client on your reference
list.
•
Include the quotation prices on specific items as requested
•
Include other expenses or costs associated with the performance of this service
4. The quotation must also include Attachment “B” a current signed W-9 Form

Quotations must be received in the First 5 Fresno County Office on or before Monday, October
20th, 2014 at 12:00 noon PST. All submissions will be time stamped. Quotations received after
the above date and time will not be considered.
All material received in response to this RFQ shall become the property of the Commission and
will not be returned to the vendor. Regardless of the vendor selected, the Commission reserves
the right to use any information presented in a quotation. The content of each vendor’s
quotation shall become public information once a vendor has been chosen.
Confidentiality of Responses:
F5FC cannot guarantee the confidentiality of information submitted by the organization/agency.
In the event that F5FC receives a request for records or court order that F5FC reasonably
determines compels its disclosure of the proposal, F5FC shall provide such records as it deems
appropriate. All materials submitted as part of a vendor’s response to this RFQ become the
property of F5FC.
Miscellaneous Information:
1. Issuance of this RFQ does not constitute a commitment by the Commission to award a
contract. The Commission reserves the right to reject any or all proposals received in
response to this RFQ, or to cancel this RFQ if it is in the best interest of the Commission to
do so.
2. By submitting a quotation, vendors authorize F5FC staff to verify any or all information
and/or references given in the quotation.
3. F5FC staff reserves the right to approve all subcontractors proposed by the primary
contractor. Approval is based on the subcontractor contract language and budget with the
primary contractor.
4. F5FC staff reserves the right, after vendor is chosen, to amend the resulting services as
needed throughout the term of the contract to best meet the needs of all parties.
5. As of the issue date of this, RFQ, and continuing through the public notification that a vendor
has been chosen, all First 5 Fresno County staff and Commissioners are specifically
directed not to hold any unscheduled meetings, conferences, or technical discussions
regarding this RFQ with prospective service providers. “Off the record” contacts can
potentially taint the Commission’s decision-making process. Please do not attempt to initiate
this type of communication.

6. The contact person for each quotation, whether selected for award or denied, will be notified
in writing of the decision. Upon notification of a chosen vendor, staff and vendor will agree
and finalize a detailed scope of work and budget.
7. No formal Bidders’ Conference will be held. Questions may be directed to the Operations
Officer as noted above.
8. Insurance. On or before the Effective Date of any agreement, Contractor shall furnish to
the Commission satisfactory proof of the required insurance (Certificates of Insurance),
which shall include a commitment by Contractor’s insurers that they will mail notice of any
cancellation or reduction of coverage below the amounts herein required by the
Commission, at least thirty (30) days prior to the effective date of such cancellation or
change. Such required insurance shall include:
(a) General Liability. Comprehensive general liability coverage of at least one million
dollars ($1,000,000) per occurrence for bodily injury, personal injury and property damage.
If commercial general liability insurance or other form with a general aggregate limit is used,
either the general aggregate limit shall apply separately or the general aggregate limit shall
be twice the required occurrence limit (i.e., $2,000,000).
