REPORTING INCIDENTS
Important Facts

It benefits the business to know what’s going on, where damage is occurring, near-misses,
accidents and injuries. You can’t fix the problem if you don’t know about it.
Remember

Your incident reporting system should be simple and accessible. It is usually via a register or
form.

Workers should be encouraged to report incidents. Start this from day one in the induction
program.

When an incident is reported, review what happened to identify contributing factors to
determine if this could happen again, either at your shop or at others. Could it happen again
to someone else?

Don’t automatically assume the worker has done something ‘wrong’. We all make mistakes.
If one person can, another could too. Look at the process and if it can be improved.

Without singling the injured person out, let others know what has happened. This can be
done through a toolbox meeting.
Caution
Certain types of incidents need to be reported to the safety regulator (WorkSafe). If they are not
reported, this is a breach of OHS legislation which can result in prosecution and a fine.
