This year our board approved an exciting new strategic plan that will serve as a roadmap for Baycrest’s next five years. Our vision is bold: to lead the world in exemplary geriatric care, science and education. To that end, the plan focuses not just on growing our expertise in elder care, but on how Baycrest will integrate clinical care, research and education to help those in mid-life and younger preserve their cognitive, mental and physical health so that they experience the best possible wellbeing in old age. As an academic health sciences centre solely focused on aging, we are well positioned to lead change in Dr. Anthony Melman, Board Chair the fields of geriatrics and gerontology and to define the future for healthy aging worldwide, particularly in the area of brain fitness. Baycrest will spearhead the design of a system focused on prevention, independence and support that meets the needs and expectations of an informed and health-conscious aging population, and supports the policies of government. The stories in this Annual Report demonstrate that we have the building blocks in place. Our programs teach people lifestyle practices that prevent untoward complications of disease and support others in selfdirecting their own care. Our outreach programs identify and intervene to assist those at risk in our community. Our memory training programs help people maintain or improve cognitive function so that their minds stay in shape as long as possible. Our ambition to be a world leader in healthy aging is greatly supported by the current and emerging neuroscience discoveries by our Rotman Research Institute as well as the experience and knowledge emanating from its Kunin-Lunenfeld Applied Research Unit. Some of these discoveries are being made in partnership with others and all will be applied across the globe. Our staff continuously evaluates operations, makes adjustments, sets new expectations, refocuses and strives for greater heights. Like many other organizations this year, Baycrest was hit by the significant downturn in the economy. We did what we had to do to meet the immediate challenge, and we will continue to make needed budgetary decisions without ever losing sight of our commitment to maintaining the highest standard of care for those we serve. Did I leave my phone in the basement again? What is that woman’s name? of the memory lapses that draw older adults to the Memory and Aging Program at Baycrest. Some need reassurance that their experience is normal; others want to know how to cope with everyday forgetfulness. These are common examples Participants in the five-week program learn the difference between normal and abnormal memory changes. They are taught simple compensating techniques for normal lapses, such as jotting down a reminder of where they parked the car, choosing one specific place to keep the telephone, and associating a name with a narrative that will help them recall it later – for instance, “this person’s name is Rose and roses are my favourite flower.” The program also educates participants about lifestyle changes that have been shown to improve memory – practices such as eating healthy food, exercising regularly, managing stress, staying intellectually stimulated, and being socially active. 50 % risk of Alzheimer’s People with mild cognitive impairment, which puts them at a 50 per cent risk for dementia within five years of diagnosis, are referred to the research-based Memory Intervention Program to help them maintain their independence for as long as possible. They learn to organize their activities by writing down reminders about such things as appointments, tasks they need to complete, and when they should take their medications. Lifestyle changes that may improve memory are also taught. 4 Baycrest 2008/09 Annual Report 60 + brain easily distracted Scientists at the Rotman Research Institute have found a mechanism in the brain they believe explains why older adults are more easily distracted than younger people. For example, older adults may find it more difficult to concentrate on a newspaper article. One solution is to concentrate on one task at a time, advises lead study author Dr. Cheryl Grady. “It may be as easy as turning down the radio when reading, or staying off the cellphone when driving a car.” Baycrest research has shown that adults whose memory function is at the low end of normal use the same brain regions as young adults when retrieving information, but they perform poorly on the task. By contrast, older adults whose memory is as good as that of younger adults use additional brain regions for retrieval, suggesting that the brain may be malleable to help offset age-related memory decline. A new Baycrest study is using brain imaging technology to see if memory training of those at the low end of normal induces a compensatory pattern of brain activity similar to that shown by older adults with high-level memory function. Baycrest 2008/09 Annual Report 5 Wellness Programs Wellness Programs Ida Kestenberg believes what’s good for the heart is good for the brain – and research increasingly supports that belief. at the gym in Baycrest’s Joseph E. and Minnie Wagman Centre for 30 years now and the result is a trim figure and a sharp mind. The 87-year-old has been working out All-around fitness is the aim of the many programs offered by this multilingual centre for older adults who live at the Terraces of Baycrest retirement residence or in the local community. For cardiovascular health, members can choose aerobics, weight training, aqua fitness, dance, yoga or tai chi. For maintaining a healthy brain there are computer classes, a creative arts program, a poetry group, and a book club. There’s even a program where seniors visit local schools to help teach children to read. High in omega-3 fatty acids, salmon, mackerel, sardines and bluefin tuna protect against inflammation and are good for the brain. To subscribe, go to www.baycrest.org/smartaging. 6 Baycrest 2008/09 Annual Report And for social interaction – also good for the brain – the Terraces offers, among other things, movies and concerts, a sing-along group and a billiards club. “I can’t say enough about this place,” says Ida. “It has helped keep me healthy over the years and I’ve made many friends here too.” 2.4 million Canadians Being told you have a chronic disease such as Type 2 diabetes (which affects more than 2.4 million Canadians) can be overwhelming, as can coping with the after affects of a stroke. Baycrest’s various self-management programs are designed to provide clients with the skills and resources they need to meet the daily challenges of their condition so that they can still have fulfilling and satisfying lives. Baycrest 2008/09 Annual Report 7 Community Outreach Community Outreach A long-time season ticket holder for the Toronto Maples Leafs, Joey Bonamasso has seldom missed a hockey game in more than 60 years. and an unsteady gait didn’t stop him. But he did need help coping at home. The Community Outreach Team visited to assess the 85-yearold’s situation and referred him to the Saul and Rae Saltzman Day Treatment Centre at Baycrest, where he was outfitted with a hearing aid, hand splints and a walker to help him move around safely – especially at the Air Canada Centre. Growing deafness, arthritic hands 700 people a year The Geriatric Psychiatry Community Service sees about 700 people a year. A team of psychiatrists, nurses, social workers and occupational therapists provides in-home and outpatient care to older adults who have problems such as depression and memory loss. “Our role is not just to keep people living in the community longer, but also to improve their quality of life,” notes Dr. David Myran, medical director of the service. 8 Baycrest 2008/09 Annual Report In 2008, the multi-disciplinary outreach team made close to 470 home visits to frail seniors like Joey. Team members assessed and recommended solutions for complex medical, functional and social problems – falls, for example, or difficulty with bathing or swallowing, or isolation. They referred some clients to Baycrest Hospital for inpatient assessment and treatment; others were recommended for outpatient care. In cases where a mental health issue was identified, clients were referred to Baycrest’s Geriatric Psychiatry Community Service. The Day Treatment Centre provides rehabilitation and education to frail seniors who are finding it difficult to cope at home. The aim is to improve their physical health and keep them functioning well psychosocially. When they finish treatment, clients are often referred to other resources in the community to help them maintain what they have achieved. Baycrest 2008/09 Annual Report 9 Hospital Programs Hospital Programs By the time Ida Tesler was admitted to Baycrest Hospital, Ben and Hilda Katz Building, last year, she had almost lost the will to live. A persistent infection had so debilitated the 81-year-old greatgrandmother that she and her family were ready to move her to a nursing home. Betty Tesler remembers her mother-in-law being “psychologically, emotionally and physically depleted, and had it not been for Baycrest, she might not be here now.” Ida spent five weeks being evaluated and treated by an interdisciplinary team in the Geriatric Assessment and Treatment Unit (GATU) and 12 weeks as an outpatient in the day hospital. “GATU enables us to tease out all the factors contributing to a patient’s problems, to educate the patient and family about these factors and develop a corresponding treatment and discharge plan,” explains medical director, Dr. Shelley Veinish. “The goal is to have people return to the community and reduce, if not eliminate, recurrent trips to hospital emergency departments or admission to acute care hospitals.” Today, back home and with her health greatly improved, Ida Tesler is smiling, confident and eager to get on with life. Older adults, particularly if they are cognitively impaired, find emergency departments in large general hospitals confusing and distressing. Baycrest clients who develop heart problems, infections, pneumonia or other urgent conditions are treated close to home, in the Acute Care Unit in Baycrest Hospital, where they are known and where the focus is on geriatric care. 10 Baycrest 2008/09 Annual Report 20 years For 20 years, interdisciplinary teams in the inpatient Behavioural Neurology Unit have found ways to calm patients’ agitated and aggressive behaviour in a compassionate and safe environment. Baycrest is a leader in this highly-specialized and growing area of brain disorders management. Five of the unit’s 20 beds are funded by the Ontario government for patients admitted from other nursing homes overwhelmed by residents’ difficult behaviours. Baycrest 2008/09 Annual Report 11 Research Studies Research Studies What is it about music that makes us want to move? are hoping that music training will help Baycrest clients affected by stroke regain lost movement. “Previous research using brain imaging technology shows which parts of the brain are stimulated when healthy people hear music and respond by moving to the beat,” explains Dr. Takako Fujioka. Now, she and her fellow scientist, Dr. Bernard Ross, are investigating whether engaging people who have suffered a stroke in musical activities could stimulate those same parts of the brain and help improve their ability to move their limbs. Two scientists at the Rotman Research Institute Using a keyboard and drum pad, study participants are asked to repeat a basic sound pattern. “I am musically trained so if a client can get a simple melody or beat going on the drums, I can join them on the keyboard and we can have a small concert. It can be a lot of fun and a great sense of accomplishment for the client,” says Dr. Fujioka. Caregivers who look after people with dementia at home report that Internet-based support programs help them cope with the demands of this stressful role. A Baycrest study has found that those who use videoconferencing to connect with each other are more satisfied with the experience than caregivers who meet face-to-face for group support. The videoconferencing group said they liked the convenience of meeting without having to leave home. 12 Baycrest 2008/09 Annual Report 400 attend conference Some of North America’s best cognitive scientists gathered in Toronto in March to report on their work in understanding common threats to memory and other mental powers beyond Alzheimer’s disease. The scientists presented to more than 400 attendees at the 19th annual Rotman Research Institute conference. They discussed how the brain changes with healthy aging and the factors that may influence those changes for better or worse. Topics included the impact on health of disruptions in sleep; how chronic stress impairs memory; the role of genes in aging; how estrogen may protect women’s brains; and how “cognitive reserve” allows some individuals to maintain good brain function in old age. Baycrest 2008/09 Annual Report 13 2008/09 Highlights of the Year 2008/09 Highlights of the Year Speak brain talk In 2008, Baycrest launched Aging, Innovation and the Mind, a speaker series aimed at baby boomers interested in keeping their minds sharp as they age. 90 The series kicked off with an exploration of Beach Boy Brain Wilson’s creative mind and its association with mental illness. The talk was delivered by Dr. Brian Levine, a scientist with Baycrest’s Rotman Research Institute (RRI), and was hosted by Monica Matys of CFTO-TV. In other talks, psychologist Dr. Brian Richards described for CBC Radio’s Nora Young the potential impact on aging of emerging technologies. In a talk hosted by Canada AM’s medical contributor Dr. Marla Shapiro, RRI scientist Dr. Fergus Craik outlined how memory works and how it changes with age. Dr. Carol Greenwood, a scientist with the Kunin- Lunenfeld Applied Research Unit, discussed how food affects the brain with host Charmaine Gooden, a contributing editor at Zoomer magazine. Today 1918 Grow Celebrating 90 Years The forerunner of today’s Baycrest was founded in 1918 in downtown Toronto by a small group of Jewish women who would be gratified – and astonished – to see what their legacy has produced 90 years later. Since those early days when the Ezras Nashim Society established the Toronto Jewish Old Folks home, the spirit of volunteerism, combined with the expertise of staff in elder care and the discoveries of leading scientists, has supported the growth of the 21st century Baycrest, a place known worldwide for excellence in care, research and teaching, particularly in the area of brain disorders. 14 Baycrest 2008/09 Annual Report Prevent reporting infections publicly Managing and preventing the spread of infectious disease is a key part of Baycrest’s commitment to excellence in patient care and patient safety. To that end, the organization publishes its infection rates on the Baycrest website in accordance with the Ministry of Health and Long-Term Care’s new reporting framework on patient safety indicators. Connect baycrest calling Using a mix of English, French and Hebrew, Baycrest resident Jenny Sinder and her son, Julian Grigg, recently engaged in a lively conversation, complete with hand gestures and smiles. Nothing unusual there, except that mother and son live 9,200 kilometres apart. The pair were connected from Toronto to Tel Aviv through Skype, a software application for making free long-distance videoconference calls over the Internet. The call was an experiment in how Skype could be used to bring Baycrest families closer together. To view the rates, please visit www.baycrest.org. Baycrest 2008/09 Annual Report 15 2008/09 Highlights of the Year 2008/09 Highlights of the Year “Sometimes you are the neediest person in the room and sometimes you are the one offering the support.” Titled “Tug of War,” this fine sculpture was gifted to Baycrest this year by the Kimel family. The artist is Prince Monyo Mihailescu-Nasturel of Romania. His bronzes are displayed in public buildings around the world and exhibited in the courtyard of his gallery in Palm Beach. Support caring for the caregivers Dementia is a family affair and adult children who care for parents suffering from the condition need a great deal of support. Baycrest’s Caring for Your Loved One is a free six-week program that brings caregivers together to share experiences and coping strategies. If they wish, participants can continue on in an alumni group. One participant, Patti, says the program allows her to share a laugh – or a cry. “Sometimes you are the neediest person in the room and sometimes you are the one offering the support.” Give Minimize Discuss They’ve been dubbed the “Silver Tsunami.” As baby boomers edge closer to their retirement years, they represent the largest, healthiest and most educated pool of potential volunteer talent the volunteer sector has ever seen. The challenge for volunteer recruiters will be to attract and retain this target group whose members will want opportunities that better reflect their values, experience and skills, and also keep them physically and mentally healthy. BRAVO (Baycrest Research About Volunteering Among Older Adults) is a new pilot study, funded in part by the federal government, which aims to provide more evidence that volunteering is an effective prescription for healthy aging. Previous research has mostly focused on how volunteers feel about what they do; this study will investigate the health benefits. It’s a message volunteer recruiters say could be very persuasive for harnessing the large wave of boomers who will soon have more time to give back to their communities. A study led by Baycrest scientists shows that unhealthy, high-fat meals are not only bad for the waistline, but may adversely affect memory in older adults with Type 2 diabetes immediately after consumption. Published in the July 2008 issue of Nutrition Research, the study suggests that taking high doses of antioxidant vitamins C and E with the meal may help minimize a slump in memory. In 2008, Steve Paikin, host of TVO’s The Agenda, interviewed Dr. Donald Stuss about the brain and how memory works. A worldrenowned neuroscientist, Dr. Stuss retires this year from his position as vice- president of Research at Baycrest. The discussion was so fascinating that TVO re-aired it a few weeks later. Clinician-scientist Dr. Tiffany Chow, Sam and Ida Ross Memory Clinic, joined a TVO panel discussion on Alzheimer’s the night before. Volunteering keeps you healthy 16 Baycrest 2008/09 Annual Report Fatty food fogs memory in the media Lead From upstart to world leader In a relatively short time, Baycrest’s Rotman Research Institute has become a “jewel” in the crown of neuroscience, according to a panel of highly respected experts from Canada, the U.S. and Europe who came to Baycrest last November to conduct a five-year review of the institute. “We view the Rotman as one of the pre-eminent neuroscience research programs in the world,” said Dr. Robert Knight, review panel chair and director of the Helen Wills Neuroscience Institute at the University of California, Berkeley. We’re very proud of the hard work, energy and creativity of our board, volunteers and staff, and appreciate the steadfast commitment of our supporters. Sports fans and hockey icons alike couldn’t get enough of the two-day tournament and draft night extravaganza, presented by Scotiabank, which has now raised more than $7 million in its four-year history. Tribute An Evening of Celebration More than 650 people celebrated 90 years of Baycrest breakthroughs and five leading couples last October in Toronto. For the first time in its 19-year history, the Baycrest Golf Classic, now known as Strokes Fore Stroke, raised a record $500,000 last August. Recognized as one of the best golfing experiences in the Greater Toronto Area, the exclusive group of 72 golfers played their own ball from start to finish. Proceeds from the 2008 event went to the Brain Health Centre clinics. The newly created Sam Ruth Award was presented to Stuart Schipper, who said he was honoured to receive an award named after the former Baycrest CEO and Baycrest Foundation president who started participating in the 100-kilometre single day ride at the age of 80. Walk Play Boardwalk Unites Staff, Residents & Supporters Despite the rain, a record 300 walkers raised $123,000 in the 4th annual Baycrest Boardwalk last June. Protected by umbrellas and rain hats, participants enjoyed a leisurely stroll around the Baycrest campus, with a refreshment stop at the Reuben Cipin Healthy Living Community, and a sumptuous barbecue at the Terraces of Baycrest. “It was moving to see the great turnout of staff, families, caregivers and volunteers all walking together for Baycrest,” said Irwin Wortsman, who chaired the event with his wife, Elayne.
