Just one year ago, this magazine was in it’s infancy, and we are now well on our way to producing the premier fire service publication in the state. This edition continues to feature some of our newest contributors. D/C Frank Viscuso of Kearney discusses incidents at electrical facilities, and expands on his Officer Development series with “Leadership Traits”. This article is a must read for all officers, especially those up and coming officer candidates, whether it be in a career or volunteer department. As summer leads to fall and the brisk nights that lead to winter, FDJC D/C Mike Terpak joins us again and reviews boiler emergencies. Lt. Mike Daley of Monroe Township continues his extrication series that was featured on the cover of our May edition. This edition we step away from our traditional remembrance of a member of the fire service and remember all those responders who gave their lives in the tragic events of September 11, 2001. The impact this day had on emergency services in our country is unimaginable. Countless members of New Jersey’s fire service answered the call that day and for months afterward assisting our brothers in many ways, shapes and forms, and Jersey Firefighters salutes all of you for your service. I was personally involved in managing the funeral of Father Mychal Judge, OFM, the FDNY chaplain who was the first reported victim that tragic day. He was buried in Totowa, N.J. at a Franciscan Friar’s burial site. The Friar that he inspired to serve the fire service, Father John Piccione OFM, (who was remembered in JFF May edition) asked me to assist in the preparation to bury Father Judge, and it became an experience that I live with still today. Being healthy and fit is beneficial not only to the individual firefighter but also to others in the fire department and the general public. Fire departments can play a key role in the physical preparedness of their firefighters – career as well as volunteer – by encouraging participation in a health and fitness program and providing opportunities to do so. PROGRAM COMPONENTS An effective health and fitness program for firefighters can be implemented with a four-pronged approach, involving wellness screenings, health education, physical training and job-specific testing. Let’s take a detailed look at each of these components. Wellness Screenings There are at least two reasons why screenings should be done. First, screenings can collect data on the health and fitness of firefighters that can be compared to future screenings. Second, screenings can identify problem areas and/or medical conditions that may impact a firefighter’s health and/ or job performance. Firefighters should be screened on a periodic basis. Screenings should include measures of height, weight, body composition, Body-Mass Index, resting heart rate and blood pressure, cholesterol, joint flexibility, aerobic fitness, anaerobic fitness and muscular strength. Health Education It’s important that fire departments provide educational offerings for their personnel. Arming firefighters with solid information can give them direction and purpose. A good idea is to use topics that relate to the wellness screenings. For example, workshops can be given on how to decrease blood pressure and how to improve flexibility. Other relevant topics are weight management, basic nutrition, low-back care, injury prevention, smoking cessation, stress management, diabetes and cholesterol. Fire departments can tap into local resources for individuals to conduct workshops. Qualified educators who may volunteer their time can be found at hospitals, colleges, YMCAs, fitness centers and high schools. Nonetheless, it’s wise to check their backgrounds and credentials to make sure that the person is really a resident expert and not the village quack. Physical Training Firefighters should engage in four different types of physical training: flexibility, aerobic, anaerobic and strength. • Flexibility Training. Being more flexible enables firefighters to exert their strength over a greater range of motion. Improving flexibility also allows firefighters to move their joints through a greater range of motion which makes it easier for them to perform job-related functions that might otherwise be difficult such as entering or exiting through a window while wearing restrictive gear. 8 Jersey Firefighter | september/october 2012 • Aerobic Training. Firefighters who have a high level of aerobic fitness can function for longer periods of time at a lower heart rate. This aerobic advantage means that they can perform job-specific tasks with less effort since their heart won’t have to work as hard. Also, aerobic training helps to modify several factors that contribute to the risk of coronary artery disease and heart attack (which is the leading cause of on-duty death among firefighters). • Anaerobic Training. In contrast to the low-intensity, long-duration efforts of aerobic training, anaerobic training involves a series of high-intensity, short-duration efforts. These types of efforts are the ones that firefighters might normally employ in the performance of their jobs. • Strength Training. Increasing strength improves a firefighter’s capacity to execute job-specific tasks more easily and be more resistant to fatigue. In addition, strengthening muscles, bones and connective tissues can reduce the frequency and severity of injuries. Strength training also increases muscle mass and decreases body fat which makes a firefighter more functional. Job-Specific Testing The main purpose of fitness testing is to evaluate physical readiness. Traditional fitness testing – such as that which employs push-ups, sit-ups/crunches and other calisthenic-type activities – is certainly a means of assessing fitness and, to some degree, readiness. However, it falls short of evaluating whether or not someone is physically prepared to perform the strenuous duties of a firefighter. The best way to do this is to administer job-specific (aka fit-for-duty) testing. The fact of the matter is that fitness testing for firefighters must be representative of the job requirements of firefighters. In other words, the tests must measure their ability to perform specific jobs that they’re most likely to encounter in the line of duty. Suffice it to say, push-ups and sit-ups/crunches don’t pass muster. A great example of a job-specific test is the Candidate Physical Ability Test (CPAT). Developed by the International Association of Fire Chiefs and the International Association of Fire Fighters, this pass/fail test consists of eight job-specific elements that are done in a continuous circuit, including a stair climb, hose drag, equipment carry and forcible entry. Furthermore, the CPAT is taken while wearing a 50-pound vest to simulate the weight of self-contained breathing apparatus and protective clothing along with safety gloves and hard helmet. Though the CPAT was intended to vet candidates as part of pre-employment screening, there’s no reason why it can’t be used to evaluate veteran firefighters. Job-specific testing should be conducted at least twice per year, preferably every quarter. Bottom line: By implementing the aforementioned components, a fire department can help its firefighters enhance their health and fitness and, at the same time, maintain a constant state of physical readiness. By Dr. Maz A new treatment approach may end your daily battle and your frustration with chronic fatigue syndrome. Why would a person feel tired all the time? This could have multiple factors associated with it. Feeling tired is a functional problem. What I mean by that is that the body is failing to do what it is naturally set to do. As a result, it will over work or under work causing debilitating symptoms. In most cases routine diagnostic studies do not provide the true cause of these symptoms and patients are “labeled” as having XYZ condition, fibromyalgia, chronic fatigue… What should a person do who is suffering with chronic fatigue syndrome? For instance, Cortisol levels should be high in the mornings and low at night. When it is high you feel energetic. When it is low you feel relaxed or tired. Stress plays a big part in Cortisol levels. Chemical stress such as alcohol or tobacco use, emotional stress such as relationship issues, and physical stress such as simply lack of exercise and being behind a computer or driving all day, could cause “Adrenal Fatigue”. This is a condition where the adrenal glands slow down. According to some literature more than 80% of the population is suffering from this condition. The economic stress, 24/7 workday and popularity of sugar and caffeine are enabling this condition to be a potential epidemic. Studies have shown that many individuals have decreased Cortisol levels in the afternoon which is the reason that they crash, and elevated levels at night causing the lack of sleep. Very good question. Being tired all the time is classified as a chronic condition. Unfortunately, most doctors are very good at treating acute conditions, but when it comes to chronic conditions, they drop the ball. There are not many doctors who are trained in assessing the true cause of “why” these functional symptoms occur. As a result the patients are given medications to simply mask their symptoms or brain firing patterns so they can have more energy or sleep better. Most medications have adverse side effects which have proven to become long term. In addition the body will develop dependency to such treatments. Unfortunately, people watch too much TV and rely on information obtained from the world wide web where pharmaceutical companies advertise about new medications and pills for these conditions and emotionally manipulate viewers into believing in these drugs as a way to fix their symptoms. If you are feeling drained both mentally and physically, experience body aches, mental fogginess, or blood sugar imbalances, there is a good chance that you are suffering from adrenal fatigue. Many of these patients report the use of coffee or other stimulants in order to be able to focus and get transient energy. It can also cause digestive problems. Autoimmune dysfunction, thyroid problems and blood sugar disregulation. So what should people do who have chronic daily fatigue? Good question. The medical and patient model has been to treat with medication, which forces the body to artificially produce more energy. You and I know this is not a cure, these remedies can have severe side effects while putting stress on your organs. We need to help the body to function properly so that these symptoms will potentially disappear. In most cases there is a neurological, structural and metabolic cause and all three must be addressed for optimum results. People should report their symptoms to a doctor who understands functional neurology and functional medicine. Unfortunately, there are not many experts to refer to. These doctors, whom specialize in this area, are trained to order a certain panel of diagnostic tests, that are not commonly ordered, or even worse at times over looked. This usually happens because of the lack of training in this particular area. Based on the results, a trained doctor will use their expertise and assess what is causing the patient’s symptoms. In most cases, the patient has been suffering from some sort of an autoimmune condition. This means the body has an imbalance or an infection that your immune system constantly is attacking. It is very common among people who suffer from chronic fatigue, diabetes, thyroid issues, hormonal imbalance, fibromyalgia and gastro intestinal complications such as irritable bowel syndrome. Patients often complain of being tired all the time even after sufficient rest, and the feeling of indigestion and bloating after eating even small meals. What are these tests that we are talking about? One that we commonly order is a very comprehensive blood test. For example, Thyroid condition is a common problem many female patients have and are taking medications for it as long as they are alive. Usually when blood tests are ordered by most physicians, they look at one marker (TSH) in order to diagnose whether a thyroid problem is present. In our evaluation we look at several markers and assess if the patient has a true thyroid problem or another condition which is causing the thyroid gland to be affected. Is there any other test that is usually done? Absolutely, based on the patients needs and symptoms. A very important test we order is Adrenal Stress Test. Adrenal glands are known as stress glands. They are located on top of your kidneys. Its job is to release the proper amount of chemicals such as Cortisol, Testosterone and Estrogen. These are the hormones in our body that manage stress, maintain libido and control our immune system. Another factor that should be addressed is a chronic infection in their gut. We have seen in many cases that patients have parasitic infection which was over looked for years and once identified and eliminated from the body the patient had a drastic improvement in their quality of life. As you may know 80% of your immune system is related to your gut function. Wow, so how do you treat this? So how do you at MedWell approach these chronic conditions? Shahab, M.D. Jersey Firefighter | september/october 2012 11 safety Oil Burner Emergencies By Mike Terpak, Jersey City Responding and operating at an oil burner incident is a common, everyday occurrence for many in the fire service. During the colder weather months, firefighters will respond to numerous emergencies that involve oil-fired heating systems. For fire departments that experience challenges from oil fire heating systems, fire officers need to be prepared to deal with the common delayed ignition, and the not so common white ghost. This training bulletin is not designed to turn you into a heating system technician. It is designed to provide the firefighter and fire officer with a few operational tips with oil burner incidents. Delayed Ignition: Delayed ignition, puff backs, kick backs, or as they are also called, blow backs, can occur in a heating system when unburned atomized fuel is ignited at the start of a burner cycle resulting in an explosion. The explosion itself can vary in size and effect. It can be as minor as a small thud quickly followed by a puff of smoke. This is the most common. It could also be more intense resulting in the dislodging of the heating units flue pipes filling the basement with smoke, or it can blow open the fire box door sending fire across the room. Well-maintained systems have protective systems in place to prevent such occurrences, but as in any mechanical system, faulty or poor maintenance is a high possibility. OPERATIONAL TIPS for DELAYED IGNITIONS Enroute/Arrival Steps: • Ensure all members are in full PPE/SCBA – delayed ignition incidents have the potential to be dangerous to say nothing of the hazards of the oil-saturated smoke. • Observe conditions and odor upon arrival – With a delayed ignition, there is often a distinct odor that identifies the incident. Also be aware of heavy smoke coming from the chimney, or smoke that may indicate more than a simple puff back. Be prepared, you may actually be entering into a basement/structure fire. Action Guideline: • Assign members to bring in a few fire extinguishers – Extinguisher options should include a CO2, dry chemical, or AFFF extinguisher. • Order a hose line stretched – Fire extinguishers by themselves are not enough. Stretch a 1-3/4 hose line as a backup. • Prior to entering the basement/cellar area, shut the remote control switch to the heating unit – This is usually identified as a red electrical switch/box installed outside the burner room. • Ventilate the affected area. • Shut off the fuel supply from the oil tank into the burner. If approachable, this will generally be a copper hose line that is goes directly from the tank to the burner. It can be strung along the wall, floor or attached to the underside of the first floor joists. Trace out the line and find the shutoff valve. • Avoid operating in front of or near the burner unit – the heating unit can violently pulsate and force the firebox door open injuring a firefighter • Extinguish any fire outside the burner. • Monitor any fire in the firebox and allow it to burn itself out - Applying any water in the fuel box can result in devastating steam/oil burns to your firefighters. • Search all floors for any overcome building occupants. • Check for any fire extension – Thoroughly check and remove any 12 Jersey Firefighter | september/october 2012 affected combustibles in the basement and around structural members in the basement area. • Check for any fire extension in and around the flue pipe. • Check for a possibly chimney fire. • Check for fire extension on the floor directly above. • Always use utilize thermal imaging when checking for fire extension. • Once under control – advise the homeowner of findings and the service needs of the heating system • Check CO levels prior to allow occupants and owner to re-occupy the building. • If the building cannot be reoccupied due to a loss of heat, assist the building owner/tenants with relocation to a warm shelter. (i.e. Red Cross, etc.) White Ghost: A white ghost is a situation that describes a lethal combination of vaporized oil, heat and air, all in a confined space. The right combination of the three can produce an explosion and accompanying fireball that can relocate the structure’s walls. Although their occurrences are few and far between, they are the type of incident that you will never forget. A white ghost is a white cloud of super heated vaporized oil that is released from the combustion chamber of a heating unit. As we mentioned above, when a heating unit fails to ignite the vaporized oil in the fire box, a delayed ignition or puff back can occur dislodging the units flue pipe or forcing opening the combustion chamber’s door. These openings will allow the highly heated oil cloud to escape into the basement/cellar of the structure. As the vapor escapes from the openings, it will expand as it is heated by the chamber walls and flue pipe. This release of oil vapor creates an explosive atmosphere that is seeking the right combination of air and heat to ignite. If it finds one, the results can be devastating. OPERATIONAL TIPS for WHITE GHOSTS Enroute/Arrival: • Ensure all members are in full PPE/SCBA – These can be devastating incidents causing severe damage and death. • Observe smoke color, location and any accompanying odor. A strong smell of oil accompanied by what appears to be a white cloud from the basement is indicating a white ghost. Action Guideline: • Evacuate all members from the area/building – This should include the basement area as a minimum. • Evacuation may include the entire building – If conditions indicate the need to evacuate the structure, treat the incident as a defensive operation until sufficient ventilation and cooling can take affect. • Ensure protective hoselines are stretched and charged. • When possible, attempt to shut down the heating unit at the remote switch. • Attempt to eliminate all ignition sources. • All mitigation steps must be done under the protection of stretched and charged hoselines. • Ventilate the area. • Operate a fog stream to cool the vapor cloud and space. • Be prepared to deal with an explosion, extending fire, and a possible structural collapse. • Incidents that involve super heated, vaporized oil are just two more examples of situations that fire fighters and fire officers need to be prepared for. To date, he has photographed over 1,000 fires and his work has been featured in numerous newspapers and magazines. His son, Chris, has followed in his footsteps and photographs fires, as well. Tompkins owns Peters Camera Shop in Bergenfield, and in recent years he and another photographer have been putting together a unique 9/11 photo shoot involving various fire departments. The photo shoot, which will continue this year, is an effort to give back to those brave men and women who too-often make the ultimate sacrifice, as they did on Sept. 11, 2001. JerseyFirefighters: In general, when did your interest in photography start? Tompkins: Photography for me, in general, started back in high school. In fact, I took it as a cake course. I figured I could squeak on through without doing much of anything. But I got the bug and I really started getting into it. The camera shop that I now own here in Bergenfield was where I had worked for the previous owner. The interest just kept growing. JerseyFirefighters: When did you first start doing fire photography? And what led you into that? Tompkins: I guess I always had an interest in the fire service. My grandfather was a captain with Engine 9 in Paterson. JerseyFirefighters: Okay. You did tell me that. Tompkins: He was in Engine 9, and then he went to combustibles. He retired, I think, about ‘60 or ‘62. It was a ways back. But he also lived four doors away from the Trenton Street Firehouse. Q& A JerseyFirefighters: Okay. Tompkins: So, when I was like ye high we would go over and visit and it was like, “Okay, Grandpa. Take me to the firehouse.” So, that’s where the interest in the fire service started. It was long before photography. I always thought of fire photography as being quite challenging, because it’s never the same. So, I got a scanner and started chasing the local stuff, and then my first published picture was on the cover of The Bergen Record for a Teaneck fire. So, it took off from there. JerseyFirefighters:: What year was that? Tompkins: I believe it was 1980. JerseyFirefighters: Other than The Record, what other publications have you had your pictures published in? Tompkins: Almost everything I can think of. As far as newspapers, I’ve been in The Herald, The Record, The Post and the local weeklies. All of the major fire publications; First Responder, the magazines, Engineering, Firehouse, Fire Rescue, etc. I just can’t remember. It’s been so long. JerseyFirefighters: Do you have one memorable event that you covered that jumps out at you? Tompkins: There are actually so many. Probably the first one that really did it was the Fulton Street fire in Paterson. I mean, that was just so immense and so spectacular that it really cemented everything. I was just walking around going “Wow,” as I was watching what the guys were doing. It was just incredible. JerseyFirefighters: That was 1977, I believe. Right? Tompkins: ‘77 or ‘78. JerseyFirefighters: So, you’ve been around the block a few times, huh? Tompkins:: Oh, yeah. JerseyFirefighters: I know today that we feature your photos and your son, Chris, got bitten by the same bug. How did that come about? Tompkins: I find that ungodly cool. I mean, most of the time when kids hit their teenage years their parents are not cool whatsoever. But it’s just so great to be able to have a similar interest and be able to do this stuff together. And he’s not cutting corners. He’s doing it the way it really needs to be done to be done the right way. I’m very proud of him. He’s doing a hell of a job. 14 Jersey Firefighter | september/october 2012 To Advertise Call: 201.881.5100 JerseyFirefighters: Good. Obviously, you’re in the business professionally. How does your role as a photography business owner and your passion for fire photography mix? Tompkins: Well, that more than the modus apparatus. I’m not big into that. I will do it, but it’s really the fires that I find the most challenging and the most interesting. JerseyFirefighters: You’re able to tie that back to the business, at times? Tompkins: To a point. It’s so specialized that it’s tough to do it, but I do get the other fire photographers coming in and asking questions. But it’s far from being a main part of the business. JerseyFirefighters: Did you do any photography in New York on 9/11? Tompkins: Not there. No. I was already here at the store when they hit and I just thought it was going to be so well-covered. Then it ended up shutting down Manhattan, so I didn’t even try. Although, since then, we’ve come up with an idea. Myself and Eddie Gray, who is a heck of a video fire photographer, have been doing a 9/11 photo shoot that a lot of departments have taken part in, because they find it so meaningful. We go down to Jersey City and we shoot the rigs with the contributing light in the background, either with or without the guys. It’s really struck a nerve with a lot of the guys and it’s just something that we do to give back. We give them cards and free CDs. It’s just something that we do every year. JerseyFirefighters:: How many are you expecting this year? Tompkins: You’ll have some that will show up without telling us they’re coming. But, so far, we have 30-something that have said they are coming. It’s scary. Who knows? JerseyFirefighters: Maybe you’ll top last year’s number. Tompkins:: We’ve got them coming from all over. I mean, from places I never heard of. I think one is Continental Village. It’s somewhere on the Putnam/Westchester border. They’re coming down. And I don’t have a definite on it, but possibly the Air Force base from outside of Atlantic City might be coming up. JerseyFirefighters: And that’s in progress right now? Tompkins: Yes. It just sort of amazingly took off. Last year, we shot 103 rigs. JerseyFirefighters: Wow. Tompkins: Exactly. It’s just incredible. You know, it’s such a unique thing. JerseyFirefighters:: Wow. This month we will focus on gaining access to perform patient contact and treatment, efficient tool usage and operation, and we will highlight some simple procedures to perform on the emergency scene. 1 2 3 4 16 At this point in the incident, the on-scene surveys have been completed, hazards have been secured or abated, and vehicles have been stabilized horizontally, vertically, and internally. It is imperative to get medically trained personnel into the vehicle to begin to stabilize the patient while the disentanglement takes place. The quickest method to achieve this is to remove the glass, from a location furthest from the victim, so that any broken glass is kept far from the patient. Once the glass is compromised, all glass should be placed under the vehicle, when applicable. To do this, it is paramount to identify what type of glass is in the vehicle: Tempered Glass is found most frequently in side and rear windows. It is single layer, but stressed on the outer surface. This glass will shatter into small pieces when broken, so it is advisable to cover the glass with duct tape, spray adhesive or sheets of adhesive paper so the glass can be collected and removed without exposing the victims to sharp edges (photo 1). Rescue teams carry spring loaded center punches to break this type of glass, but the point of a Halligan Tool or any other pointed tool can work as well. Laminated Glass is found most commonly in front windshields. This type of glass is constructed of polyvinyl butyral plastic sandwiched between two layers of glass. When this glass is broken, it will remain together as a unit but will crack and tear. This type of material can be removed with a variety of tools, such as an axe, a reciprocating saw, a pneumatic windshield saw, and even a hand saw (photo 2). Enhanced Protective Glass (EPG) is a newer class of laminated glass, located in side and rear window assemblies. These glass panels will react in a similar fashion as laminated windshields, as the construction methods are very similar. Most rescuers will attempt to attack the side and rear window panels as if they are constructed of tempered material, but two or three attempts with a center punch on these panels will yield little more than a few small marks in the window. Furthermore, these panels will usually be marked in the lower corner of the window where the word “laminated” or “EPG” may be present. Once the presence of EPG is found, removal can be performed with the same tools used for laminated glass panels. Jersey Firefighter | september/october 2012 While patient care is being performed, crews can begin to make purchase points, spaces in the vehicle that the spreaders can be placed to displace the vehicle’s materials. One method of setting purchase points is with the use of the Halligan Tool and the Flat Head Axe, or the “irons”. The Halligan Tool is set into place with the Adze end of the tool into the gap, usually between the door and the post. The tool is driven into place with the Flat Head Axe or other suitable tool, and the shaft of the tool is then lifted up and down, in order to create a larger “point” for the spreader tips to be placed (photo 3). Another method to gain a purchase point is by using the hydraulic spreaders. In one method, the tips of the spreaders are placed between the top of the door skin and the “A” post, the rear of the hydraulic tool is angled upward, and the spreaders are then opened (photo 4). This vertical crush action will begin to displace the door outward, providing a point for the spreader tips to begin to attack the hinges on the doors. Another technique that is successful is by opening the spreaders as wide as possible, and using the tool’s arms to compress metal together. The spreader arms can be placed on a front fender, in a location just behind the center of the front wheel, and by squeezing the fender together a purchase point can be made to access the hinges. To access the pin side of the door, the spreaders are opened and placed diagonally onto the door assembly, with the tip side of the tool pointed closely to the approximate pin location. By squeezing the door assembly, the metal will deflect and expose the striker pin and Nader Safety Latch on the door post assembly. No matter what technique is employed, keep in mind that the “golden hour” and the “platinum ten” clocks are still ticking away; use a technique that can be deployed quickly and efficiently. Now that purchase points have been made, it is time to displace some metal. A few safety points to consider when deploying hydraulic extrication tools: 1. DO NOT position yourself between the tool and the vehicle. When the tool begins to displace metal, the tool can react and shift towards the vehicle. Once it pins the rescuer against the vehicle, serious injury can result. 2. Full PPE is a MUST. There will be flying glass dust, shards and splinters of metal, plastic and other materials reacting violently on the scene, and a rescuer can get injured quickly. There will also be the potential of exposure to bloodborne pathogens and body fluids; protect yourself against ANY potential injury and exposure. 3. Be aware of your surroundings. There will be plenty of activity around the rescue scene, and some actions will produce reactions that will have to be addressed (i.e., loose cribbing materials, tool proximity to patient, etc…). 4. COMMUNICATE! Noise, confusion and tension run rampant on-scene. A good method of communication during disentanglement incorporates hand signals. For example, teams of two are commonly assigned to To Advertise Call: 201.881.5100 disentanglement tasks. One rescuer operates the tools, while the second backs up the first rescuer, and places one hand on the operator’s shoulder. A common signal technique uses a series of shoulder taps: • One Tap means STOP!; • Two Taps means continue to operate; • Three Taps means we need to get back NOW, and I will explain it to you later… Door Removal The rescuers begin the maneuver by setting purchase points in the most accessible area of the pin assembly. The tips are set into place, with the tool assembly just short of perpendicular to the door. This position will allow the outer spreader arm to displace the door in an outward fashion, fatiguing the striker pin assembly and causing it to fail (photo 5). Once the striker pin assembly has failed, the tips are then placed between the post and the door to tear the door from the hinges. Placing the spreader tips inside the hinge itself can cause the large hinge pin to fail, spreading the hinge assembly to the point of failure of the large hinge pin. Once the door is displaced, it may be necessary to cut any wiring that runs from the passenger compartment assembly into the door for power window, door locks and mirrors. NOTE: This example starts the displacement at the pin side of the door, but the evolution could begin at the hinge side of the door, based on accessibility on the accident scene. Roof Flap and Removal It may become necessary to provide more space to work around the patient for removal. One method to gain more room is to “flap the roof”. This maneuver requires two rescue teams, one at each side of the roof assembly. Rescuers utilizing the cutters will cut the “A” posts, as low to the dashboard area as possible. Once these posts are cut, it may be necessary to support the roof, so it does not drop down onto the patient. Horizontal relief cuts are then placed behind the patient, and in front of the “B” post of the vehicle. A rigid pipe or suitable tool (Pike Pole, Wrecking Bar, pipe, etc…) is then placed across the roof at the relief cut location. Rescuers then lift up on the front of the roof and “flap” the roof backwards (photo 6). Large SUV’s, station wagons and multi-passenger vehicles are common candidates for roof flap operations. While this is a rapid method of gaining more space, there are some drawbacks: first, when this maneuver is performed on a high-speed roadway, the winds of passage from vehicles traveling past the incident can cause the roof to fold back down on the victim, and the rescuers. It is advisable to tie the roof down to the rear of the vehicle. Secondly, the “flap” may not be feasible due to roof damage from roll-over incidents; in this case, rescuers are best served by removing the entire roof assembly. In this scenario, “A” posts, “B” posts, and “C” posts (4 door vehicles) are all cut as low as possible, and the entire roof assembly is lifted completely off of the vehicle. While this will provide an enormous amount of room, more rescuers are needed to lift the assembly off of the vehicle. If the scene is short staffed on personnel, it may be safer to perform the roof “flap”. Dashboard Push During frontal collisions, it is possible that the front/ dashboard area can become displaced on top of the victim in the vehicle. In this scenario, it will be necessary to “push” the dashboard off of the victim. Access to the vehicle’s rocker panel and “B” post assembly is a necessity. A hydraulic extension ram is placed between the “A” post and the “B” post in a diagonal position; the cylinder/base of the ram is placed at the bottom of the “B” post, while the traveling/extension end is placed on the “A” post, just below the top of the dashboard. If no “B” post is present, it will be necessary to use a ram with an extension to maximize the travel of the cylinder and displace the dashboard (photo 7). It is more efficient to utilize two rams, one on each side of the vehicle, but if only one is available, then it should be placed on the side dictated by the medical needs of the victim. At this point of the maneuver, an important question is raised: Is there a need for a relief cut in the bottom of the “A” post? The deciding factor is based on the needed travel of the dashboard assembly. If the front of the vehicle is displaced, and the victim can be removed by restoring the dashboard back to the original position, then the relief cut may not be needed. However, if the lower extremities of the victim are entangled in the pedal assemblies, then it will be necessary to displace the dashboard beyond its original location to access the pedal area. In the latter case, a relief cut would be needed to facilitate this space (photo 8). 5 6 7 Conclusion The skills and techniques presented here are some of the more common maneuvers that rescuers have utilized in the successful disentanglements of many victims. They are also building blocks for more intricate maneuvers that may be required on-scene. Next time, we will discuss a few different techniques that can add speed, space, and efficiency to your rescue teams’ operations. The most likely answer would be that some true leaders are born, but most are made – “self-made”. A smart fire officer will concentrate on developing the leadership qualities and traits that are necessary for success. For the most part, leadership traits in any profession are universal. In the fire service, however, there are specific qualities that an officer absolutely must possess. In this article, we are going to discuss 12 leadership traits that firefighters – and the public – look for in a fire service leader. Officers, and firefighters in general, who regularly demonstrate these leadership traits, will earn the respect and confidence of their peers. By possessing these characteristics, you will make it easier for people to want to follow you. The less time you have to spend on getting others to follow you, the more time you get to spend refining exactly where you want to go and how to get there. On the following pages you will find definitions of each trait, followed by suggestions on how to develop the areas where you feel you are lacking. The 12 Leadership traits are: • LOYAL • SELFLESS • EDUCATED • TOUGH • ADAPTABLE • EMPATHETIC • DETERMINED • ASSERTIVE • ENTHUSIASTIC • COURAGEOUS • RESOURCEFUL • HONORABLE LOYAL because situations escalate quickly on the fire ground. A “routine” fire can turn into a life-threatening situation in seconds, which is why there is no such thing as a “routine” fire. Your ability to evaluate and revise your strategies on the fly is imperative. Suggestions for Improvement: Again, never stop educating yourself. The more you know, the better equipped you will be to react and adjust. Practice being proactive, not reactive. The more capable you are at seeing problems before they happen, the more effective you will be as a firefighter. DETERMINED Definition: Being determined means you have laser sharp focus on the goal you are trying to achieve. There’s a saying, “Brick walls aren’t there to keep you out, they’re there to see how bad you want to get in.” That’s the way an aggressive, strong-minded firefighter should think when performing a duty on the fire ground. The word Determined, with regards to firefighting, is synonymous with Heroic. Suggestions for Improvement: Never, ever, give up. Don’t stop until the goal has been achieved. This does not mean you should ever defy the IC’s orders on the fire ground if there is a need to change tactics. It simply means, when firefighters say they risk a lot to save lives, they don’t just say it… they live it. Definition: Being loyal means you are devoted to the fire service, your community, your department, and your crew. Loyalty should flow evenly, up and down the chain of command, to seniors, subordinates, and peers. To be loyal, is to be dependable and trustworthy, which means that you can be relied upon to perform your duties properly, and trusted to complete your assignment, every time. ENTHUSIASTIC Suggestions for Improvement: Refrain from discussing the problems of the department (or your company) with outsiders. Never talk about seniors or superior officers unfavorably in front of your subordinates. Also, carry out every task to the best of your ability regardless of whether you like, or agree with it. Once a decision is made, and you are given an order to execute it, carry out that order willingly as if it were your own. Suggestions for Improvement: Understanding and believing in the mission of the fire service will add to your enthusiasm for the job. Our mission is to reduce the loss of life and property, and protect the weak. To do this, everyone will have to fulfill a role on the fire ground. It’s important to understand why even the uninteresting jobs must be done with the proper attitude. EDUCATED Definition: Being educated means that you continue to acquire the appropriate information from reliable sources, and that you understand your fire ground duties; the policies and procedures of your department; and the science of firefighting. Your education should be broad, and you should NEVER stop trying to improve your knowledge base. The day you think you know too much, is the day you should consider retiring. Suggestions for Improvement: Increase your knowledge by committing to continual learning and remaining alert. Constantly listen, observe, and learn about things you don’t understand. Read fire service text books, attend educational seminars, talk to street-smart firefighters, and ask questions. There is an endless supply of knowledge available. You just have to seek it. ADAPTABLE Definition: Being adaptable means you are able to quickly adjust to rapidly changing conditions. This trait is paramount for all firefighters 18 Jersey Firefighter | september/october 2012 Definition: Enthusiasm is defined as a sincere interest and exuberance in the performance of your duties. If you are enthusiastic, you are optimistic, cheerful, and willing to accept challenges. You are the type of person who is eager to take on more responsibility. The fire service needs more leaders with this trait because enthusiasm is contagious. RESOURCEFUL Definition: Being resourceful means you are capable of skillfully, safely, and promptly navigating your way through a variety of situations, regardless of the tools, staffing, and resources that are – or aren’t – available at any given moment. If you are resourceful, you are creative and you will always be looked at as a person with ingenuity that shows initiative, and can get the job done, no matter what. Suggestions for Improvement: Think outside of the box. Don’t get tunnel vision. There are always alternative ways to accomplish a task. Once again, being knowledgeable is the key to success in this area. It’s not enough to simply know what tools are available. You must know how to use them. To improve your resourcefulness, you must also work on staying mentally and physically alert. SELFLESS Definition: The true definition of the word selfless is having little or no concern for oneself. Firefighting is arguably the most selfless job on the To Advertise Call: 201.881.5100 planet. It’s our job to put the safety and well-being of others ahead of our own. Although selfless is synonymous with self-sacrifice, this doesn’t mean you should disregard safe practices. To be selfless also means you are considerate of others and openly give credit to those who deserve it. Suggestions for Improvement: Think more about others and less about yourself. Avoid using your position or rank for personal gain, safety, or pleasure at the expensive of others. Be considerate of those you work with and give credit where credit is due. It’s amazing how much your team can accomplish when you praise often and in public. TOUGH Definition: To be tough is to be strong and resilient; able to withstand adverse conditions. With toughness comes the ability to endure great strain without breaking. This means both physically and mentally, and it’s a necessity for firefighters of all ranks. There is nothing easy about fighting fires. If a person is not tough, this simply isn’t the right profession for them. Suggestions for Improvement: You have to learn to love the fight itself. Champions in every arena love to compete. However, it is vitally important to cultivate your love of the fight more than your love of winning. This way of thinking will help improve your mental toughness. Improving your physical toughness will come from a consistent workout program. EMPATHETIC Definition: It’s been said, “Firefighters are constantly meeting new people and spending the worst moments of their lives with them.” Don’t lose sight of that fact. Your job isn’t just to put the fire out, it’s also to provide guidance for the people we serve and protect. Showing empathy means being kind and tactful when dealing with people who have just lost property, valuables, or worse – loved ones. Suggestions for Improvement: Treat others the way you would like to be treated. ASSERTIVE Definition: To be assertive, is to be aggressively self-assured. Assertive firefighters are confident. They know what to do on the fire ground (and around the fire house), and they know how to do it. More importantly, they don’t need to be told to do it. They just do it, and when they recognize an unsafe practice, they are confident enough to stop the act before someone gets hurt. Suggestions for Improvement: Know your job. Take a moment to think about your options then be aggressive and self-assured when you make a decision. You should be firm, but remember the third trait of a strong fire service leader is that you must remain adaptable, so you can quickly adjust your strategy when necessary. COURAGEOUS HONORABLE Definition: To be honorable means you are worthy of honor and high respect. This is a byproduct of the other traits listed above. When you put honesty, integrity, sense of duty, and sound moral principles above all else, you are operating in an honorable and ethical way. Suggestions for Improvement: Be absolutely honest and truthful at all times. Stand up for what you believe is right. Always do the right thing. If it doesn’t feel right, it probably isn’t. If it is not legal, moral or ethical, DON’T do it! Because it is important to always remember these essential leadership traits, use the acronym LEADERS TEACH. Each letter in the acronym corresponds to the first letter of one of the traits. By remembering this acronym, you will be better able to recall the traits. The irony of this acronym is that it is actually the 13th trait of a good fire service leader. They TEACH others what they know. By teaching, you instill confidence, Eleanor Roosevelt said, “A good leader inspires people to have confidence in the leader, a great leader inspires people to have confidence in themselves.” There are obviously other important traits for firefighters, like humility, good judgment, strong motivational skills, and the ability to communicate, but the ones listed above are vital to your success as a leader. Your skill at exhibiting these qualities will be firmly linked with people’s desire to follow your lead. Exhibiting these traits will inspire confidence in your leadership ability. Not exhibiting these traits or exhibiting the opposite of these traits will decrease your leadership influence with those around you. Traits associated with leadership failure To be thorough, we should take a moment to discuss some of the traits associated with failure. As important as it is to understand what makes a good leader, it’s equally as important to understand why so many fail in leadership positions. Here is a quick list of some traits associated with poor leadership. • Lack of passion • Unclear vision • Poor communication skills • Risk aversion • Callous • Unethical • Poor self-management • Incompetent • Plays the victim • Tears others down • Micromanager • First to take the credit • Last to take the blame When someone in an authoritative position displays poor leadership traits, the mission they are trying to accomplish is doomed to fail. A fire service officer will never earn the respect of his or her crew unless they develop the qualities that align with those of great leaders. “A good leader inspires people to have confidence in the leader. Courage is what allows you to remain calm while recognizing fear. Moral courage means having the inner strength to stand up for what is right and to accept blame when something is your fault. Physical courage means that you can continue to function effectively when there is physical danger present. When firefighter’s under your leadership look at some action you have taken and think, “That just goes to show why you are the one in charge”, you are on the right track. If these moments are infrequent, it is likely that some demonstrations of competency will help boost your leadership influence. By consciously making an effort to develop and exhibit the traits listed in this article, people will be more likely, and willing to follow you. By exhibiting these traits on a regular basis, you will earn the respect of others and increase your effectiveness as an influential leader. Suggestions for Improvement: You can begin to control fear by practicing self-discipline and calmness. If you fear doing certain things that are required in your daily life, force yourself to do them until you can control your reaction. Lead by example. It doesn’t take a hero to order someone into battle; it takes a hero to lead others into battle from the front line. Lastly, keep in mind, courage isn’t the absence of fear; it’s the management of fear. The event was open to the public, firefighters, EMS and their families. There was face painting, shell painting, a bouncy house and portable maze for the children. Traditional block party food was available. Newly designed fire and EMS t-shirts were also available. If you are interested in additional shirts go to Surf City FD face book page or the department web site. Special appearance was made by Fire Station 343 honoring the fallen members of the FDNY. There was a silent auction of donated items from local businesses and local sport teams. Home baked goodies were also available. Many surrounding communities came by and christened the truck. The new Engine 2 replaces a Pearce pumper that has been rededicated as Engine 6. One of the highlights of the celebration was the ‘dunk tank’. Residents who made a donation to the fire department got multiple chances to dunk a member of the North Arlington Engine 2. The unit is equipped with a light tower, coffin style compartments and rehab awnings on both sides of the apparatus. In Howell Township, Southard Fire District 3 placed into service a 2012 Spartan/Rescue 1 heavy rescue unit that also assumes Hazmat, scuba, RIT and command post duties. It has a 4-bottle air cascade system, 35 kW generator, 9000-wt light tower and a 9000-lb capacity portable winch with four receivers. In addition, there is a 7-man cab plus three seats in the command post area, two rehab awnings, coffin style compartments and roll-out compartment draws under the box and LED lighting. With the assistance of Homeland and Port Security funding, the Camden County Office of Emergency Management placed into service a Mobile Ventilation Unit. The large-diameter positive pressure ventilation fan is mounted on a roll-off assembly using a 2012 Ford F-550. This unit, which is the only one on the East Coast, can be used for incidents involving large structures, tunnels, barges, ships, etc. The main Super Vac fan is rated at 250,000 CFM entrained at 4,000 RPM. It has a Detroit Diesel DD13 500-hp engine and Allison EVS4000 P automatic transmission. Other features include a Harrison 10 kW generator, back-up camera, rear mounted booster reel, front discharge and three crosslay hose beds. Paramus Truck 4 was automatically dispatched due to Maywood Truck 17 being OOS for the last year. Shortly after Maywood companies were dispatched, the Hackensack Fire Department dispatched a first alarm assignment to the area of Central and The Esplanade for a reported smoke condition. The location of the house fire was one block from the Maywood/ Hackensack border. Rochelle Park chief arrived and confirmed the working fire in a 2.5 story dwelling. Heavy fire was visible from the second floor and rear of the structure.Maywood ass’t and dep chiefs arrived on scene shortly after. Maywood Engine 19 arrived first due and immediately began an aggressive interior attack. Hackensack Ladder 1 and Engine 2 arrived shortly thereafter and immediately went to work. As companies became engaged, Rochelle Park Engine 1 and a Saddle Brook FAST Team were dispatched to the scene, while Lodi and Garfield engine companies covered Maywood firehouses. The fire was under control in 40 minutes, however salvage and overhaul operations continued until 10:30 p.m. due to the large amount of storage in the attic and throughout the second floor. The fire origin appears to be in the rear of the dwelling. Electrical power ranging from 115,000 to 500,000 volts is carried great distances on towers to substations where the voltage is stepped down by the use of power transformers, to sub-transmission or distribution voltages before being sent above ground or underground to their industrial and residential customers. Electrical facilities can be found in any municipality throughout America. As with downed power lines, fires involving electrical facilities such as substations require extreme caution. They should be treated with the same caution you would a delayed bomb. The first rule at electrical facility fires is never enter a substation or approach an electrical facilities until a local utility company representative indicates it is safe to do so. Substations carry hazardous materials, such as asbestos and PCBs, and have electric equipment that may contain large quantities of oil or natural gas. A large quantity of oil in failing electrical equipment can cause a boiling liquid expanding vapor explosion (BLEVE). Explosions at these facilities can be powerful enough to project steel and other equipment great distances. Again, it’s important to maintain a safe distance until local utility personnel respond and make the facility safe for the fire department to use conventional firefighting techniques. Until then, fire personnel should secure the area, and wait. Attempting to rush in and extinguish fires involving electrical facilities can be deadly. Electrical Facility Fires should be treated with the same caution you would a delayed bomb. Here are 10 operational steps you can take when arriving on the scene at Electrical Facility Fires: pon arrival, perform your size-up - Don’t rush in; use extreme 1. U caution. Assume the area is energized. 2. Establish command - This should become a unified command with utility company personnel so you can coordinate with subject matter experts. 3. Locate the hazard and immediately notify the utility company Communicate the situation through your dispatcher to the utility company dispatcher. Report the exact location and thoroughly describe the damage. Wait for the utility company to respond before taking further action. 4. Establish a safety zone - Remain at least 500 to 1,000 feet away and upwind. Use caution tape to identify your safety and hot zones, and secure the area. Request law enforcement for pedestrian and traffic control. 5. Wear full personal protective equipment (PPE). 34 Jersey Firefighter | september/october 2012 6. Establish an ample supply of water and foam, and set up for foam operations – It has been recommend that we use a 2½-inch hose line, AFFF foam at 250 GPM. Foam operations technique for application at these types of incidents is as follows: • Place a blanket of foam around the base the equipment involved first. • Cover the ground surface under and around the equipment involved. • Use fog pattern to cool the vessel/facility and then attack main fire. 7. Evaluate and protect the exposures – If water is used to protect exposures, prevent runoff from impacting the energized area. 8. Wait for the utility company to de-energize the equipment before using water - Do not use water until a utility representative indicates it is safe. The application of water could trigger an explosion or possible electrocution if not de-energized. Use unmanned hose lines whenever possible. 9. Do not go beyond the established safety zone without guidance from the experts – Coordinate with the on scene utility company representative before entering or approaching a substation. They will know which areas are safe and which are not; you will not. 10. Do not leave the scene until the danger has been removed – These incidents require time, patience, and coordination. If there is no life hazard or exposure threat, there is no need to rush.
