PERA is the 21st largest public pension plan in the United States. All employees are eligible for this benefit. Perks and Benefits Benefits All employees are eligible for some employer provided or employee elected perks and benefits. All full-time and Part-time (hourly) employees who work at least 20 hours a week (4 hours per day; .5 FTE) are eligible to enroll in medical, dental, and vision coverage. It is a means of obtaining additional paid sick days Can help employees avoid or postpone going on unpaid medical leave DPS provides Long Term Disability to eligible employees at no cost 60% of monthly earnings after a 90 day waiting period Employee Assistance Program (EAP) EAP Services Include: Confidential counseling for a wide range of problems Unlimited over the phone support Up to 5 face-to-face sessions per issue per year Work-life solutions Financial information and support Legal support GuidanceResources is available to you 24 hours a day, seven days a week. There are two ways to access your GuidanceResources benefits: 1. Call 855-327-1377 to speak to a counseling professional who will listen to your concerns and can guide you to the appropriate services you require. 2. Visit GuidanceResources online at www.guidanceresources.com and enter your Organization Web ID, which is DPS. Your Health Your Way Medical, Dental, and Vision Insurance Healthcare and Dependent Flexible Spending Accounts Your Health Your Way Enrollment at DPS is active Employees working more than 30 hours a week must elect coverage or waive coverage All eligible employees have 30 days from their official start date to enroll in coverage Insurance is effective on the first day of the month following 30 days of employment Plans will be in effect through June 30, 2015 Employees classified as .75 FTE or higher (30 hours a week) will be enrolled automatically into the lowest cost DPS medical plan if no action is taken in the first 30 days The following presentation is a broad overview of benefits at DPS. Medical Insurance A Consumer Driven Health Plan (CDHP) offers lower premium costs than a PPO or an HMO, in exchange for a higher deductible and the elimination of co-pays DPS offers new employees a choice of four plans from two different providers: Kaiser and Cigna Both providers offer coverage at two levels, $2000 deductible and a $3500 deductible Kaiser $3500/$7000 CDHP Kaiser $2000/$400 0 CDHP Cigna $3500/$700 0 CDHP Cigna $2000/$400 0 CDHP Employee Only $282.06 $323.10 $283.39 $307.00 Employee and Spouse $662.84 $759.29 $665.39 $720.83 Employee and Children $558.48 $639.74 $561.21 $607.97 Family $922.34 $1056.54 $925.84 $1003.00 Coverage Level Health Savings Accounts (HSA) A Health Savings Account is designed to be coupled with a CDHP plan and allows an employee to set aside and use pre-tax dollars to pay for qualified medical, dental or vision expenses Funds are kept in a bank account in your name and administered by WageWorks If you change health DPS will contribute (HSA) account plans or jobs, you do not lose your balance $62.50 per month to your Health Savings Account You Are Eligible to Open a Health Savings Account (HSA) if you meet all of the criteria: You are enrolled in a DPS CDHP plan You or your spouse do not have a medical Flexible Spending Account (dependent care FSA is OK) Note: A limited FSA allows you to have an FSA specifically for vision and dental You are not enrolled in or covered by a non-qualifying or “low” deductible plan (HMO/PPO) You are not enrolled in Medicare or Tricare You can’t be claimed as a dependent on someone else’s tax return Flexible Spending Accounts (FSA) DPS offers three types of FSA accounts: Healthcare Flexible Spending Account (HC FSA) Dependent Care Flexible Spending Account (DC FSA) Qualified Transportation Benefits (both transit and parking benefits) To enroll in Commuter Benefits, please visit WageWorks www.wageworks.com. Dental The EPO Plan You must see a Delta Dental PPO Provider You pay co-pays for covered services You have no annual deductible or benefit maximum Locate a Delta Dental network provider at www.deltadentalco.com The Premier Plan You may see both in-network and out-of-network care You will pay less for your care if you see a dentist in the Delta PPO network Coverage Level EPO PPO + Premier Employee Only $28.86 $35.97 Employee and Spouse $58.23 $69.06 Employee and Children $71.32 $97.90 Family $100.67 $131.06 Vision The provider is Vision Service Plan (VSP) You can go in-network or out-of-network You will maximize your savings if you stay in-network Coverage Level VSP Premium Employee Only $7.09 Employee and Spouse $15.78 Employee and Children $16.28 Family $23.37 Locate a VSP network provider at www.vsp.com (Choice Network). Life Insurance DPS provides basic life and AD&D insurance to all benefits-eligible employees at no cost Active full time employees get 2x annual earnings to $300.000.00 Active part-time (hourly) employees get a life benefit of $2,500.00 Employees can purchase insurance for the: Employee: $10,000 increments up to $500,000 ($100,000 guarantee issue amount) Spouse: $10,000 increments up to $100,000 ($20,000 guarantee issue amount) Dependent Children: $2000 or $5000 with some age restrictions Can I Get More Help???
